Abstract: 

Background and Objective: Valeriana Officinalis administrated for treatment of many gastrointestinal disorders traditionally and probably reduced smooth muscle contraction and bowel movement. The aim of this study was to determine the effect of Valeriana Officinalis extract on Ileum contraction of Guinea pig ileum in In Vitro model.Materials and Methods: This experimental study done on five male Guinea pigs. The animals were sacrificed and the pieces of ileum with 2-3cm long (at least 6 pieces) dissected and established in normal Tyrod solution plus Carbogen gas in 37ºc. Tissue fixed between of two electrodes that connected to stimulator apparatus. The contraction of ileum carried to physiograph by isotonic transducer and recorded. A solution with 5 concentration of Valeriana Officinalis extract was added to the organ bath during ileum stimulation with stimulator and the muscle contraction recorded in 7 responses.Results: Hydroalcoholic extract of Valeriana Officinalis root in concentrations of 5, 15, 50, 150, 500 mg/ml during 0.1 Hz stimulation decreased Ileum contractions in a dose dependent manner. The percent of reduce contraction was 34.45%, 36.07%, 47.53%, 56.42%, and 76.22% respectively. Also Ec50% of this effect of Valeriana Officinalis was found to be 50 mg/ml (P<0.05).Conclusion: This study showed that Valeriana Officinalis extracts can decrease ileum contraction in dose dependent manner.

Abstract: 

Background and Objective: There is growing evidence that excess generation of highly reactive free radicals, largely due to hyperglycemia, causes oxidative stress, which followed by further exacerbating the development and progression of diabetes and its complications. The purpose of the present study was to determine the effect of alpha-lipoic acid (ALA) on blood glucose and HbA1c in type 2 diabetic patients.Materials and Methods: In this clinical trial study, fifty-seven type 2 diabetic patients (14 male and 43 female) with the mean age of 53.5 years old were involved in this study. Upon arrival, subjects were randomly divided into either experimental (n=29) or control (n=28) groups. Experimental group received 300 mg alpha-lipoic acid daily for eight weeks where control group received placebo for eight weeks. After an overnight fast patients' blood samples, were drawn and analyzed for fasting blood glucose, 2 hours post-prandial glucose and HbA1C. In addition, antropometric indeces for each subject was measured at the beginning and at the end of the study.Results: There is no significant difference regarding weight and BMI in two groups before and after intervention. Also our findings indicated significant decrease in fasting and post-prandial glucose level, in experimental group, after intervention (p<0.05), but no significant change was seen in HbA1c level. There were no significant changes in parameters measured in control group. There was also a significant decrease in fasting blood glucose in experimental group when compared to control group (p<0.05), but there is no significant changes in HbA1c level.Conclusion: This study showed that alpha-lipoic acid supplement as an important antioxidant reduces blood glucos concentration in type 2 diabetes.

Abstract: 

Background and Objective: Plantar fasciitis is due to irritation and inflammation of the plantar fascia, which may occur in patients aged 30-60 years old. The aim of this study was to investigate the effect of new modified foot orthosis on improving the quality of daily life, sport and recreational activities in patients with plantar fasciitis.Materials and Methods: This clinical trial study was done at orthotics and prosthetics clinics in Tehran during Aug 2007 to May 2008. 15 patients with plantar fasciitis including 4 male and 11 female with ranging age 25-45 yrs old (35±5.02) were selected by non probability sampling. Patients used new modified foot orthosis. FAOS questionnaire was used for evaluating the activity daily life and sport and recreational activities. 2 stages included: once before intervention and the second stage after 4 weeks after intervention. Data analysis has been done with paired T-Test, Smironov-Kolomogrov by SPSS-13.Results: Quality of daily life was 44.30±10.16 before interventional treatment and 61.08±16.26 after intervention and difference between two stages was not statistically significant. The results for sport and recreational activity were 26.50±10.09 before interventional treatment and 52.50±12.23 after intervention and difference between two stages was statistically significant (P<0.05).Conclusion: This study showed that the new modified foot orthosis was no effective on improving of activity daily life but, was effective in sport and recreational activities in patients with plantar fasciitis.

