Abstract: 

Introduction: Conventional psychiatric treatments have weaknesses in the treatment of schizophrenia spectrum disorders and have little effect on skill-training and community-based areas. Therefore, the present study aimed to investigate the role of the Community Re-entry Program as a community-based complementary therapy in increasing therapeutic compliance and reduction of relapse and rehospitalization in patients with schizophrenia spectrum disorders. Methods: The present study was performed by quantitative method with repeated measures and using randomized clinical trial (RCT) research design. The statistical population of the study included all patients who were admitted to the acute wards of Raazi Psychiatric Hospital between January 20 and March 20, 2017, with a diagnosis of at least one of the schizophrenia spectrum disorders. A total of 80 patients with inclusion criteria were selected by simple random sampling and randomly assigned to the intervention (40 patients) and control (40 patients) groups using a random number table. The Intervention method was the “, Community Re-entry Program”,which was performed in 16 group sessions with a frequency of 4 sessions per week, in combination with conventional psychiatric treatment in the experimental group, while the patients in the control group received only conventional psychiatric treatment. The only tool used in the present study was a demographic questionnaire and the variables of therapeutic compliance and reduction of relapse and rehospitalization were also evaluated at intervals of 1, 3, and 6 months after discharge through telephone interviews with patients' families. The use of drugs, relapse of the symptoms, and rehospitalization of patients were evaluated in the mentioned time periods. Collected information and data were analyzed using SPSS-16. Results: The results showed that the implementation of community re-entry program in combination with conventional psychiatric treatment had an effect on increasing therapeutic compliance ( = 40/145, P= 0/001) and reduction of relapse ( = 45/54, P= 0/001) and rehospitalization rates ( = 40/75, P= 0/001) in patients. Conclusions: The Community Re-entry Program, as an adjunct to conventional psychiatric treatment, is one of the effective interventions to increase therapeutic compliance and reduce the rate of relapse and rehospitalization. In order to compensate for the weakness of conventional psychiatric treatment in increasing therapeutic compliance and reducing the relapse and rehospitalization rate of patients with schizophrenia spectrum disorders, non-pharmacological complementary therapies such as Community Re-entry Programs can be used.

Abstract: 

مقدمه: درمان های روانپزشکی متعارف در درمان اختلالات طیف اسکیزوفرنیا با ضعف هایی مواجه اند و بر حیطه های مهارت آموزی و جامعه محور تاثیر چندانی ندارند، لذا پژوهش حاضر با هدف تعیین اثر بخشی "برنامه بازگشت به اجتماع" به عنوان یک روش مکمل درمانی جامعه محور در افزایش متابعت درمانی، کاهش عود و بستری مجدد در بیماران مبتلا به اختلالات طیف اسکیزوفرنیا صورت گرفت. روش کار: مطالعه حاضر با روش کارآزمایی بالینی تصادفی (RCT) با اندازه گیری مکرر انجام شد. جامعه آماری شامل تمام بیماران مبتلا به دست کم یکی از اختلالات طیف اسکیزوفرنیا بود که در سه ماهه پایانی سال 1396 در بیمارستان رازی بستری شدند که با استفاده از نمونه گیری تصادفی ساده تعداد 80 نفر از بیماران واجد ملاک های ورود به مطالعه انتخاب و به صورت تصادفی با استفاده از جدول اعداد تصادفی در در گروه های مداخله (40 بیمار) و کنترل (40 بیمار) گمارش شدند. روش مداخله ای مورد استفاده در این پژوهش "برنامه بازگشت به اجتماع" بود که در قالب 16 جلسه با تواتر 4 جلسه در هفته به صورت گروهی در کنار درمان روانپزشکی متعارف در گروه آزمایش اجرا شد در حالی که بیماران گروه کنترل صرفا تحت درمان روانپزشکی متعارف قرار گرفتند. تنها ابزار مورد استفاده در پژوهش حاضر پرسشنامه جمعیت شناختی بوده و متغیرهای متابعت درمانی و کاهش عود و بستری مجدد نیز در فواصل 1، 3 و 6 ماه پس از ترخیص از طریق مصاحبه تلفنی با خانواده بیماران ارزیابی شدند، بدین صورت که مصرف داروها، عود علایم بیماری، و بستری شدن بیماران، در بازه های زمانی مذکور مورد ارزیابی قرار گرفت. داده ها با استفاده از نرم افزار اس پی اس اس نسخه 16 تحلیل شد. یافته ها: یافته های حاصل نشان داد که اجرای برنامه بازگشت به اجتماع در ترکیب با درمان روانپزشکی متعارف بر افزایش متابعت درمانی (001/0=p، 145/40، ) و کاهش میزان عود (001/0=p، 54/45، ) و بستری مجدد (001/0=p، 75/40، ) بیماران تاثیرگذار بوده است. نتیجه گیری: برنامه بازگشت به اجتماع، درکنار درمان روان پزشکی متعارف از مداخلات موثر بر افزایش متابعت درمانی و کاهش میزان عود و بستری مجدد است. به منظور جبران نقطه ضعف درمان روانپزشکی متعارف در افزایش متابعت درمانی و کاهش میزان عود و بستری مجدد بیماران مبتلا به اختلالات طیف اسکیزوفرنیا می توان از درمان های مکمل غیردارویی از جمله برنامه بازگشت به اجتماع بهره گرفت.

