PREVALENCE OF EXTENDED-SPECTRUM BETA-LACTAMASE (TYPES TEM, SHV, CTX-M) IN E. COLI ISOLATED FROM EMAM REZA HOSPITAL IN TABRIZ-IRAN

Abstract

ESCHERICHIA COLI as the most common cause of Gram-negative sepsis. Especially threatening is the dramatic worldwide spread Escherichia strains whose ability to produce plasmid-coded extended- spectrum beta-lactamases (ESBLs) renders them resistant to the bactericidal activity of third-generation cephalosporins. Various types of ESBL, especially TEM and SHV enzymes, have been described worldwide. The present study was followed to determine the prevalence of various types of ESBLs in E. coli strains isolated from in-patients in intensive care unit of Emam Reza hospital in Iran. During one year, 32 strains of ESCHERICHIA COLI were collected from in-patients in intensive care units in Tabriz Emam Reza hospital and identified by current biochemical methods. Antimicrobial susceptibility test were done by DISK AGAR DIFFUSION method by cefotaxime, ceftazidime, ceftriaxone, azteronam, amikacin, piperacillin/tazobactam, ceforoxim, cefepem, imipenem disks. As a confirmatory test ceftazidime/clavolanic acid were used in combined disk test for determining ESBL-producing isolates. blaTEM genes, bla SHVgenes and bla CTX-M genes of them were investigated by PCR. All of strains were sensitive against imipenem. 93.7% of isolates were ESBL producer. 87.5% of strains have had blaTEM gene, 12.5% bla SHVgene and 31.2% bla CTX-M genes. Recent surveys showed that ESBL-producing E. coli isolates were colonized in intensive care units of Emam Reza Hospital, Tabriz, Iran by high rate which will be alarming. At present the first-line therapy against ESBL-producing strains is imipenem.
