Abstract: 

Statement of Problem: With more polymerization rate there would be less time for the composite to flow and consequently sudden hardening occurs. This leads into composite detachment from cavity walls resulting in micro leakage between the restoration and tooth, and will decrees the bond strength.Purpose: The aim of this study was to investigate the shear bond strengths of the enamel and dentin resin composite using Suggested Progressive program (SUP) and some other light exposure patterns. Materials and Method: In this experimental in vitro study, the test groups were composed of 40 dentinal and 40 enamel samples prepared on facial surfaces of 80 human canine teeth. 2x2mm polyethylen cylandric molds were filled in bulk with Tetric Ceram composite and were placed on the prepared surfaces. Irradiation patterns for 4 groups of 10 dentinal and 10 enamel samples were as follows: Group 1, HIP (High Intensity Program). Group 2, LOP (Low Intensity Program). Group 3, PUL (Pulse Program) of Astralis 7 light curing unit exposure patterns, and Group 4, SUP (Suggested Progressive Program). After curing and 24 hours incubation in 37°c, the samples were thermocycled under 500 cycles (5-55°c), and mounted with acrylic resin. Shear bond strengths of all samples were obtained using the Instron testing machine with 0.5 mm/min crosshead speed. For statistical analysis, one way and two way ANOYA and Duncan testes were used by SPSS software. Results: There was no significant difference between the shear bond strengths of four light exposure patterns in dentinal and enamel groups (p<0.001). It was also revealed that various light exposure patterns had no significant effect on shear bond strength of composite to enamel and dentin (p=0.388).Conclusion: SUP light exposure pattern has the same capability of producing appropriate bond strength with the tooth structures as other light exposure patterns.

Abstract: 

Statement of Problem: Alveolar bone level measurement in periodontal disease is very important because it may affect planning of forthcoming surgery, treatment response and prognosis. Surgical approach is the most accurate procedure for obtaining this measurement. Nowadays, transgingival probing as a non-invasive and accurate method has been claimed to be a proper substitution for surgical measurement and direct visualization.Purpose: This study aimed at evaluation of diagnostic value of bone sounding technique in estimation of bone level in I, II and III-wall periodontal defects.Materials and Method: Ninety vertical defects in 37 individuals with chronic periodontitis having one, two or three-wall defects were assessed. Bone levels were measured by UNC-15 probe.The resulted values were compared with the values obtained during surgery. Collected data were analyzed using Paired samples T test and Pearson's correlation coefficient.Results: High and significant correlation was found between the results of transgingival probing and surgery methods (0.97, 0.94 and 0.95 in one, two, and three-wall defects respectively (p<0.001)). The mean bone levels measured by transgingival probing were 0.21, 0.53 and 0.19 mm less than their assigned values in surgery and there was no significant difference between one and three- wall defects (p<0.00I).Conclusion: The findings of this study indicated a high correlation between the two procedures, bone sounding and surgery, in detection of bone level. Due to the negligible difference, it seems that trans gingival probing could be considered as a feasible replacement for invasive surgical approach in detection of alveolar bone level.

Abstract: 

Statement of Problem: In most shear bond strength (SBS) studies, different cross head speeds have been used. The effect of different speeds on the shear bond strength has been studied in very limited researches.Purpose: The aim of this study was to evaluate the influence of changing cross head speed of testing machine on the shear bond strength and failure mode of orthodontic adhesive (Transbond XT) in bracket bonding.Materials and Method: For this experimental study, 175 human premolars with intact enamels were divided into seven equal groups of 25. After surface preparation, brackets were bonded to teeth using transbond XT adhesive. For deboning process various cross head speeds of 0.5, 1, 2.5, 5, 7.5, 10, 25 mm/min were applied. Shear bond strength and Adhesive Remnant Index (ARI) were recorded for different groups. One way ANOVA and Tukey tests were used for bond strength comparison. Data on shear bond strength evaluations and modes of bond failure were analyzed using Kruskal- wallis and spearman tests.Results: The means of shear bond strength for the applied 0.5, 1, 2.5,5, 7.5, 10 and 25 mm/min cross- head speeds were 16.39, 14.37, 13.30, 14.12, 13.32, 16.58, and 13.83 respectively. Shear bond strengths in groups receiving 0.5, 10 mm/min cross head speeds, were significantly greater than other groups. No significant differences were obtained between these two speeds, as well as, between 1, 2.5, 5, 7.5, 25 mm/min speeds, but there were significant differences in the shear bond strengths between the two groups with the cross head speeds of 0.5, 10, and 1, 2.5, 5, 7.5, 25 mm/min. Considering the ARI, no differences were found between the groups. Also, there was no relation between ARI and bond strength in groups with different cross head speeds, except in 5mm/min.Conclusion: Cross head speed of Instron is an effective factor on the SBS of the adhesives, but it does not have significant effect on the failure mode of bonding materials.

