Abstract: 

Background & Objective: The effects of acute and chronic exposures to opiate drugs on anxiety process are controversial. Acute morphine injection showed the beneficial effects on anxiety. Morphine withdrawal induced severe anxiety response in morphine dependence rats. Whereas, the effects of chronic administrations of morphine on anxiety process are less studied. Furthermore, this study was designed to assess the role of morphine dependence on the level of anxiety in Rat.Materials & Methods: In this experimental study, Twenty male Wistar rats (250-300 gr) were made dependent by chronic administration of morphine in drinking water that lasted at least 21 days. Control groups received only sucrose in their water. This study utilized the elevated plus-maze model to evaluate anxiogenic-like behavior in rats. Four fundamental behavior patterns were recorded for 5 minutes: the time spent on open arms, the number of entries into open arms, stretched-attend posture and defecation. Immediately after test, the locomotor activity of each animal was tested by using an automated activity monitor system. The data were analyzed by independent t-test and two-way analysis of variance (ANOVA).Results: Finding indicated that the time spent on open arms and the numbers of entries into open arms were significantly shorter in morphine dependence group than control group (P<0.05). Also, the numbers of stretched-attend posture and defecation were significantly higher in morphine group (P<0.05). Whereas, there were no significant differences between groups in locomotor activity.Conclusion: This study showed that dependent rats may rapidly predispose anxiogenic- like effects in stressful conditions and without the effect on motor activity.

Abstract: 

Background & Objective: Neural tube defects, growth retardation and nail hypoplasia are most common features of teratogenic effects of carbamazepine. This study was done to determine the effects of carbomazepine on eye development in Mice fetuses.Materials & Methods: In this experimental study 40 BALB/c pregnant Mice were divided into four groups. Experimental groups I and II received 15 mg/kg daily 6-15 GD (gestational days) and 30 mg/kg daily 6-15 GD intraperitoneal of carbamazepine, respectively. All drugs recolved in Tween20. Two control groups received normal saline or Tween 20. Dams were dissected on GD18 and embryos were collected. After observation of eye malformation in fetuses, we employed routine histological processes to  stain the samples and also skeletal staining was performed.Results: Calvaria deformations, finger anomalies, brachygnathia and short tail in experimental groups I and II were 7% and 10.8%, 13.3% and 16.6%, 7.8% and 11.7%, 10.2% and 9.2% respectively. Ten of fetuses (8.6%) in experimental group I and nine of fetuses (7.5%) in the experimental group II had eye malformations. Premature opening of one or both eyes with mild to severe exophthalmos occurred in both of the experimental groups. Also, histological examination showed deformed lens, retinal folds with undeveloped layers, corneal fold with absence of surface epithelium.Conclusion: This study revealed that administration of carbamazepine during embryunic period can induce eye malformations in Mice fetuses.

Abstract: 

Background & Objective: In addition to genetic and hormonal effect, various environmental influence on hair growth. For instance, stress can release epinephrine in the blood circulation, and epinephrine can effect on various tissues, especially on skin and hair tissue. Many investigators try to find effective drugs for the hair growth. Because most of these drugs are vasodilator agents, we design present study to determine the local effects of epinephrine administration as a vasoconstrictor agent on growth of hair.Materials & Methods: In this experimental study, 45 Wistar male Rats were divided in three groups, which taking 1.5ml/kg epinephrine, 1ml/kg epinephrine two times/daily for two months and control group which reciving normal salin with same valume for two months respectively. Following observing the macroscopically findings and segregation of the skin, the slide were prepared and the number of hair follicles were examinated.Results: The epinephrine causes degeneration of hair folicule, sebasus gland, and decreasing number of hair folicule in comping with control group (P<0.05).Conclusion: This study showed that the subcutaneous adminstration of epinephrine cause the hair lesions, and this significantly is related directly with the dosage of epinephrine.

