Abstract: 

"Cerebral hemorrhage" is a term used for extravasated blood in the cranium that leads to neurological disorders. Seizure is one of these disorders. This study assesses the rate of seizure among patients who were brought to Bahonar Hospital for cerebral hemorrhage and whose diagnoses were confirmed by computed tomography scanning. Patients who experienced seizures in the first week following trauma and the spontaneous cases were considered. In other words, early seizures were studied. The results show that the rate of seizure is 5.5% which is not significant for age, sex, location of hemorrhage, cause of bleeding and size of hematoma. Rate of seizure has significant relationship with decreased level on conciousness (p<0.001) and surgical operation has no effect on seizure.

Abstract: 

Spirometry and flow-volume loop show the abnormal pattern of pulmonary dysfunction and the site of the obstruction of upper and peripheral airways.For determining the shape of flow - volume loop in obstruction of carina and bronchus we examined 19 patients with large airway obstruction documented with fiberoptic bronchoscopy via pulmonary function testing and flow-volume loop performing. Two patients about 70% obstruction of carina had F-V loop compatible with upper aiway obstrucion. 10 patients with obstruction of right or left main or intermedius bronchus had PFT criteria of restrictive pulmonary dysfunction but no change in F-Vloop. In seven patients with lobar bronchus obstruction this obstruction did not change the shape of flow-volume loop. We reached to the conclusion that F-V loop in obstruction of carina is the same as fixed upper airway obstruction but obstruction in main and intermediate bronchus’s show normal F-V loop with restrictive pattern, and obstruction of lobar bronchus dose not significantly change the shape of F-V loop or pulmonary function parameters.

Abstract: 

It is common to use muscle relaxants, such as succinyl choline to produce muscle relaxation during tracheal intubation.Based on some new researches alfentanyl was reported to be used without succinyl choline for tracheal intubation. The goal of this study was to evaluate the quality of tracheal intubation during general anesthesia with alfentanyl in order to see whether it can be used as a substitution for succinyle choline when this drug is contraindicated or not available. For this purpose 132 patients in ASA I & II physical condition were divided randomly into case and control groups for evaluation of the quality of intubation. In control group sodium thiopenthal 5 mg/kg and succinyle choline 1.5mg/kg, while in case group sodium thiopenthal 5 mg/kg as well as alfentanyl 20mg/kg were used Intravenously. The results were assesd by using chi-square formula.Laryngoscopic qualification such as mandibular (jaw) relaxation, coughing condition, limb movement and ease of laryngoscopy showed no difference between the two groups (P>0.05). The only differing parameter was the position of vocal cords (P<0.0018) which was more acceptable in control group using, succinly choline.

Abstract: 

In this Case report we presented an eighteen year old pregnant woman who suffered from intermittent fever, night sweat, weight loss and neck mass. She was referred to infectious disease clinic of the FIROOZABADI hospital and clinical and Labratory examinations revealed no hematologic, collagen vascular, malignant or infection diseases. However histopathologic biopsy of the neck lymph nodes revealed Necrotizing Histocytic lymphadenitis or Kikuchi Fujimoto disease.Although no specific treatment or medication were prescribed, but all of the symptoms and signs of the patient disappeared after delivery.

Abstract: 

It is a common Knowledge that the ECG diagnosis of myocardial infarction in the presence of left bundle-branch block (LBBB) is extremely difficult and often impossible. Physicians frequenlly recommend that patients with chest pain in the presence of LBBB receive thrombolytic therapy or urgent coronary arteriography assuming that acute injury and ischemia cannot be interpreted in the presence of LBBB.In this study we retrospectively examined the electrocardiograms of 64 patients with complete LBBB and acute chest pain and reviewed twelve electrocardiographic criterias of acute ischemia. All, 24 cases had acute myocardial infarction confirmed by enzyme studies.The criteria of ST segment configuration (the j point depression and horizontaly or upward convexity of ST segment) that was concordant with QRS-complex had an accuracy of 82.81% and included 23 of 24 (94.8%) acute myocardial infarction patients. The absence of this criteria and terminal S wave that had positive QRS-complex had highest accuracy (90.6%) to rule out myocardical infarction.