Abstract: 

Statement of Problem: The most important aspect of child behavior management is pain control. Since dental injection is usually considered as the most stressful procedure in dentistry, good pain control during injection and dental procedures is a major factor affecting children's cooperation. Purpose: The purpose of this study was to compare pain perception and efficacy of anesthesia in inferior alveolar nerve block using 27- and 30-gauge short needles. Materials and Method: In this clinical trial study which was conducted according to a split mouth design, 27- and 30-gauge needles were used for all patients. Forty patients between 5 and 8 years of age requiring local anesthesia for pulpotomy treatment in both mandibular second primary molar teeth were selected. Immediately after injections, pain ratings were obtained using face and SEM scales. Both nurses and children were blind about the type of used needles. Data were analyzed using Wilcox on Matched-pairs Signed-ranks Test.Results: The 30-gauge resulted in significantly lower pain than 27-guage needles. The success rate of local anesthesia technique was 95% for both types of needles. There was a significant correlation between the two pain measurement methods for both gauges (p<0.01). The mean intensity of pain was higher in girls than boys with both gauges. There was no significant difference between age groups regarding pain intensity. Conclusion: Due to the less pain experience and similar efficacy with the use of 30- compared to the 27-gauge short needles, it can be concluded that the use of 30-gauge needle is more appropriate for inferior nerve block anesthesia in children.

Abstract: 

Statement of Problem: The primary treatment of chronic periodontitis is scaling and root planning (SRP). In deep pockets, surgery is recommended following primary treatment. It has been claimed that local delivery devices can improve healing of inflammation.Purpose: The purpose of this study was to compare the efficacy of adjunctive use of sub gingival minocycline application in periodontal pockets in patients after SRP with those treated only by SRP.Materials and Method: This study was performed on fifteen patients with probing depth of 5mm or more. Probing depth, clinical attachment level, plaque index and bleeding index were evaluated at base line, and then primary treatment (SRP) was performed in all patients. In every patient one tooth was selected as experimental and one as control. In pockets of the test teeth, minocycline gel 2% was applied in weeks 0, 2, 4 and then was compared with control teeth in week S 2, 4, 16 which received only SRP treatment. For statistical analysis paired t-test and student t-test were utilized.Results: The mean of probing depth at base line, and 16th weeks in experimental group, was 6.53±0.83 and 3.46±0.76, while in control group it was 6.33±0.83 and 4.30±0.75 respectively. Pocket depth reduction in experimental group was more significant (p<0.001). The attachment level in experimental group at base line, and 16th weeks was 0.65±0.14 and 1.66±0.36 and in control group it was 0.78±0.16 and 1.33±0.36. The increased level of attachment in experimental group was more significant (p<0.001). The Bleeding and plaque Index also reduced in both group but they were more significant in experimental group (p<0.001).Conclusion: In this clinical trial, the results suggest that SRP with adjunctive sub gingival administration of minocycline gel is more effective compared to SRP alone. This may be related to good access and high concentration of drug in the area.