Abstract: 

Background & Objective: On the bases of the antiinflammatory, antimicrobial and antioxidant effects of matricaria chamomilla (chamomile) extract. This study was design to determine the effect of topical hydroalcoholic chamomile extract on burn wound healing in albino Rats.Materials & Methods: In this experimental study 30 male albino Rats were randomly divided into 3 equal groups, as sham, control (olive oil) and treatment. Second degree burning was induced in 20% of whole surface area of animal body by diving the back of animal into boiling water for 8 seconds. Animals of control group received no treatment. Animals of olive oil and treatment groups were treated topically by olive oil and hydroalcoholic extract dissolved in olive oil twice a day respectively from the first day of burn induction to complete wound healing.Results: The results showed that there was not significant difference between olive oil and sham groups in all days of experiments.  Also there was significant difference in wound healing (p<0.05) between olive oil and treatment groups (62.96±2.70 versus 74.77±2.50) from the day of 20 to the end of experiments.Conclusion: This study showed that the hydroalcoholic chamomile extract has a treatment effect on burn wound healing in animal model.

Abstract: 

Background & Objective: The breast cancer is the most common malignancy in women. MDM2 expression is determined in some carcinomas, sarcomas, leukemias,and breast cancer. The purpose of this study was to determine the expression of MDM2 gene in primary breast cancer and it’s relationship with grade, stage and axillary lymph node involvement.Materials & Methods: the study was the cross-sectional one, that was performed on 75 samples of patients with breast cancer admitted in Mostafa Khomeini hospital Tehran, Iran (2000-05). After preparing the samples, a tissue section from each samples was obtained. One of the tumoral sections and one of the lymph node sections were stained by H&E. We determined the type of the tumor, the number of lymph nodes, the stage and the grade of the tumor. We studied MDM2 with polyclonal antibody by IHC.Results: The mean±SD of patients were 54±12.5 years old. 69.3% of patients had lymphoid involvement. 32% of samples were positive for MDM2. 58.7% of samples were in stage II and the most of patients (42.7%) were in grade III. In this study, there was not any relationship between MDM2 and tumor grade, but there was a relationship between stage of tumor and lymph node involvement with MDM2 gene (P<0.05).Conclusion: This study showed that MDM2 gene is a prognostic factor in breast cancer.

Abstract: 

Background & Objective: More than 50% of patients with hypertension will have end organ damage (such as: CHF, retinopathy, CVA or renal failure).Left ventricular hypertrophy (LVH) is an independent risk factor for death and cardiovascular disease .Both renal dysfunction and LVH are signs of end organ damage so we carried out this study to evaluate the correlation between LVH in echocardiography and kidney function in patients with essential hypertension.Materials & Methods: This descriptive analytic study was carried out on 102 patients whome reffered to 5 Azar hospital in Gorgan-Iran, suffering from essential hypertension and had proved LVH in echocardiography with no other disease during 2005-6. Kidney and urinary tract assessment such as serum BUN, Creatinine level and kidney sonography was done on subjects. Left ventricular hypertrophy was defined as a left ventricular mass index (LVMI)>=135g/m2 in men and>=110g/m2 in women. Renal function was estimated by means of the serum Creatinine level and proteinuria and sonographic findings of kidney and urinary tract. Data was analysed by Spss-11.5 and Chi-Squre test.Results: The average age of patients were 60±8.8. 58%, 36% and 7% of patients had mild, moderate and sever LVH respectivly. The means of Creatinine and BUN level were 1.09±1.12 mg/dl and 20.6±10.76 mg/dl had direct correlation with kidney dysfunction (P<0.05). There was a significant relationship between LVH severity and presentse of Kidney scar in sonography (P<0.05).Conclusion: This study showed that a very large fraction of the subjects with essential hypertension have kidney problems. Therefore, serum BUN and Creatinine level and sonographic scar of kidney can be reliable markers for assessing and controlling target organ damages that are induced by Essential hypertension.

Abstract: 

Background & Objective: ADHD is the most common mental disorder in children. Awareness of students' mental health has an important role in programming for decrementing their disorders and, improving their mental health and also preventing the complications of their decreased mental health. The aim of this study was to determine the prevalence of ADHD and some of the affecting factors among school children in Nishaboor localed in the North–East of Iran.Materials & Methods: This descriptive study was done on school children in Nishaboor during 2006. Subjectes selected by stratified cluster sampling. Data collection was done using the Conner's Parent and Teacher Scale questionnaire, and each student's questionnaires were completed by both the teacher and the parents. Data analysis was performed regarding to the mean scores of teachers and parents. The score 15 was chosen as the probable indicator for ADHD.Results: 722 students (79.6%) were involved this study. Prevalence of ADHD was 12.5%, CI95%: 10–14.8%. There was no significant difference in gender distribution. The prevalence of ADHD had a significant rise with age (P<0.05). The highest prevalence was in spring born and the lowest was in the summer born school children (P<0.05). The prevalence of ADHD had a significant relationship with father's education and was lower in students whose father had higher educations (P<0.05). Variables such as type of delivery, pariety, mother's education had no significant relationship with the prevalence of ADHD.Conclusion: In this study the prevalence of ADHD is higher than similar studies. Risk groups to be followed up by the responsible organizations.

