Abstract:

Introduction: Epilepsy is a common neurological disorder that may last a lifetime and will require regular medical care. One of the reasons the disease caused by traumatic head injuries. Some of our nation's veterans during the Iraqi imposed war against Iran, dealing with head trauma after being diagnosed with epilepsy.Purpose: The seizure of the war wounded and other patients with epilepsy and the factors affecting the occurrence of seizures.Material and Methods: Data recorded from male patients with epilepsy referred to Shafa Neuroscience Center between 2007 and 2011. The reason for referring patients is diagnosis of epilepsy and evaluation before surgery. All patients participating in this study have been under the V-EEG.Result: Logistic regression was used to analyze data. The results indicate that among war wounded, seizure Type, duration of epilepsy and IED covariates, only the war wounded has significant effect on seizure accordance (P<0.05).Discussion and Conclusion: Odds of Seizures occurred for war wounded is approximately 4.2 times odds of other patient.

Abstract:

Chemical warfare agents especially sulfur mustard has been widely used in Iraq-Iran conflict. Long-term complications of the respiratory system, eyes and skin that are most common involved organs in victims, proposed possible involvement of immune and inflammatory reactions. IL-19 is a novel member of IL-10 superfamily that has ascribed to be a pro-inflammatory cytokine in the most studies and anti-inflammatory in some studies. According to the importance of IL-19 as a member of IL-10 family, we decided to evaluate serum level of this cytokine in the sera of chemical victims. This case-control study was conducted on 144 severe lung injured sulfur mustard war victims, 57 mild to moderate lung injured sulfur mustard war victims and 55 unexposed participants as control group. The serum concentration of IL-19 was measured by a sandwich ELISA technique (R & D system Eliza kit) and read by ELISA reader instrument. The present study showed that although the serum level of IL-19 in the unexposed control group is more than the exposed group and also is increased in the mild to moderate exposed group compared to the severe exposed group, but these increases are not statistically significant. Median and mean of the serum IL-19 level in the exposed group increase with the increasing FEV1, FVC and decrease with the increasing FEV1/FVC but it was not statistically significant. There were significant correlation between serum level of IL-19 and FEV1/FVC in the exposed group (P=0.022). Although there is no significant difference in serum level of IL-19 between exposed and non exposed groups, but the role of this cytokine as an inflammatory factor in local inflammation and inflammatory processes is not completely rejected.

Abstract:

Objective: The effects of lateral wedge insoles with and without subtalar strap on vertical component of ground reaction force were compared. Methods: Twenty five patients aged over 40 years with grades I or II of medial compartment knee osteoarthritis based on Kellgrene and Lawrence grading system were participated in this study. They were tested in 3 conditions while walking in a 3-meter walkway: bare foot, lateral wedge insole and lateral wedge insole with subtalar strap. Three successful trials were recorded in each condition. Gait analysis was performed to compare the immediate effect of lateral wedge insole with and without subtalar strap on vertical component of ground reaction force. Results: There was significant effect of lateral wedge insole with subtalar strap on second peak of vertical ground reaction force related to lateral wedge insole and without insole conditions. But no significant effect of lateral wedge insole with and without subtalar strap was found on first peak of vertical ground reaction force and walking speed. Conclusion: The results of this study suggest that lateral wedge insole can reduce second peak of vertical ground reaction force. Also it was understood lateral wedge insole with subtalar strap might be more efficacious than lateral wedge insole in mild knee osteoarthritis patients.

Abstract:

Objective: Parkinson’s Disease (PD) is the most common movement disorder in the world that causes Hpokinetic dysarthria in patients. 80-90% of these patients have speech and language disorders. Diadochokinesis(DDK) test is one of the best and most common tools to evaluate neurological disorder speeches of any type and any severity. The purpose of this research is to configurate relation of oral DDK and disease severity.Material and Methods: in this cross sectional, analytical and noninterventional study 27 PD patients were chosen from clients of movement disorder clinic of neurology ward of Rasool Akram educational and treating center using convenience sampling and under inclusion criteria conditions, Mean of monosyllable diado test is evaluated by speech and language pathologist and evaluation of disease severity is done by neurologist using UPDRS-III. Diado of 21 age-sex matched group was assessed. Pearson and Spearman correlation coefficient and independent sample t-test are used to analyz data. Results: PD patients showed significantly slower diado (P<0.001). no significant relation is found between DDK and disease severity (rs=0.360, P=0.065) There is positive correlation between toe tapping part of UPDRS and DDK (rs=0.412, P=0.033). Sexuality doesn’t affect the results. Conclusion: this study shows DDK rate Parkinson patients is lower compare to normal people. There is no correlation between mean monosyllable DDK and disease severity (based on UPDRS-III). Sexuality doesn’t affect the mean DDK of patients.

