Abstract: 

Introduction: Currently, a few percent of Cardiopulmonary Cerebral RESUSCITATION (CPCR) has been successful which would lead to prolonged survival of the patients. Hence, this study aimed to review the effect of managing in-hospital cardiopulmonary cerebral RESUSCITATION on the RESUSCITATION outcomes.Methods: This was a pre and post quasi-experimental study. In the pre-intervention phase, eighty patients were studied in a selected educational hospital in Tehran who underwent CPR, in terms of short and long term RESUSCITATION outcomes in 2009. The CPCR management process was conducted for five months. After stabilization of this new system, the RESUSCITATION outcomes again were reviewed by another eighty patients. For recording short and long term RESUSCITATION outcomes was used standard Utstein form. The statistical analysis was done by SPSS 16 software and descriptive-analytical statistical.Results: The short term outcomes increased after the intervention from 18.8% (pre) to 31.3% (post) and the long term outcomes also were boosted from zero percent (pre) to 2.5% (post). Generally, after the intervention, 66.3% of the RESUSCITATION were unsuccessful, 31.3% were successful in short term and 2.5% brought about the long term survival; and the patients with moderate to good brain function discharged from the hospital. This result had a significant difference with the situation of pre intervention phase (p<0.05).Conclusion: Performing the CPR management process can be applied as a model for performance of the managers of health-treatment centers to increase survival rate of the patients after the CPR.

Abstract: 

BACKGROUNDAND OBJECTIVE: Several studies have shown that hospitals with trained and designated emergency personnel have improved survival performance and hospital discharge. The aim of this study was to determine the effect of trained team on improving the outcomes of cardiopulmonary RESUSCITATION (CPR) in Imam Khomeini hospital, Tehran, Iran.METHODS: This clinical trial study was performed on 87 patients as control group and 53 as intervention group. In control group, after announcing CPR code, routine team (a nurse, Intern and medical resident) performed CPR. In intervention group CPR team included two trained nurses and an emergency medicine resident that were present in emergency department A checklist was used to evaluate the performance and quality of CPR team that was prepared based on American heart association. Physiologic variables and patient condition was obtained from the file. Data was analyzed by parametric and non-parametric statistical methods and p<0.05 was considered significant.FINDINGS: The results showed that the mean age of resuscitated patients in control group was 58.1±19.6 that in comparison to second group (59.4±19.2), there was no significant difference. Also there was no significant difference in physiologic variables of patients before occurrence of cardiac arrest in two groups. However survival in males was more successful but this difference was not statistically significant There was a significant difference in survival after CPR, between control and intervention group (18.4% versus 30%, p<0.01 and also the mean of hours' survival 28 versus 88.44 hours, p<0.001).CONCLUSION: According to the results of this study, trained and designated emergency personnel have improved event and survival. So CPR team training and continuing education for all nurses and residents should be considered. CPR team should be aware of up to date knowledge of CPR in order to decrease the mortality rate.

Abstract: 

Sudden cardiac arrest (SCA) is a leading cause of death in many societies (about 700,000 a year in Europe, and 330,000 in the United States and Canada of which 250,000 have occurred out of hospitals). Most victims of SCA demonstrate ventricular fibrillation (VF) at the first analysis of heart rhythm. Cardiopulmonary RESUSCITATION (CPR) plus defibrillation within 3 to 5 minutes of collapse can produce survival rates as high as %49 to %75. The American Heart Association (AHA) established guidelines for RESUSCITATION in 1974 and has continued to revise CPR and Emergency Cardiovascular Care (ECC) guidelines based on the latest findings. Updates to AHA guidelines are published approximately every 5 year, and are used as a basis for other major organization's RESUSCITATION training. The AHA published new guidelines for CPR and ECC in Nov., 2005. These guidelines were the product of rigorous documentation, analysis and evaluation of existing scientific literature related to emergency care by 380 international experts on RESUSCITATION convened by the International Liaison Committee on RESUSCITATION (ILCOR) over a 3-year period preceding the 2005 consensus conferences on CPR and ECC. The most significant changes in the new guidelines attempt to simplify CPR instruction, to increase the number of chest compressions delivered per minute and to reduce interruptions in chest compressions during CPR. This article reviews the newest recommendations for CPR in individuals at all age groups.