Abstract: 

Background & Objective: Monitoring the depth of anesthesia is very important to prevent undesirable events during surgery, such as intra operative awareness and overdosing. It is shown that anesthetic agents have direct effects on synaptic activity of brain neurons. So there is a great interest on electroencephalogram analysis as a depth of anesthesia estimator. Due to difficulties in visual explanation of EEG, automatic and computer based signal processing methods have been used to assess the depth of anesthesia. Investigating the relationship between conscious level of patients and electrical activity of brain neurons was the main aim of this study.Materials & Methods: In this study, EEG signals of six patients undergoing aortic valve replacement surgery have been acquired and recorded in a computer. After applying signal processing methods to these data, 3 different measures included temporal, spectral and bispectral parameters have been extracted. Mean values of mentioned parameters in different anesthetic regimens and levels have been analyzed by ANOVA in SPSS software.Results: Extracted temporal parameter is correlated with depth of anesthesia in deep anesthetic levels and spectral one is correlated with depth of anesthesia in moderate and light levels (P<0.05). Bispectral parameter is correlated with the depth of anesthesia only in ICU (P<0.05).Conclusion: Findings of this study confirm the relationship between consciousness and electrical activity of brain neurons and recommend the use of EEG processing techniques to monitor, control and estimate the depth of anesthesia in operating room and ICU ward.

Abstract: 

Background & Objective: Vaccination is one of the most effective ways in preventing the hepatitis B viruse. This study was done to evaluate the response rate to hepatitis B vaccine in under one-year children in Gorgan, Iran.Materials & Methods: This cross-sectional descriptive study was carried out on 215, children of 7-12 month of age (55.3% male, 44.7% female), in Gorgan, northern Iran during 2006. These subjects already had received the three-time vaccination against hepatitis B. Anti-HBs, Anti-HBc and HBsAg level of these children serum were determined, using ELISA technique.Results: In 30 (14%) subject's serum, there was not detected any antibodies against the viruses. Out of 185 children positive for HBS, 4 cases belong to HBC viruses. As a whole, 86% of the samples, had more than ten international unit of HBS. The mean±±D of the titred antibodies in male and female were 158.84±12 and 187.55±13.83 respectively. The response not to three-times vaccination in male and female were 84% and 89.9% respectively.Conclusion: This study showed that in spit of vaccination, there are cases with virus-contamination. The reasons for absence of any response to the vaccination in some of this children can be due to deficiency in the immune system, improper genetic background which can not be stimulated by that vaccination, inadequate proper conditions for the preservation of vaccine, and the type vaccine used. Also the procedure of vaccine inoculation, the vaccine preservation condition and transportation should be taken into considertion.

Abstract: 

Background & Objective: The atherosclerosis of left anterior descending (LAD) coronary artery because of subtending more than 50% of left ventricular myocardial mass, has long been the focus of concern regarding appropriate treatment. This study aimed to evaluate efficacy of Percutaneous Transluminal Coronary Angioplasty (PTCA) with stenting for treatment of proximal LAD disease.Materials & Methods: In this descriptive study, 75 patients with isolated proximal LAD atherosclerotic lesion treated with PTCA and stenting were followed up for 9 months. The stenosis of all patients was documented by coronary angiography prior to enrollment. Exercise tolerance testing (ETT) by Bruce protocol was performed before angioplasty and at the end of follow up period as well.Results: There were 44 (58.7%) males, and 31 (41.3%) females. The mean age of patients was 59.4 (range, 39 to 83 yrs). Clinical outcomes of 73 (97.3%) patients were satisfactory. Only two patients developed re-stenosis during the follow up period. The first patient developed myocardial infarction at the 3rd day of her admission, and the second one had an abnormal ETT after 9 months. Percutaneous coronary revascularization with stenting was performed, and both of them were discharged without any problem.Conclusion: This study demostrated that the nine months prognosis of patients with isolated stenosis of proximal LAD coronary artery that treated by PTCA and stenting was acceptable. This treatment improves clinical status and results in lower hospital adverse event rates and in appropriate clinical outcome.

