Abstract: 

Background: The aim of this study was to investigate the effect of subcutaneous injection of metoclopramide, pethidine, and ketamine on postoperative pain after inguinal hernia repair under general anesthesia compared to control group. Methods: In this clinical trial study, 104 patients were selected and randomly divided into four groups of 26 patients. The patients received subcutaneous injection of each drug at the incision site. In the first group, 0. 5 mg/kg ketamine, in the second group, 0. 75 mg/kg pethidine, in the third group, 0. 1 mg/kg metoclopramide, and in the control group, normal saline were administered. We evaluated the pain intensity [by visual analog scale (VAS)], hemodynamic parameters, extubation and recovery time, drug complications, total rescue analgesic consumption, time to first analgesic request, and satisfaction score for the initial 24 hours after entrance to recovery. Findings: The mean of VAS at the most of measuring times (P < 0. 050) and the mean of total rescue analgesic consumption (P = 0. 007) were significantly lower in the ketamine and pethidine groups compared to metoclopramide and control groups. The satisfaction score for postoperative analgesia in pethidine group was significantly higher than the other groups (P < 0. 001). Conclusion: Pethidine and ketamine have equal effects on postoperative pain; but the patients were more satisfied with pethidine. The effect of metoclopramide on the postoperative pain was lower than the other two drugs; moreover, its effect was equal to the placebo and was associated with more adverse reactions.

Abstract: 

Background: The H1N1 influenza virus is a highly pathogenic virus that threatens human life. Vaccination is an effective way of preventing and controlling influenza. Production of recombinant hemagglutinin in the baculovirus expression system, in the insect eukaryotic cell substrate (Sf9), has been suggested as an effective strategy. Methods: The H1N1 influenza virus hemagglutinin gene sequence was prepared from National Center for Biotechnology Information (NCBI). After designing a specific primer, the sequence was provided using restriction digestion, cloned into pFastBacHTA plasmid, and transferred to the DH10Bac cell to produce a recombinant bacmid. After extracting the relevant plasmid, it was transfused into the insect cell; and after the expression of the protein by Sf9 cell, the presence of recombinant protein was confirmed using sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) and Western blot methods. Findings: The hemagglutinin gene (654 bp) was cloned in pFastBacHTA plasmid using the two enzymes of BamHI and Xhol. Sf9 cell expressed a protein weighing approximately 60 kDa after receiving the recombinant bacmid protein. The extracted protein was identified and confirmed using SDS-PAGE and Western blot methods; and protein concentration was measured by Lowry method. Conclusion: The baculovirus system is useful for the production of proteins with complex structures. Generally, it can be concluded that this protein is highly expressed in insect cells. Due to the similarity of this system with the human system, it can be a suitable alternative for embryonic eggs in the future, and can be used in vaccination.

Abstract: 

Background: Several adverse effects (AEs) such as ocular AEs have been reported following systemic consumption of isotretinoin. Here, we report a case of bilateral chalazion, conjunctivitis, and hordeolum associated with systemic isotretinoin. Case Report: A 20-year-old girl was treated with oral isotretinoin because of severe inflammatory acne, and developed bilateral chalazion, conjunctivitis, and hordeolum about 4. 5 months after initiation of treatment. Two weeks after cessation of oral isotretinoin, patient’ s eye condition improved. However, after re-initiation of oral isotretinoin treatment with a reduced dose, her conjunctivitis relapsed; therefore, the treatment was once again terminated. Conclusion: Since the patient’ s condition was bilateral, recovered after cessation of therapy; and once again, relapsed after re-initiation. It was most likely a complication of the medication and not a coincidence.
