Abstract: 

The purpose of this study was to assess the facilitating impact of craniosacral therapy on inhibition of hypertonicity of the lower limb in children with dipelgia spastic cerebral palsy. 100chilren with cerebral palsy were studied and 36 children selected, aged 3 to 8 years, were randomly assigned to a control and an experimental group. Muscle tone was assessed using modified ashworth scale, passive range of motion by goniometer, neurodevelopment level by bobath scale All children were Pre-Post-tested with in an interval of three months. A further finding of the analyses revealed that: -Significant reduction was observed in hypertonicity of the hip adductors and the ankle planter flexors. -Significant increase was observed in passive rang of motion of the hip abduction and the ankle plantar dorsiflexion. -There was found no significant difference of reduction in hypertonicity of the knee flexors, of increase in passive range of motion of the knee extensor, of improvement neurodevelopmental level. -Meaningful relationship was observed between reduction hypertonicity of the hip adductors and improvement of the neurodevelopmental level. It should be mentioned that in all above cases was set at 5%. Implications for clinical O.T. are mentioned.

Abstract: 

Introduction: The common sexual complains in patients with coronary heart disease and post myocardial infarction are decrease in libido, impotence, and premature or delay ejaculation. Cardiac rehabilitation could decrease many of the psychological features of myocardial infarction and also increase exercise capacity of patients. Rehabilitation may also improve sexual disturbances in these patients directly or indirectly. This study is a clinical trial that evaluate the effect of rehabilitation on sexual problem of post MI patients. Methods: 60 patients took part in this study. All of them were men aged between 35 and 65. All patients had myocardial infarction one month age.30 patients were referred for cardiac rehabilitation (Case), and 30peopel were patients who were not recommended to take part in rehabilitation because their physician did not believe on rehabilitation. Questioner for anxiety, depression, impotency libido and premature ejaculation were evaluated by before and after study period. The cardiac rehabilitation composed of 24 sessions. Each session consisted of one hour of aerobic exercise (10min warm up, 10min cool down and 40 min isotonic exercise). Results: after cardiac rehabilitation the scores for anxiety, depression premature ejaculation and impotency were decreased and the scores of libido were increased. In both case and control groups, the changes were significant by paired t test p<0.05. The differences between case and control were significant for depression, libido and impotency by independent t test p<0.05. The differences between case and control were significance for depression, libido and impotency by independent t test.   Discussion: cardiac rehabilitation could improve sexual problems in post myocardial infarction patients directly and indirectly by effect on psychological characteristics

Abstract: 

The idiopathic long Q-T syndrome is an infrequently occurring disorder in which affected individuals have an unusual electrocardiographic repolarization abnormality presenting as syncope loss of consciousness related to ventricular tachycardia or fibrillation. Congential long Q-T prolongation can be associated with congenital deafness in an autosomal recessive manner (Jervell and Lange-Nielsen syndrome). The purpose of this stuff was to screen this electrocardiograpic abnormality in deaf-mute school children in our population, which has not been yet performed. Of 1190 patients with hearing loss, 779 had congenital sensorineural deafness (CSD), aged 13±3.8 years (4-24), 63%female and 37%male. The family history of deafness was as follows: Cardiac axis deviation was found in 56(7%)patients. Electrical conduction abnormalities were found in 12(15%) patients, Wolff-Parkinson-White syndrome, sinus bradycardia, and sinus aryhmia were found in 2(0.25%), 4(0.5%), and 3(0.38%) patients, respectively. The Q-T interval,and Q-Tc duration were 312.6±28.9ms. (200-500 ms. Median 320ms), and 383.6±29.3ms (232-527ms, median 413ms), respectively. Long Q-Tsyndrome was found in 4(0.5%) patients (3F, and 1M). Two of these 4 patients had total deafness and 2 had profound hearing loss. None of the patients with mild deafness had Q-T prolongation. Only one of these patients were symptomatic, and had been treated as a case of epilepsy for several years. This data supports the presence of long Q-T syndrome in patients with sensorineural hearing loss in our population, so routine electrocardiographic screening of anyone with congenital deafness is warranted to prevent subsequent associated cardiac arrhythmias and sudden cardiac death.

Abstract: 

A common from of therapy in physicaltherapy is the strengthening of muscles. As regards the probability of increasing muscular strength by electrical stimulation, we have treated the effect of that on muscle strength and endurance. 34 healthy male students of rehabilitation sciences college (29-31 old) were subjected to two weeks of interferential current stimulation program, consisting of 4 session per week, each of 15 minutes duration. The stimulation was a 40-60 Hz beat with 25 second on and 35 second rest time. We were tested all of subjects before and after the program. Assessment parameters were: 1) Maximum circumference of right arm at rest (cm) 2) Max. circumference at max. Isomeric contraction 3) Max time (sec) that he can hold a constant weight. 4) Integral of interference pattern at Max. contraction (µv.s) 5) Amplitude of the M. response (mAmp) 6) Duration of the M. response (ms.) T-Pair test employed to data processing, confirm significant changes on 1) rest arm circumference 2) contracted arm circumference. 3) Muscle Endurance. 4) IEMG 5) M. response Dur. In conclusion we can develop muscle strength and endurance by specific program mentioned above.