Abstract: 

Background and Objective: Autism is a neurodevelopment disorder of the brain, which occurs in first few years of life, and characterized by symptoms such as qualititative impairments in verbal and non verbal communication, reciprocal social interactions, inability to communicate with others, stereotyped patterns of behavior, loss of eye contact and inappropriate facial expressions. Its prevalence is 2-5 in 10000 children and is greater in boys. Due to retarded language development, differential diagnosis other than those used with communication, behavioral and sensorial deficits should be made with Sensorineural or conductive hearing loss. The aim of this study was to determine whether children with autism have abnormalities affecting the cochlear nerve or auditory pathway in brainstem.Materials and Methods: In this case – control study, auditory brainstem responses were recorded from 12 autistic children and 12 normal children aged 3-12 years in rehabilitation Zafar central in Tehran – Iran during 2005. Absolute latency values of waves I, III and V and interpeak latencies of I-III, III-V and I-V were compared with the results of 12 normal children as control group.Results: Wave V could be traced down to 25 dB nHL in both normal and autistic groups. Absolute latency of Wave V and Interpeak latencies of III-V and I-V were significantly prolonged in Autistic children in comparison with control group.Conclusion: This study showed a slowing in nerve conduction in auditory pathway in brainstem of autism patients. The brainstem lesion may be a part of neurological damage in autistic children that accounts for deviant language, cognition and social development. Prolongation of wave V, III-V and I-V IPLs can be a marker for early diagnosis of autism.

Abstract: 

Background and Objective: A common complication after general anesthesia is nausea and vomiting followed by different problems such as spasm, hypoxia and pulmonary aspiration. This complication is more common in patients with full stomach, Eye injury, head trauma, cesarean and laparoscopy. Propofol and metoclopramide are two common drugs to prevent nausea and vomiting after operation. On the other hand adding dexamethasone to the above drug has an important effect on decreasing nausea and vomiting. In this study, the effect propofol and metoclopramide associated with dexamethasone on nausea and vomiting after operation was investigated.Materials and Methods: In this clinical trial study, 100 patients with ASA I, II classes, aged 16-60 years with selective orthopedic surgery randomly have divided into two groups. In group one, 48 patients received metoclopramide (10mg) with dexamethasone (8mg) and in group two, 52 patients received propofol (20mg) with dexamethasone (8mg), five minutes before the end of operation. Prevalence of nausea and vomiting in both groups was considered after 4 hours and results were analyzed by Chi-Square, t-student and Fisher exact tests.Results: The rate of nausea in group 1 and 2 was 35.4% and 11.5% respectivly (P<0.05). The rate of vomiting was 27.7% and 7.7% in group 1 and 2 respectively (P<0.05).Conclusion: This study showed that the antiemetic effect of propofol with dexamethasone is more effective to prevent nausea and vomiting than metoclopromide with dexamethasone.

Abstract: 

Background and Objective: Colorectal carcinoma accounts for nearly 10% of all incident cancers. The stage of the disease is the most important prognostic factor. The main purpose of this study was to evaluate the effect of some presumed prognostic factors on the survival rate of patients with nonmetastatic rectal adenocarcinoma.Materials and Methods: In this retrospective cohort study, 76 patients (m/f:40/36) with nonmetastatic rectal adenocarcinoma whome were referred to oncology department of Omid Hospital between 2001-06 were evaluated. All patients underwent surgical resection and those with T3-T4 and/or lymph node involvement received adjuvant radiotherapy and chemotherapy. Disease free survival was assessed from the date of diagnosis to the date of recurrence using Kaplan-Meyer method. Log-rank test was used to compare survival curves between groups. Multivariate analysis was performed using stepwise backward Cox Proportional Regression method.Results: Stages 1, 2 and 3 were detected in 4, 34 and 38 cases. 48 patients had well differentiated tumors. 11 cases presented with obstruction. With a median follow up time of 18 months, 17 patients experienced recurrence. For all cases 3-year survival rate was 68.2%. The 3-year survival rate was significantly better for stage 1,2 compared to stage 3, patient without obstruction compared to those presented with obstruction, cases older than 50 compared to younger patients and patients with well differentiated tumor compared to moderately or poor differentiated tumors (P<0.05). The gender and the site of tumor had not significant effects on survival. In multivariate analysis only stage of the disease remained significant predictor of survival (p<0.05).Conclusion: This study confirmed that the stage of the disease is the most important predictor of survival. Although younger ages (<50), moderately or poorly differentiated tumor and presence of obstruction at diagnosis were associated with decreased survival in univariate calculations, they lost their significance in multivariate analysis

Abstract: 

