Abstract: 

Background and Aims Diabetes mellitus as a chronic disease is a source of stress for family members, especially if changes due to complications of the disease cause conflicts in family roles and duties. The purpose of this study was to determine the effect of family-based empowerment program on perceived threats and self-efficacy of families of diabetic elderlies at risk of falling. Methods This is a clinical trial study performed on 60 elderly active members of the elderly with type 2 diabetes mellitus who were referred to Endocrinology Subspecialty Center in Bojnord City, Iran in 2016. The study samples were selected by convenience sampling method and after matching their demographic characteristics, they were randomly divided into control group (n=30) and experimental group (n=30) (simply using even and odd numbers for each group). Family-based empowerment intervention was administered to the experimental group for eight 45-minute sessions. The data collection tool was a researcher-made threat and self-efficacy questionnaire. Data were analyzed using SPSS version 18 and analyzed at a significant level of P<0. 05. Results The mean (SD) scores of the perceived threat dimension in the experimental group before the intervention were 55. 03(7. 26) for its severity and 55. 16(7. 56) for its sensitivity. Also, The mean (SD) self-efficacy score of the experimental was 56. 36(6. 92) before the intervention. After the intervention in the experimental group, the severity score increased to 64. 4(23. 71) and sensitivity to 64. 60(4. 33) and self-efficacy to 63. 90(4. 70) (P<0. 001). But in the control group, this difference was not significant (P>0. 05). Conclusion Based on the results, implementing a family-based empowerment program has increased knowledge, improved attitudes and self-efficacy in the families diabetic elderly, helped them to care and adopt preventive measures. Thus, we suggest interventions like family-based empowerment model for prevention; improving and promoting the quality of care; and encouraging independence of the elderly.

Abstract: 

Background and Aims Mental health and quality of life of the patients with thalassemia due to multiple physical, psychological, economic and social problems of this chronic disease is low. This study aimed to investigate the effect of Orem self-care on the mental health of patients with thalassemia major. Methods This study was quasi-experimental with pretest posttest design. The study population included all patients with beta thalassemia living in Dezful City, Iran in 2015. Of them, 80 people after obtaining informed consent, were selected by convenience sampling method and randomly divided into two groups of 40 people in the intervention and control group. The relevant data were collected through the SF-36 questionnaire and the training package based on Orem self-care framework. The obtained data were analyzed using SPSS-22 and GEE (Generalized Estimating Equation) models. Results The results showed a significant difference in the level of mental health in the pretest and posttest among patients with thalassemia (P<0. 005). The mean (SD) mental health score in the intervention group before the start of the self-care model was 74. 32(3. 23) and three months after the intervention was 79. 08(2. 34). This proves the positive effect of education on self-care pattern of Oram on the mental health of patients with thalassemia. Conclusion With the proper training of Orem self-care to patients with beta-thalassemia major, their mental health can be elevated and reduce the psychological burden of the disease. Therefore, this method can be more effective if it is used as a simple and low-cost supplemental non-pharmacologic supplement for patients with thalassemia.

Abstract: 

Background and Aims Exercise plays an important role in controlling the health of patients with breast cancer. This study aimed to review the effect of 10 weeks Pilates training on resistin and adiponectin of women with breast cancer. Methods Statistical population of this quasi-experimental study were women with breast cancer who referred to health centers and private clinics in Shiraz City, Iran. Of them 24 women were selected as study sample. Subjects were randomly divided into groups of Pilates training and control. The training group performed Pilates for 10 weeks, 3 sessions per week and 60 minutes each session, but the control group did just their daily activities. Blood samples were taken before the study and 10 weeks later from all subjects. The data were analyzed by Kolmogorov-Smirnov, dependent t test and two-way ANOVA tests (P≤ 0. 05). Results Ten weeks Pilates training has significant effect on the reduction of resistin and adiponectin in women with breast cancer (P≤ 0. 05). Also, Pilates training has no significant effect on fasting blood glucose, body mass index, and waist to hip ratio of women with breast cancer (P≤ 0. 05). Conclusion Patients with breast cancer can benefit from Pilates training along with other routine therapies. Further advances in the study of adipokines and their response to exercise may lead to improved conditions for patients with breast cancer.

Abstract: 

Background and Aims Cardiovascular disease as the most common cause of death in the world; is closely related to lifestyle and related factors. Islamic lifestyle by taking the material and spiritual aspects can play an important role in improving patients. Therefore, this study aimed to evaluate the relation between health promotion behaviors and Islamic lifestyle in patients with acute coronary syndrome. Methods This cross-sectional study was conducted on 228 patients with acute coronary syndrome. The subjects were collected by convenience sampling method. Data were collected through questionnaires including demographic data and disease and health promotion behavior. The SPSS V. 18 was used for performing descriptive and inferential statistics. Results There was a direct and significant correlation between total score of health promotion behavior lifestyle and total score of Islamic lifestyle (r=0. 567, P<0. 001). Also there was a significant relationship between total score of Islamic lifestyle and all aspects of health promotion behavior lifestyle except beliefs, health and security (P<0. 001). Conclusion Due to the direct relation between Islamic lifestyle and health promotion behavior lifestyle, special attention of the treatment team in general and nursing intervention in particular is necessary towards the ischemic heart disease patient’ s lifestyle.