Abstract: 

Statement of Problem: A removable partial denture (RPD) is a common treatment modality for restoration of partially edentulous ridges. However this type of treatment may produce serious periodontal problems for remaining teeth.Purpose: The aim of this study was to determine the effects of RPDs with RPI design on the periodontal indices of abutment and non abutment teeth.Materials and Method: For this interventional and prospective study, 22 patients (30-68 years old; mean age of 48 years) were selected. All patients has Kennedy class I partial edentulous areas and were supposed to use removable partial dentures. Forty eight abutment and 48 non abutment teeth were assigned for this study. Clinical parameters including clinical probing depth (CPD), clinical attachment level (CAL), keratinized gingival width (KG), gingival index (GI) were measured in abutment and non-abutment teeth at baseline,1,4 and 12 months after delivery of RPDs. Data were analyzed using wilcoxon test.Results: All clinical indices with the exception of gingival index, at the time of baseline to 12 months, showed significant differences between abutment and non-abutment teeth (p<0.001).Significant differences were also noted for all indices between abutment and non-abutment teeth after 12 months. Conclusion: The results of this study indicated that removable partial denture with RPI design plays an important role in producing and intensifying the periodontal diseases in both abutment and non abutment teeth. It seems that this effect is due to the accumulation of more bacterial plaque. So, use of appropriate design and recommendation of good oral hygiene may decrease the severity of side effects.

Abstract: 

Statement of Problem: One of the most important roles of dental radiographies is periapical lesion diagnosis. Conventionl radiographs have some problems such as need of high dose exposure or special equipments. Digital radiography as a new method in dentistry has made a great revolution.Decrease in exposure dose, no need for dark room and other options that can facilitate the diagnosis are among their advantages.Purpose: The aim of this study was to compare the digital and conventional radiography in diagnosis of preapicallesions in dry human mandible.Materials and method: In this experimental study, the skull mounted teeth with 50 constructed preapicallesions were evaluated using conventional and digital radiography. The radiographs of the artificial lesions at the apexes were investigated by two expert radiologists. In this study the zooming as well as contrast and density changes of digital radiographs in Cygnus media software, were recorded. Data were analyzed using chi square test.Results: According to the findings, the mean of correct diagnosis of periapical lesions were: 66% for density-contrast change with digital method, 40% for digital zooming, and 39% for conventional method. According to chi square test there was a significant difference between the first methods (contrast- density change) with the other two methods (p<0.00 1). Conclusion: In this evaluation, among three different radiographic methods used, digital radiography with the ability of changing density and contrast was superior and more accurate than conventional radiography and zooming methods in diagnosis of the lesion's size.

Abstract: 

Statement of Problem: The success rate of light composite restorative treatments is in close relation to the light intensity of the light curing units.Purpose: The purpose of this study was to evaluate the rate of light intensity of visible light curing units, before and after voltage stabilizer application.Materials and Method: This study was performed on 82 light curing units in all clinics and dental offices of Yazd city. For evaluating light exit intensity, Hilux radiometer was used. At first, the light intensity was measured, and then by use of 220 V electrical voltage stabilizer the light exit intensity was recorded. Results were analyzed using Pearson and Spearman tests.Results: Mean range of light intensity before and after voltage stabilizer was 241.3 and 272.6 mW/cm2 respectively. There were statistically significant differences between before and after use of voltage stabilizer (p<0.05). Considering the light intensity, 64.7% of the light curing units were suitable for optimum curing and 8.6% were in group that required increase light exposure time.21.9% did not completely cure the composites, and 4.8 % were in group with higher light intensity 2 (over 500 mW/Cm2).Conclusion: Light intensity output of the curing units in dental clinics of Yazd city were less than expected, but by using voltage stabilizer, the light intensity was significantly increased.

Abstract: 

Statement of Problems: Helicobacter pylori (H.pylori) are considered to be a pathogen responsible for gastritis and peptic ulcers as well as a risk factor of gastric cancer. Recently, H.pylori has been detected in dental plaques, hence oral cavity has drawn attention to be the second site for H.pylori. However, it is not yet clear whether the oral cavity acts as a reservoir for this bacterium.Purpose: The aim of this study was to clarify whether the oral cavity is the reservoir for H.pylori, and to find out any relationship between gastritis and dental plaque the infection. Materials and Method: In this study, PCR was used for detection of Helicobacter pylori as a noninvasive method with higher sensitivity and specificity. Samples of the supragingival and sub gingival dental plaque were collected from 67 patients (44 with periodontitis, and 23 suffered from both gastritis and periodontitis). Using sequences of different H. pylori genes, four pairs of oligonucleotide primers were designed to optimize a sensitive and specific protocol for diagnosis and detection of H.pylori. The data were analyzed using Chi Square and Fisher statistical tests. The level of significance was considered at p£0.05.Results: According to the results of this study, prevalence of H.pylori in dental plaque of the whole samples was low (5.9%), but there showed to be a significant relationship between H. pylori infection in dental plaque of patients with gastritis (p<0.05).Conclusion: Although the prevalence of the Helicobacter pylori in dental plaques was shown to be very low, it might be necessary to pay close attention to dental plaques as the possible source of re-infection after treatment of gastritis. It is recommended that in treating gastritis, along with antibiotic therapy, attempts should be made in dental plaque removal and educating patients with proper health practices.