Abstract: 

Helicobacter pylorus is one of the world's most common pathogens. It colonizes about 60% of the world population. There is very little data present on the prevalence of H.pylori infection among healthy population in Iran. Our aim was to determine the seroprevalence of H.Pylori infection among children and young adult in Tehran and its relationship with a number of sociodemographic factors.In a cross sectional investigation, the seroepidemiology of H.Pylori infection was studied using an enzyme linked immunosorbent assay (EL/SA) for anti H.pylori IgG in 431 asymptomatic 10 - to 25 year - old volunteers attending 5 out - patient clinics in Tehran. 42.7% of the studied population was seropostive for H.pylori. There was an age - specific increase in H.pylori infection and there were significant differences in H.pylori infection rate related to residence site in Tehran, family size, parents educational level, economic status, water source and keeping pets. There was no significant difference in H.pylori infection related to gender. The results of logistic regression analysis showed that age, residecne site, family size, Parent's educational level, water source and keeping pets had the most significant association with H.pylori. It was concluded that the prevalence of H.pylori infection in Iran is generally higher than in developed countries and the rate of acquistition of H.pylori infection increases with age and inversely related to socioeconomic status.

Abstract: 

It is becoming apparent that one-stage primary resection and anastomosis of the left colon for acute sigmoid volvulus is safe and economical. This paper reviews 15 colonic resections with primary anastomosis, which were performed on unprepared bowel in patients with acute sigmoid volvulus at Firoozabadi hospital. The post-operative hospital stay was reduced. The death rate was 13.3% and complication rate 6.7%. These results suggest that this procedure is safe for nonischemic acute sigmoid volvulus.

Abstract: 

Carpal Tunnel syndrome (CTS) is one of the most common peripheral entrapment neuropathy in upper extremity. This syndrome is more common in women. It manifests with pain of the hand, arm and forearme. This study confirms that some patients with CTS have also cervical radiculopathy.In this study 113 patients who electro diagnostically confirmed CTS were studied for association of radiculopathy. Majority of patients were between 20 to 50 years old. The mean symptom duration was 21.3 month. %16.7 of cases had double crush syndrome (CTS associated with radiculopathy).

Abstract: 

Infection of middle ear and infection of sinuses are prevalent diseases in human populations which are caused by various etiological agents. To determine and compare the frequency of different virulent bacteria and fungi, the following study was conducted.58 Patients with sinusitis and 52 with otitis media, which were candidates for surgery, were studied irrespective of age and gender. Tissue samples or samples from local discharges obtained at the operation room were transferred to the laboratory in suitable transport media and were directly studied and subsequently cultured for the presence of aerobic and anaerobic bacteria and fungi.In the sinusitis patients the most prevalent virulent agents were bacteria (65.51%). The most prevalent aerobic bacteria isolated were S. aureus and S. epidermis is (62.92% in total) (P<0.000). Also in the patients with otitis media the most prevalent virulent agents were bacteria (71.14%).Considering the results of this comparative study, the frequency of aerobic bacteria was more than that of anaerobic bacteria in the patients of otitis media and the most prevalent aerobic bacteria are Pseudomonas and S. aureus specious (57.13% in toltal) (P<0.000).

Abstract: 

Infection and Peri-renal abcess have always been a diagnostic problem for radiologists.They cause high morbidity and mortality in late diagnosis.Ultrasonography (US) and CT-scan enable the radiologist to search the retroperitoneal space clearly and make a smart diagnosis by US and then drained with US guide are introduced.Low cost, high speed of drainage and complete response to therapy are the advantages which can introduce ultrasonography as a good alternative to open surgery.