Abstract: 

Introduction: Improving emotion regulation can be helpful in coping with Covid-19 grief disorder. The aim of this study was to compare the effectiveness of emotion-focused therapy and mindfulnessbased cognitive therapy of emotion regulation in women with Covid-19 grief disorder. Methods: The method of the present study was quasi-experimental with pre-test, post-test, and twomonth follow-up. From 25 to 45 years old women with grief disorder (caused by Covid-19) in Tehran in 2020-2021, 45 people were selected by voluntary sampling and randomly assigned to three groups (15 people in each group). The research tool included a scale of difficulty in emotion regulation (Gertz and Roemer, 2004). One group received emotion-based therapy (10 sessions of one and a half hours) and one group received mindfulness-based cognitive therapy (9 sessions of one and a half hours). The control group was placed on a waiting list and did not receive any intervention during the study. Results: The results showed that both interventions had a significant effect on emotion regulation components compared to the control group (P<0. 01). The results also showed that emotion-focused therapy significantly reduced the mean components of emotion rejection, difficulty in impulse control, lack of emotional awareness, and limited access to emotion regulation strategies compared to mindfulness-based cognitive therapy. Conclusions: The results indicate the competitive effectiveness of emotion-focused therapy compared to mindfulness-based cognitive therapy on emotion regulation.

Abstract: 

Introduction: Success in quitting addiction requires completing the treatment process. The personality of the addict or its psychological disorders may play a mediating role in adherence to treatment. The aim of this study was to investigate the mediating role of personality components in the relationship between anxiety, stress, depression, and adherence to treatment. Methods: This descriptive-correlational study was conducted in addiction treatment clinics in Saqez with the participation of 131 men members of the Association of Anonymous Addicts. Data collection tools were the NEO personality questionnaire and the DASS-21 questionnaire which was completed as a self-report. Data were analyzed by the chi-square test, logistic regression, and Pearson correlation coefficient in SPSS 18 software. Results: Depression and personality components (neuroticism, introversion, flexibility, agreement, responsibility) predict adherence to treatment in addicted patients, but among these variables, the neuroticism component is stronger (p= 0. 001, β, =0. 426, R 2 = 0. 178). Also, the components of personality and adherence to treatment are predictors of anxiety, among which the predictive flexibility is stronger (p = 0. 001, β,= 0. 313, R 2 = 0. 021). Finally, the components of personality and adherence to treatment, except for responsibility, are predictors of stress, among which the predictive flexibility is stronger (p = 0. 001, β,= 0. 354, R 2 = 0. 041). Conclusions: Considering the effect of personality traits of addicted people in the occurrence of psychological problems such as depression, stress, anxiety, and adherence to their treatment, it is recommended to pay attention to the personality component and psychological problems of these people in education and treatment.

Abstract: 

Introduction: Teacher effective support, student emotional support, and academic emotions have important effects on a variety of student academic motivational orientations. Since no research has been done in this field, in this study, the relationship between teacher affective support and student emotional support with Intrinsic motivation, Extrinsic and Amotivation with the mediation of academic emotions is assessed. Methods: This study was a correlational study. The population of this study included junior high school students (grades 7, 8 & 9) in the year 2020-2021 in Tehran. The study sample consisted of 295 students who participated in the study by available sampling. Teacher affective support, student emotional support, academic motivation, and achievement emotions questionnaires were used to collect data. Data collection was performed virtually through a Porsline sub-program due to the COVID-19 prevalence. For data analysis, the path analysis model was used in R software version 4. 1. 1. Results: The results showed that teacher affective support and student emotional support with the mediating role of academic emotions had an effect on intrinsic, extrinsic, and motivation. The final model had a good fit and the RMSEA index was less than 0. 001 and the model fit index (AGFI <0. 9). Teacher affective support and academic emotions had a direct effect on intrinsic motivation, extrinsic motivation. Student emotional support had a direct effect on intrinsic motivation and extrinsic, but its relationship with motivation was not significant and had no effect. Teacher affective support and student emotional support had a direct effect on academic emotions. Conclusions: Findings showed that teacher effective support, student emotional support, and academic emotions have an effect on intrinsic and extrinsic motivation. Therefore, teachers should pay special attention to the emotional support of students and encourage students to support each other emotionally, and allow them to express emotions in the classroom to increase their academic motivation.