Abstract: 

Background and Aims Couple conflict is an inevitable event, because no two people are similar in interests, feelings, choices and behaviors. At the same time, marital conflict can interfere with interpersonal relationships and cause family problems. The main goal of the present study was to determine the difference between the early maladaptive schemas and emotional collapse and happiness in women with marital conflicts and women without conflict in Rudsar City, Iran. Methods The present study was an analytical descriptive comparative study. Using Krejci Morgan‘ s Table, 103 women with marital conflict covered by the centers of the Rudsar Relief Committee in 2015 who were referred to the family courts of this city were selected by convenience sampling and compared with another 103 women without marital conflict. The data were collected through inferential marital conflict questionnaire, alexithymia scale of Toronto, happiness scale questionnaire, and initial inappropriate schema questionnaire. Data were analyzed using Lavin, Box’ s M and multivariate analysis of variance in SPSS version 18. Results The results showed that the variables between the two groups alexithymia, happiness and there schema (P<0. 01). Conclusion According to the results of the research, happiness can reduce marital conflict in couples. Also, due to the difference in study groups regarding the emotional variables and inappropriate schemas, it is possible to reduce marital conflicts by educating couples in this regard.

Abstract: 

Background and Aims Caring for patients with epilepsy, creates many challenges for their family caregivers. The implementation of family-based interventions can facilitate the caring duty of caregivers and increase the quality of care for patients. This study aimed to investigate the effect of family-based care program on care burden in family caregivers of patients with epilepsy. Methods This clinical trial study was conducted in Ayatollah Kashani and Hajar hospitals in Shahrekord City, Iran for 7 months in 2016. Samples were selected by convenience sampling method and randomly assigned in the intervention (n=50) and control (n=50) groups. For the intervention group, a family-centered care program was implemented including self-efficacy, self-esteem and evaluation, through education, support, problem solving and group discussion during four sessions. Data were collected by using Persian version of Zarit's burden of care scale from the intervention and control groups in three stages; before, immediately, and two months after the intervention. Data were analyzed by descriptive and analytical t-test and ANOVA with repeated measurement in the SPSS. Results The mean score of burden of care in the intervention and control groups were 37. 42 and 34, respectively before the intervention (P<0. 179). The mean score of care burden in the intervention and control groups immediately after the intervention was 21. 36 and 28. 7, respectively and two months after the intervention was 15. 78 and 27. 92 respectively. Conclusion The results showed that a simple, low-cost and feasibility program of family-based care has significantly reduced the care burden of caregivers of epilepsy patients. This family centered program can help to play a better role for family caregivers with physical and psychological stress.

Abstract: 

Background and Aims Today, 60% to 85% of chronic patients are cared by home caregivers and prolong care causes bitter experiences for the caregivers. This study aimed to assess the burden of care for patients undergoing hemodialysis from the point of family caregivers’ views. Methods In this qualitative study, we used content analysis approach for data collection and analysis. The study participants were 17 family caregivers selected through purposive sampling method from four medical education centers affiliated to Ahvaz Jundishapur University of Medical Sciences, Iran. The relevant data were collected by quasi-structured interviews. Results The following categories were emerged from interviews as follow: "care challenges", "the nature of long-term care", and ‘ care in the shade". The categories share the main theme of "victim of the situation" experienced by the family caregivers of the patients undergoing hemodialysis. Conclusion Monitoring the mental status of the caregivers, which reflects their experiences and attitudes is an appropriate tool for assessing the physical, mental, social, and functional aspects of care burden on caregivers and reflects their needs in the care process.

Abstract: 

Background and Aims Chronic kidney disease is a progressive disease with poor prognosis. Chronic illness and prolonged need for dialysis may lead to mental disorders such as anxiety. The current study aimed to assess the effect of breathing anxiety level of patients undergoing dialysis. Methods This study was a clinical trial. A total of 100 patients undergoing hemodialysis were selected by convenience sampling method. Then the samples were randomly assigned to experimental and control group. The experimental group received breathing intervention for eight sessions in a month under the guidance of a nurse and the control group received routine care. The research instrument was a tow-part questionnaire of demographic data and Spielberger’ s State-Trait Anxiety questionnaire. Data were analyzed by descriptive statistics (frequency, percentage, mean and standard deviation), and inferential statistics (independent t test, repeated measures and Chi-square). We used SPSS version 18 and P<0. 05 was considered as significant level. Results The mean scores of the situational anxiety before and after the intervention in the control and experimental group showed a significant difference. This difference in the control group was lower at all times compared to the experimental group (P≤ 0. 000). Conclusion Breathing intervention was effective in reducing situational anxiety in patients undergoing hemodialysis.
