Background: Justified supply of financial resources in health systems expresses the notion that the risks confronted by families due to the related costs to health affairs and not the risk of diseases to be distributed according to their expending abilities. A system with justified financial support is to be assured about financial security of all the individuals in a community. Progressive increase of health expenses in the country leads to increasing payments by the people. The first service package in Health Transformation Plan deals with reducing payments by the inpatients in the dependent hospitals to Health and Medical Sciences Ministry. In this study, we are going to deal with the realization of the mentioned plan.Methodology: This study was done in the first 9 months of commencing the plan, by investigating the income information by 5 dependent governmental hospitals to Medical Sciences University in the city of Isfahan, which included all the specialized fields.Findings: Regarding the inflation rates, the average pay out of pocket reduced by 43.17%, considering the mean rate of 2.18 and the standard deviation of 1.82. This could be due to paying the subsidies for health by the government.Conclusion: Analyzing the findings indicated that the government could have significantly reduced the pay out of pocket, and have an effective step in achieving its aims. However, analysis of the reasons for increasing the costs and the increasing trends in going to health centers seems to be essential.

Introduction: In any health system, various measures have been taken to prevent occurrence and spread of epidemic outbreaks. preplanning and preparing for any crisis is very important in pre-cirsis phase of management and control of any epidemic situation.in October 2012, after Eltor Cholera outbreak in Iraq’s Kurdistan province, Iran’s Ministry of Health and Medical Education (MOHME) have altered situation in western borders cities including Marivan. The objective of this study is to investigate the process of preparing for probable Cholera epidemic in Marivan city.Methods: This is a practical study that was implemented in six steps employing different methods for data gathering including situation analysis, document, literature review, and qualitative data analysis Methods.Results: Enforcement measures and organizational coordination were proposed to control the probable outbreak such as executive and educational measures, internal and external coordination with other organizations and obliging them for hygienic measures through job description. The following items identified as the weakness of measures in marivan covering: lack of definition of the incident command system (ICS) in the health center.Conclusion: Risk assessment approach can lead to valuable results. Lack of exercise to prepare for similar events will lead to inconsistency between responsible and involving teams. In Marivan planning and coordination with other organizations were performed through disaster control approach, meanwhile risk assessment approach can lead to valuable results.

This study is aimed to compare two treatment modalities in term of decreasing depression Symptoms in divorced women in city of Tehran.It is an experimental study. the Beck Depression Test was done in divorced women with depression symptoms, then they were randomly assigned to 2 groups, one group received only pharmacologic Agents and the other group received both medication and cognitive therapy. Independent t-test was done, and data was analyzed with SPSS software.The mean score of Depression prior to treatment between two groups has no significant difference After treatment with medication only, no statistically significant changes observed in mean score of depression in control group In mix pharmaco-cognitive treatment group mean score of depression was statistically significant after treatment Based on this study, it seems that pharmacotherapy alone has not satisfactory result for treatment of these group of patients and effect of combined therapy with medication and cognitive modalities should be considered in divorced women with depression.

Introduction: Oxygen tension is one of the most important stimuli in stem cell biology. In this study, we investigate the considerable influence of hypoxia on CX43 gene expression as one of the most important gap junction on the surface of mesenchymal stem cells.Methods: Mesenchymal stem cells were isolated from C57BL/6 mouse bone marrow and cultured in DMEM medium under low oxygen tension (1% pO2). Hypoxic conditions were applied in time table as 4, 8, 16, 24 and 48 hours and reoygenation immediately was applied for 8 hours. Then, RNA samples were isolated and cDNAs were constracted for gene expression quantification using Real-Time PCR technique. Finally, the data were analyzed by statistical methods.Results: the comparative threshold cycle method (DDCT, ratio=2–DDCT) and statistical analysis showed that connexin43 gene expression had been increased in 4, 16 and 48 hours after applying hypoxic (8.61, 1.00 and 2.86, respectively) and was decreased after 8 and 24 hours hypoxic conditions (0.03 and 0.14) in comparision with normoxic cells (21% pO2). As well as the rate at Re-oxygenation for periods of 4, 8, 16, 24, and 48 h, is 3.9, 0.59, 3.16, 1.31 and 4.27 respectively.Conclusion: the alteration in oxygen tension effects on the extent of CX43 gene expression. Hypoxia and Re-oxygenation conditions resulted in the up and down-regulation of CX43 gene expression, respectively.

Introduction: For many years surgical incisions were done by scalpel. Nowadays this method has been replaced with Electrocautery. Each of these methods has their disadvantages and benefits. The purpose of this study is to compare these two methods in midline laparotomy surgeries.Methods: From 2012 until 2013, One hundred patients enter the study, underwent midline laparotomy and were divided into two Groups. In one group scalpel and in the other group Electrocautery were used. The speed of surgical resection, bleeding, postoperative pain and other wound complications were compared between two groups.Results: In this study, the mean and standard deviation speed was calculated based on centimeter per minutes. In Electracautery group result was 10.64±3.052 and in scalpel group was 8.26±3.61. The average And standard deviation of the bleeding based on number of bloody used gauzes in Electracautery group was 0.97±0.37 and in other group was 2.26±1.25.Wound complications among the patients with Electrocauter usage were 8.2% versus 11.8% in the other group.Conclusion: Amount of pain and wound complication were similar in two groups. However, using Electrocautery device increases the speed of operation and decreases bleeding amount. Therefore, surgical scalpel can be replaced by Electrocauter.

Introduction: Prenatal care is a mainstay of health care system to keep mother and her new born child healthy If for any reason it’s fundamental rules does not follow properly, many prenatal complications like Preterm labor, low birth weight, maternal and neonatal mortality will increase. The aim of this study is to determine adaptation rate of presented prenatal care in first visit and first trimester in city of Ahvaz and compare it to nationwide standards.Methods: In this Descriptive Study, 20 health care center in Ahvaz was selected by simple sampling method, Then Researchers attended in these selected centers and observe routine prenatal health cares that was performed in 196 referred pregnant women. Data was analyzed by SPSS software version 18.Results: Results showed that %88.8 of presented prenatal care at first visit had average adaptation, and %11.2 had good adaptation. furthermore, %70.4 of presented prenatal care at first trimester had poor adaptation and %29.6 of them had average adaptation in comparison to national standards.Conclusion: Presented prenatal care in first trimester in not compatible with preset national’s guidelines and need to Reevaluated again???

The word eczema refers to a broad range of conditions that begin as spongiotic dermatitis and may progress to a lichenified stage.Histologically, the hallmark of all eczematous eruptions is a serous exudate between cells of the epidermis (spongiosis), with an underlying dermal perivascular lymphoid infiltrate and exocytosis (lymphocytes noted within spongiotic foci in the epidermis).The efficacy of some of the treatments depends on the morphology of the eruption and the diagnostic classification. Protection, barrier repair and topical medication are mainstay of eczema treatment.The systemic agents used to treat severe chronic dermatitis.
