Abstract:

Introduction: Chronic renal failure is an advanced and irreversible impairment of renal function, which affects 2 to 3 percent of the world's population today. The number of patients with kidney transplant failure is estimated to be about 25, 000, according to a report from the Center for the Transplantation of Specific Diseases. Annually, about 4, 000 new patients are added to this population, with dialysis as the most commonly used treatment. Nearly 50% of these kidney patients are hemodialysis patients in Iran. Hemodialysis patients have many complications due to the chronic nature of the disease, which can decrease their self-efficacy and quality of life. Improving and supporting these patients requires active participation of the family. Therefore, the aim of this study was to improve the family-centered empowerment of hemodialysis patients in Golestan province. Methods: This experimental study was carried out on 70 hemodialysis patients in Gonbad-e-Kavoos city who were randomly divided into two groups of control and control after being matched for dialysis, education, age and sex. The family-centered empowerment model in the test group was performed in 6 sessions of 60-90 minutes. Data gathering tool was Sherer general self-efficacy questionnaire. Data analysis was done by SPSS21 software using independent t-test, paired T-test and Analysis of variance. Results: Independent t-test between the intervention and control groups did not show any significant difference. (P = 0. 97), but independent t after intervention showed significant difference between the two groups (P < 0. 01). Conclusions: Implementation of family-centered empowerment model with the participation of family members will increase self-care and promote lifestyle in hemodialysis patients. It is recommended that the patient receive the health care in the family home. Due to the low cost and safety and the impact of the family-centered empowerment model, it is recommended to use this model on other variables of nursing care or other chronic patients.

Abstract:

Introduction: Adolescence who have self-harm behaviors (SHS) or tendency to suicide experience different psychopathological problems. Two potential critical risk factors-insecure attachment and especially borderline personality organization-have been underexplored in the literature despite the fact that both of these structures are central to understanding these problematic behaviors. The objective of this research was to explore the association between adolescent insecure attachment and borderline personality organization with self-harm behaviors and tendency to suicide. Methods: This study was a correlation design. Sample group which was selected through multi-stage cluster sampling included 700 students from high schools of Tehran, district 16 in Academic year 2017-2018. All participants were asked to complete questionnaire pack including: Self-Harm behavior Inventory, Multi-Attitude Suicide Tendency Scale, Inventory of parent and peer attachment and Personality Organization Inventory. Data was analyzed by Pearson correlation method and using the SPSS-21 software. Results: The results illustrated a significant correlation between insecure attachment style and borderline personality organization with self-harm behaviors (SHB) and tendency to suicide. Conclusions: Findings of the current study are consistent with previous studies and theoretical stance indicating the predictability of self-harm behaviors / suicide tendency indices from quality of attachment style and personality organization in adolescence.

Abstract:

Introduction: Regarding the prevalence of psychological disorders in multiple sclerosis patients, some reports have considered the efficacy of a resilient model prior to previous treatments. Therefore, the aim of this study was to investigate the effectiveness of the resiliency model on improving the quality of life components in patients with multiple sclerosis followed by three months. Methods: This study was an experimental study in which pre-test-post-test design with control group was used. The statistical population of the study was 631 people. The sample consisted of 40 MS patients in Tabriz in 2018. First of all, 220 patients with multiple sclerosis, a quality of life test, were selected. Then, the subjects from the above-mentioned society, who were classified as unsatisfactory according to the cutoff criteria, were selected (82 subjects). 20 subjects were assigned to the experimental group and 20 controls were randomly selected. The intervention based on resilient training was performed in 12 sessions of 90 minutes, one session per week. After the end of the sessions, quality of life test for posttest was performed again in two groups and after three months after the end of the intervention, a post-test was performed. Data were analyzed using covariance analysis. Results: All subjects were male and the mean age of the subjects in the experimental group was 42. 35 ± 5. 84 and the mean age of the subjects in the control group was 42. 35 ± 5. 84. Data analysis showed that there was a significant difference between the mean scores of post-test in the experimental and control groups (P < 0. 01), so that the intervention improved the components of quality of life in the experimental group. Also, after a three month follow up, the model of resilience has had a lasting effect on the quality of life of MS patients. Conclusions: Considering the positive effect of resilient education, the use of this training in planning mental health interventions, especially in patients with multiple sclerosis, is recommended.

Abstract:

Introduction: In an emotional divorce with a lack of trust, respect, and affection for each other, couples try to persecute, fail, and degrade self-esteem. And each one is in order to find a reason to prove the defect and exclusion of another. The present study was conducted to determine the pervalnce and components of emotional divorce in married nurses in Tehran hospitals. Methods: The research methoed was a descriptive and corrlational study. A total of 150 married nurses from Tehran hospitals were selected through multi-stage and cluster random sampling by 2016. To collect the data, Gottman’ s emotional divorce scale and researcher-made emotional divorce questionnaire in 60 items and 11 components based on Gatman's divorce views (1997), Kessler's divorce (1975), Sternberg Love Triangle (1988) and the quality of marital relationships Marx (1989) was used. After analyzing the questionnaires, the data were analyzed with factor analysis by SPP20 software. Results: Data analysis showed that 11 factors: Criticism, insult, defensive behavior, non-cooperation, frustration, fault, erosion, mourning, foolish love, null love and separation composition can measure the variables of emotional divorce (P < 0. 05). And the physical separation component does not have the ability to measure emotional divorce (P > 0. 05). Also, the amount of emotional divorce in the sample group was 39. 3%. Conclusions: The results showed that, in fact, the significant reduction of positive interactions and satisfaction in the relationship between the two sides provides the basis for emotional divorce. This phenomenon will lead couples to divorce and formal separation if they are not treated. According to the results of this study and identifying the components of emotional divorce, it is hoped that by holding marriage specialized, Couple Therapy Courses, Pre-marriage counseling workshops, it would be desirable to prevent the occurrence of this phenomenon.