Abstract:

Background: Ankle foot orthoses are effective in controlling wide-range orthopedic deformities. Two difference methods (prefabricated and custom-made) were used for construction of AFOs. However, the high cost of custom devices prevents some patients from purchasing them. Some patients and orthotics prefer to prefabricated devices. The purpose of this study was to examine whether the prefabricated AFOs (PAFO) can provide similar pressure distribution as custom-made AFO (MAFO) in healthy people. Objective: The purpose of the current study was to the comparison of two different fabrication methods of AFO by plantar pressure in asymptomatic adults. Methods: In this study, peak pressures (PP) were recorded with Pedar-x system while pressure insole was placed between foot and AFO. Twenty-four subjects participated in this study completed a total of three 8-m walking trials in each of two types of AFOs. Results: The PP was significantly decreased in the heel (P<0.004), first metatarsal (P<0.001), and second metatarsal head regions (P<0.003) in MAFO when compared to PAFO but PP was significantly decreased in second toe (P<0.027) and toes 3-5 (P<0.003) regions in PAFO in comparison with MAFO. No significant differences were observed at the other regions. Conclusion: The results of this study demonstrated that significant differences were observed in PP at the almost regions when comparing two AFOs. So they should carefully prescribe for using with various disorders.

Background: Transcription factor NFkB is responsible for a large number of genes expression including inflammatory cytokines, chemokine, immune receptors, enzymes and other preinflammatory molecules. NFkB deviation is one of the mechanisms of some diseases especially those that are associated with inflammation or apoptosis. Sulfur mustard is an alkylating agent that can damage enzymes, DNA and other macromolecules, also induces oxidative stress responses. Results obtained from recent studies on Sardasht sulfur mustard victims 20 years after exposure showed alterations on immune and inflammatory responses. Regard to NFkB significance in inflammatory and its related cytokines in this research we assessed NFkB level in these victims. Purpose: NFkB gene expression assess in mustard victims 20 years after exposure. Methods and materials: Population study was 189 people of Sardasht sulfur mustard victims and control group include 32 people of Rabat civil. Sampling procedure was systematic random. The result analyzed by SPSS and X2 and T-test static procedures. For nonparametric analysis Mann-Witney and Kruskall-Wallis were used. NFkB expression levels were evaluated by standard PCR in peripheral white blood cells. Results: Result assessment in two groups showed that NFkB median in exposure group were 188.75 ngr/ml and in control group were 142.84 ngr/ml which NFkB expression level in exposure group was upregulated. This increasing was significant (P= 0.009).Conclusion: NFkB factor involved in many cellular functions, so its increasing or decreasing has its own results. Due to reduction of inflammatory factors in these victims, its decline was expected but the results of this study showed its increment which likely was for compensates the reduction of inflammatory factors.

Abstract:

Introduction: Mental health -as one of the two primary elements of health- is a requirement for a effective, positive and pleasant life. Also mental health of people in a society, especially that of the effective groups, is necessary for the dynamism and development of the society. The primary role of university students as future’s managers in every society shows the necessity of ensuring their mental health, finding and eliminating the negative elements affecting the mental health.Goal: This research has been done to investigate the Correlation of Social Support tolerance and Mental Health. The subjects were 1233 Children of Martyrs and war-disabled in Universities of Ilam (338 students of Associate’s Degree: 27.41 percent, 895 students of B. A: 72.58 percent. Using KORJSI AND MORGAN, 297 students were selected unintentionally and filled in the questionnaire of GOLDBERG and HILYER’S (1997) TOLERANCE AND PHILIP’S SOCIAL SUPPORT. For analyzing the results, in addition to descriptive variables, one way variance analysis, T-TEST AND structural equation modeling WERE USED. Results Findings in this research indicate a significant relationship between Social Support and Mental Health (0.333), Social Support and tolerance (0.331) and also tolerance and mental health (0323). In this regard, social support of other important people in life (0.67), social support of the family (0.56), social support of friends (0.42) have the most significant relationship with mental health. Also in the step by step regression model, social support 0.411, social support and tolerance 0.614 predict mental health variance. Also, In structural equation modeling, the direct effect of support on health is 0.411, the indirect effect of support on health is 0.42 and the direct effect of tolerance on health is 0.15. Discussion and Conclusion Social support is one of the most powerful predictors of mental health, in a sense that the more extended social network support the person has, the more mentally healthy he is. Vaccinating the individuals against stressful situations before happening, social support reduces the rate of mental stress. On the other hand, tolerance is a factor which enhances the rate of life satisfaction since it reduces individual's negative emotions and increases the mental health rate. Also tolerance is defined as a resource to overcome the difficulties in life more easily, to resist against stress and eliminating their psychological effects. Findings in this research, parallel to other researches, indicate that people with high levels of tolerance, keep their psychological health in stressful and tough situations and enjoy psychological adaptation.