Abstract: 

Background & Objective: Chest pain is one of the most important and prevalent symptom in coroner artery diseases, in which the nurse has the key role in its management. This study was done to achieve the objective of assessment of the nurse's role in pain management in patients involved in coronery artery diseases.Materials & Methods: This descriptive study was done by the use of convenience sampling method in Vassaii hospital in Sabzevar city. 95 instances of chest pain were assessed by the nursed participated. In the research project. The tools used was a researcher–made check list and the nurse's role in different aspects, including the assessment of pain, relieve measures and reevaluating of the pain was studied. All the observations was recheck by third person. The Khowledge and perception of the nurses involved this research was studied by another technique prepared.Results: The most of the nurses put priority assessement on the location of pain and the assessment of other features of the pain did not get much attention, for assessing the verbal pain, the use of verbal descriptive was the main method and no one used the visual analog scale in pain intensity. In 92.6% of the instances the nurses practiced to relieve the pain and in most instances (96.8%) medication was used. The findings showed that in 41.1% instances, no specific reassessment was practiced.Conclusion: This study showed that altough the most nurses are aware of their part in relieving the patients pain but they do not fulfill this role in pain mamagement specially in assessing and reevaluating the pain.

Abstract: 

Background and Objective: Depression is the most common mood disorder. To assess the incidence and wide spread of depression and associated factor among of Bakhteyari tribal as native population in Chaharmahal and Bahkteyari province, this study was conducted.Materials & Methods: This study was a descriptive and cross-sectional study which was carried out on 400 subjects by using a systematic random sample technique during 2006. Depression was assessed by Beck inventory depressive questionnaire. Data was Analyzed by SPSS-11 Software and t-test, Chi-Square, ANOVA.Results: Mean±SD of depression score in female and male were 18.5±10.8 and 14.9±11.45, respectivly. Depression incidence rate in population was 29.6% (CI 95%: 25.6-33.6). Intermediate and upper degree of depression prevalence was determined to be 24%. The incidence rate of descriptive in female was higher than males (P<0.05). There was a significant relationship between depression in house wife and jobless people (p<0.05). The highest incidence rate was either under 20 years or above 50 years old.Conclusion: This study showed that the depresion rate is higher among the tribe than in other groups in this province.

Abstract: 

Background & Objective: Gorgan Gulf, with unique environmental value, has been located at the eastern south region of the Caspian Sea. It has not only valuable fish species such as strugen, Caspian roach, carp and vobla fish which supply a great part of daily protein need of our country and the world, but also its beaches are used for swimming. Therefore Lack of observation for environmental regulations result in entrance of microbial pollutants into Gorgan Gulf and creates dangers both for human health and for marine environment. So the evaluation microbial pollution for these beaches and comparing them with standards is an essential need. This study was done to evaluate microbial pollution for Caspian Sea at Gorgan Gulf. Materials & Methods: 40 sea water samples were collected during spring and autumn of 2004 at Gorgan Gulf for evaluation of sea water in Gorgan Gulf microbial pollution and also the mean total number of bacteria, coliforms, fecal coli forms and fecal streptococci was determined by standard method.Results: The results indicated that the mean total number of bacteria, coli forms, fecal coli forms and fecal streptococcus were 5001/1cc, 1555/100cc, 817/100cc and 59/100cc respectively.Conclusion: Mean results compared with related standard (both for swimming beach and piscicultres) and we found that Gorgan Gulf beaches have been polluted for swimming applications, but it is not the case for piscicultures.

Abstract: 

Granuloma faciale is an uncommon cutaneous lesion characterized by asymptomatic skin nodules and plaques without any systemic presentation. The lesions mainly seen in middle aged males. We report an uncommon presentation (multiple cutaneous lesions) in trunk, upper arm and face) of a 32 years old male.