Background and Objective: Maternal position during the second stage of labor can be considered to be an intervention in the natural course of labor. This study aimed to establish the effect of sitting versus lithotomic position on duration of second stage of labor, preneal truma and neonatal outcome.Materials and Methods: This clinical trial study was done on 110 term nuliparuse women with normal conducted of labor in second stage, which divided randomly in sitting position, (n=55) and lithotomic position (n=55) in teaching hospital in Yazd, Iran during 2005-06. Duration of second stage, Perinea trauma and first 5 minutes APGAR evaluated and compared in two groups. Data analyzed by SPSS-11.5 with T student and Chi square tests. P<0.05 was significant.Results: Duration of second stage of labor was 34 and 42 minutes in sitting and lithotomic position respectively (P<0.05). Perinea damage was seen in 15 (27.3%) and 24 (43.6%) women of sitting and lithotomic position respectively (P<0.05). No significant difference existed between the groups in neonatal first 5 minutes APGAR.Conclusion: This study showed that sitting position reduce second stage duration and perinea trauma versus lithotomic position.

Abstract: 

Background and Objective: Colorectal cancer is the third common malignancy in the world. Personal habits including life style and nutrition have been proposed as risk factors of colorectal cancer. We conducted this study to assess the nutritional characteristics in colorectal cancer patients in Golestan province of Iran.Materials and Methods: This case-control study was done on 47 colorectal cancer cases, diagnosed during 2004-05 and 47 healthy controls. A questionnaire containing sociodemographic and nutritional characteristics (FFQ) was filled for each of the participants. Chi square test was used to assess the relationship between variables.Results: In each of the two groups, 40.4% were females and 59.6% were males. The mean±SD age of age in cases and controls were 52.4±13.4 and 52.1±13.1 years, respectively. Total energy expenditure of higher than 1837.5 Kcal/ day was significantly related to colorectal cancer (OR=3.8; 95% CI: 1.2-11.3). Colorectal cancer risk was higher in individual with frying cooking method (OR=3; 95% CI: 0. 8-11.1). The risk of colorectal cancer was 6.5 times higher in individuals with fat consumption of higher than 118.5 grams/day (95% CI: 1.5-28.8). No significant relationship was seen between vegetables consumption and the risk of colorectal cancer.Conclusion: This study showed that high total energy expenditure, high fat consumption and using frying method for cooking are risk factors of colorectal cancer in Golestan prevince in Nothern of Iran.

Abstract: 

Background and Objective: Gastric cancer is one of the most common cancers in the world. Although its incidence is decreasing, it rarely is detected early, and the prognosis remains poor. The aim of this study was to evaluate prognostic factors in gastric cancer using log-normal regression model.Materials and Methods: This retrospective study was done on 746 patients with gastric adenocarcinoma from February 2003 through January 2007. Genders, age at diagnosis, family history of cancer, tumor size and pathologic distant of metastasis were entered to a log-normal model. Relative risk (RR) was employed to interpret the risk of death.Results: Results indicated that patients who were upper than 45 years at diagnosis had an increased risk for death (RR=1.01 95% CI, 1.01-1.03), followed by greater tumor size (RR=1.64, 95% CI, 1.07-2.25) and pathologic distant metastasis (RR=2.14, 95% CI, 1.60-2.86) and similar results in multivariate analysis for greater tumor size (RR=2.04, 95% CI, 1.23-3.33) and pathologic distant metastasis (RR=2.01, 95% CI, 1.13-3.56).Conclusion: This study showed that the early detection of patients in younger and in primary stages and grade of tumor is important to decrease the risk of death in patients with gastric cancer and increase the survival rate.

Abstract: 

Background and Objective: Retinopathy of permaturity (ROP), a lesion in the retina of premature infants with low birth weights, can result in blindness and it is a major cause of blindness in children of developing countries. We were ought to determine prevalence and intensity of retinopathy of permaturity and investigate some related risk factors in NICU of Taleghani hospital.Materials and Methods: In this descriptive study, from March 2004 to December 2005, the result of clinical examinations and laboratory of 89 premature infants with gestational ages of less than 34week old were recorded in a questionair form. These infants, were examined by a retinologist, at the time of discharge and if necessary every 4 weeks, until retinal vascularization completed. The results of examination were recorded.Results: Retinopathy of permaturity was detected in 5 neonates; all of them in stage 1 and the most percent of prevalence in neonates with birth weights between 1001 to 1250 grams and gestational ages less than 29-week old. All cases were oxygenated and prevalence was more in the group who got oxygen via CPAP.Conclusion: Based upon this study, the most important risk factors for retinopathy of permaturity were lower birth weight and less gestational age. Moreover, it was one more time emphasized on the effect of oxygenation in developing retinopathy of permaturity.