Abstract: 

Statement of Problem: The most important aspect of behavior management in children is pain. control. The most recent topical material used for reducing injection pain is Dentipatch. No study has been performed on the efficacy of this material in Iran.Purpose: The purpose of this study was to evaluate and compare the effectiveness of Dentipatch with 20% Benzocain gel.Materials and Method: For this clinical trial study, sixty children in need of bilateral posterior infiltration of maxillary buccal mucosa were selected. All children completed the General and Dental Anxiety Scale tests, and then received bilateral buccal infiltration either following 10 minutes application of Dentipatch or 2 minutes application of Benzocaine gel. For measuring pain, children were asked to complete a Visual Analogue Scale (VAS) after each injection, and to report which injection was more painful. The tests were performed by two trained observers. For comparison of pain reduction, the data were statistically analyzed using Paired t.test and Wilcoxan Sign Rank test.Results: According to VAS, in children without dental anxiety, the scores of the gel and patch were 66.85 and 29.92 respectively p < 0.001. The dental anxious children reports were 70.85 for the gel and 25.91 for the patch (p < 0.001). According to child's report, in children without dental anxiety, the gel score was 2.31 and the patch score was 0.54, whereas in dental anxious children the scores changed to 2.81 and 0.66 with a statistically significant difference (p < 0.001).Conclusion: The effect of Dentipatch in reducing pain was more pronounced than Benzocaine gel. Further investigations are necessary in order to decrease the Dentipatch application time.

Abstract: 

Statement of Problem: Occasionally, due to overexposure or overdevelopment, radiographs become darken. There is two way for improving the diagnostic quality of these radiographs. One is using lightening agents and the other is indirect digital radiography.Purpose: The aim of this research was to evaluate and compare the diagnostic quality of dark radiographs after using lightening solutions and indirect digital radiography.Materials and Method: For this experimental study, a dentulous phantom in the department of radiology was used. With bisecting angle technique, 50 Periodical radiographs were prepared from different dentulous areas of the maxilla and mandible. The control radiographs were prepared using standard technique. For obtaining test radiographs, the exposure time was increased by 1.5 times for each area to produce dark radiographs, the test and control radiographs were scanned and saved. The test radigraphs were lightened once with lightening solution and then they were lightened using photoshap program. Three trained postgraduate students were assigned to evaluate the radiographs in all five groups of images by giving grades to different anatomic parts based on their qualities. The data were statistically analyzed using Friedman and Wilcoxon tests.Results: There were statistically significant differences between dark radiographs and the other four groups (p < 0.001). Diagnostic quality of dark radiographs was improved by using lightening solution or indirect digital radiography. The mean score devoted to diagnosis quality of images lightened by lightening solution was more than those by indirect digital technique.Conclusion: By using lightening solution or indirect digital radiography, the diagnostic quality of darken radiographs can be improved, so there will be no need for repetition and this will prevent patients from excess exposure to irradiation. Lightening solutions are more effective than indirect digital technique.

Abstract: 

The aim of this article was to present an unusual sialographic finding in a case of submandibular gland in which the trifurcation of warthon's duct acted somehow similar to the obstruction factors. A 36 years old male was referred to Shiraz Dental School with the symptom of obstruction of the right submandibular duct. After systemic and clinical evaluation and radiographic examination, sialography was performed. It was revealed that the trifurcation of proximal part of deep (horizontal) segment of warthon's duct was the cause of obstruction. The interesting point in this case was the absence of gland swelling and pain during meal time which in other cases it occurs due to the usual obstructive factors such as sialolithiasis (calcified or non calcified), stricture, mucocele or salivary cyst. To our knowledge and based on the literature review, this is the first reported case of warthon's duct showing this type of anatomic feature which was detected by sialography.