Abstract: 

Multiple sclerosis (M.S.) is a chronic disease which most commonly involves females. Prevalence of disease begins from adolescent, gradually increases until 35 years old and then decreases. It’s geographical distribution is non- hemogen and prevalence changes. from5/00000 to 250/100000. Iran is a low incident region with prevalence of 5/00000 or at least 3000 patients. This research has been done on 187 patients who admitted on Tehran University centers in 1998. Results show that 63.3% of patients were female and must of them were married. The average age at beginning was 28y. Manifestation of disease were: Extremity weakness (44/2%), vision cloudiness and diplopia (33/7%) and sensory disturbances(32/2%).

Abstract: 

Main Objectives of research were: 1) developing a Lipreading test for Persian adults, 2) Standardization of the test in a group with normal hearing & 3) Evaluation of it in a group of adults with acquires hearing impairment. First according to "Denver Quick Test o lipreading" (Alpiner etal, 1970), A lipreading test, composed of 20 simple and common sentences was developed. Then the test was performed on 105 normally hearing and vision adults with male and female talkers. Also 48 adults with acquired hearing impairment were evaluated twice, lipreading alone and lipreading +residual hearing. According to the results, "Sara Lipreading Test" is a Valid and Reliable Test for assessing lipreading ability of Persian adults.

Abstract: 

Two point discrimination (2PD) test is a sensory functional test that is both available and valid. This test is used in hand injuries and neurological examination, whose perception takes place in the sensory cortical region. In this study 2PD test was evaluated in 240 normal and blind boys and girls in the range of 10-20 years in Tehran. (cross-sectional descriptive analysis). This Test was evaluated on right and left hands by on instrument results. -The 2PD value of the normal subjects were greater than that of the blind subjects (specially At age 10-th) -The boys 2PDvalue were greater than girls. -At age 20 years, the value of 2PD was greater than that of 10-th years. -The 2PD value in dominant hands was greater than that of the undominant hands. Abstract: 

Multiple sclerosis (M.S.) is a chronic disease of central nervous system (C.N.S) which locally demolishes myelin sheaths. M.S. is one of the most important debilitating factors in the youth and middle age. It mostly common between 20-30 years of age. The incidence in the age below the 10 and above the 60 years old is rare. It has been estimated nearly 1/1milion have M.S around the world. Since M.S affects different parts of C.N.S, so the symptoms are various. Clinical development differs from the benign to rapidly developing disease. It is mostly seen with attack, and remission. First attack usually recovers but by repeating attack, neurological defects permanently remain. Etiology of M.S is unknown. Researches have showed the responsibility of autoimmune mechanism, viral infection and genetic in M.S Environmental factors in onset and frequency of attacks can be effective. Incidence of M.S in females is more than males. Geographically M.S is seen much more in areas far from equator. Hence M.S patients have wide spread symptoms of involvement in nervous system, is necessary during drug therapy physiotherapy and rehabilitation. These defects mostly occur in the from of cognitive and visual disorder, disarthery, dysphasia weakness spastisity, sensory, cerebellum and urinary and defecation disorders.In this paper I underscore the rehabilitation aspects of M.S patients.

Abstract: 

Psychological factors imply their negative effects on neurologic patients in two ways. Directly, through pathogenic processes of central nervous system, and indirectly, through maladaptive toward disability. Depression is a common finding in many neurologic diseases and may interfere with rehabilitation process. Correcting patient’s attitude toward disability, reducing environmental stresses and potentiating social support network along with antidepressant drugs often help these patients remain in rehabilitation programme. Chronic fatigue decreases the activity levels of patients and the possibility for them to achieve set goals of rehabilitation in proper times. Support, reassurance and some times antidepressant drugs may help. Convertion reactions often accompany chronic illnesses and disabilities and make evaluation and diagnosis difficult. Application of behavioral Techniques may lead to better results. Altered self image is a prominent feature in patients with spinal cord injury. Counselling and Free discussion on altered self image and sexual problems are essential and other forms of sexual expression must be taught to these patients. Chronic pain is a disabling condition. Usually physical findings are minor and it seems psychological factors play a more important role in causing it. Antidepressant drugs are effective in most cases. Intensive physical and occupational therapy must be avoided. Excessive emotionality is a consequence of executive dysfunction arising from frontal lobe injury and mostly seen in stroke, brain injured, and demented patients, Impulsivity and disinhibition may lead to agressive behavior and socially inappropriate forms of sexual expression. Judicious administration of psychotropic drugs, behavioral techniques and short-term hospitalization may be helpful. Altered cognition is a common feature of many pathological conditions of brain. Attention deficit, slowed information processing disturbed communication, and impaired memory all make patients unfit for participation in rehabilitation programme. Neuropsychological evaluation helps us to find out the type and the extent of existing impairments. These data can be used to educate therapists and give them guidelines for better interactions with the patients.