Abstract: 

Background and Objective: There are different methods to improve sperm parameters including swim up. Sperm washing is a basic step in This method during 2007. It seems that two times washing cause bether result especially in high viscosity sample and sample it is not liquefy. The aim of this study is to compare the single and double washing swim up method on sperm parameters.Materials and Methods: This study was carried out on 50 semen samples referred to Fatemeh- Alzahra infertility center of Babol University of Medical Sciences during 2007. Semen samples had the following characteristics: volume 2-5ml, count>20 million/ml, motility>50% and normal morphology>14%. Sampling condition were completely considered. Sperm parameter analysis carried out after two times washing. Data entered to SPSS-10 and analyzed by repeated measurement and paired T-Test and p-value<0.05 considered statistically significant.Results: Semen volume mean was 4.050±1.006. Mean sperm motility in one and two times washings increased (P<0.05), but there was not statistically significant differences between one and two times washing. In grading analysis, by increasing the washing, grade III, IV increase and grade I decrease (P<0.05). Sperm morphology mean increased with washing but it is not statistically different between one and two times washing.Conclusion: This study showed that swim up method is effective method for sperm preparation and two times washing cause recuperation of sperm parameters.

Abstract: 

Background and Objective: Osteoporosis is the most common metabolic bone disease that characterized by reduced bone strength. The aim of this study was to evaluate the prevalence of effective factors in decreased bone density and secondary causes of osteoporosis.Materials and Methods: This descriptive cross sectional study was done on 105 patients (76 female and 29 male) suffering from osteoporosis, evaluated in the endocrinology Department of Sina hospital, Tabriz- Iran from March 2003 to March 2006. Past medical history clinical symptoms and biochemical results were of patients. Data analyzed using SPSS-14 and chi square test.Results: Osteoporosis and osteopenia were seen in 55% and 45% of patients with reduced bone density, respectively. Daily calcium intake in patients with less than 400 mg, between 400-1000 mg and more than 1000 mg were 63.8%, 31.9% and 3.4%, respectively. The mean±SD of sera calcium and vitamin D level were 9.5±0.6 mg/dl, 45±37.1nmol/l respectively. 61.2% of patients had vitamin D deficiency. 33% of patients had secondary osteoprosis. Among the patient with primary osteoprosis 11.3% afflicted to hyper claciuria.Conclusion: This study showed that decreasing bone density was more prominate in women. The rates of daily calcium intake among patients were low. It is suggested these patients osteoporosis could be prevented by consumption food nutrient rich in calcium and vitamin D supplementation.

Abstract: 

Background and Objective: There are some reports on the association between helicobacter pylori infection and typhoid fever in different parts of the world. The aim of this study was to estimate the co infection of helicobacter pylori and typhoid fever in Ahwaz city in South-West of Iran.Materials and Methods: In this case – control study, fifty hospitalized patients with definite diagnosis of typhoid fever in Razi and Abuzar hospitals in Ahwaz were investigated. For each patient, one subject was matched with age, sex and socioeconomic characteristic as control. All of the case and control were tested for IgG –anti helicobacter pylori by ELISA method. Data were analyzed by chi squared test in SPSS software.Results: Seroprevalence of helicobacter pylori in case (66%) was higher than control (44%) (p<0.05). There was a significant difference between patients and control in male (p<0.05), but not in female.Conclusion: This study indicated that typhoid fever is associated with helicobacter pylori co-infection in this region.

Abstract: 

Cervicogenic headache (CEH) is a chronic, hemicranial pain syndrome in which the sensation of pain originates in the cervical spine or soft tissues of the neck and is referred to the head. Cervicogenic headache is a relatively common but often overlooked disorder. There is sufficient evidence to support this category and the existing diagnostic criteria are adequate. The purpose of this case report was to describe an intervention approach consisting of release and muscle energy techniques for an individual with cervicogenic headache. Clinical examination revealed myofacial trigger points in trapezius, sternocliedomastoid and upper cervical spine erectors. Visual analogue scale (VAS) identified severe pain. Range of flexion and rotation was reduced. Complete pain relief was seen following three times treatment sessions. The results indicating definite diagnosis and appropriate treatment of cervicogenic headache.

Abstract: 

Child abuse as an ominous and abnormal phenomenon was scold in every community. In attention to severity of non - accidental injuries that inflict by parents or guardian and chance of death is considerable, intervention is necessary for life saving at early stage. A large number of child abuse fatality occur in world wide, that usually does not recognize at routine visits. Therefore the need for a program to recognize and intervent is an inevitable task. This study consider child abuse case with fatal abdominal injury with subsequent death.