Abstract: 

Introduction: Emotions always have a significant effect on the formation of behavioral patterns. Therefore, behavioral interventions based on it are a suitable way to deal with psychosomatic disorders. Anxiety is an effective component in people that can potentially affect the quality of sleep. The aim of this study was to determine the effectiveness of Trans diagnostic-based Emotion Efficiency Therapy on sleep quality and emotional schemas of anxious nurses. Methods: The method of this study was Quasi-experimental with the control group and a two-month follow-up among the nurses who work in Chamran and Hazrat Rasoul hospitals in Ferdows in 2019, 50 anxious nurses were selected by purposive sampling and randomly assigned to experimental (n=25) and control (n=25) groups. Therapeutic sessions Trans-diagnostic based emotion efficiency therapy were presented in 8 sessions over two months and the anxiety inventory (Beck & Steer, 1990), emotional schemas )Suh et al 2019) and Pittsburgh Sleep Quality Index (Buysse et al, 1989) were used to collect data. Data were analyzed methods of analysis of covariance in SPSS-25 software. Results: The results showed that emotion efficiency therapy in the post-test period was effective in decreasing sleep quality (p <0. 001) and reducing both components of emotional schemas (p<0. 001) and these results were also stable in the post-test period (p<0. 001). Conclusions: Based on the findings of this study, Trans-diagnostic based emotion efficiency therapy can be effective as timely interventions to decrease sleep quality and reduce emotional schemas of anxious nurses and this intervention can be used in medical centers.

Abstract: 

Introduction: Anxiety is the most common spectrum of psychiatric disorders and one of its most important types is generalized anxiety disorder. The Sleep quality is the psychological sensation that affects the quality of life and feeling healthy and also leads to symptoms of mental and physical illness. The purpose of this study was to comparison of effectiveness of cognitive-behavioral therapy, biofeedback therapy and their Integrating therapy on sleep quality in women with generalized anxiety disorder. Methods: The research method was semi-experimental with pre-test, post-test and two-month follow-up with the control group. In this study, the statistical population of all women with generalized anxiety disorder was referred to the Aramesh center of Boroujerd city in year 1400 and using the available sampling method, 60 of them were selected and replaced in four equal groups. The experimental groups underwent cognitive-behavioral therapy (8 sessions), biofeedback therapy (8 sessions), combination therapy (8 sessions),but the control group received no training and remained in the waiting list. To collect data generalized anxiety disorder scale (Spitzer and et al, 2006) and Pittsburgh sleep quality index (Buysse and et al, 1989). Data analysis was performed using SPSS-24 software in two sections: descriptive and inferential (analysis of variance with repeated measures and Bonferroni). Results: The results of the study showed that three types of treatment in post-test and follow up had a significant effect on sleep quality in women with generalized anxiety disorder (P<0. 05). Also, Integrating therapy had stronger effects in improvement of sleep quality than cognitive-behavioral therapy and biofeedback (P<0. 05). Conclusions: Based on the results of the present study and considering the greater effectiveness of Integrating therapy, it is suggested that Integrating therapy (cognitive-behavioral therapy with biofeedback therapy) as a treatment to reduce sleep problems in women with generalized anxiety disorder in centers.

Abstract: 

Introduction: Divorce is a type of loss that affects people’, s health and causes personal, family, and social disintegration, and in most cases, its destructive effects on women are more severe. The aim of this study was to investigate the effectiveness of reality therapy based on choice theory on general health and automatic negative thoughts of divorced women. Methods: This study was semi-experimental with pre-test and post-test design with the control group. The research population was divorced women who were referred to Masire Khoshbakhti clinic of Isfahan city in the 2021 year. The research sample was 30 individuals who were selected by convenient sampling method and then randomly assigned to the experimental group and control group (15 people in each group). The experimental group trained 8 sessions of 120 minutes with the methods of reality therapy once a week and the control group did not receive any training. The research measuring instruments were the general health questionnaire (Goldberg, 1972) and the automatic negative thoughts questionnaire (Hollon & Kendall, 1980). Data analysis was performed using multivariate analysis of covariance (MANCOVA) using SPSS-26. Results: The results of the analysis showed that reality therapy had a significant effect on improving general health and reducing automatic negative thoughts in divorced women in the experimental group compared to the control group (p<0. 05). Conclusions: Due to the effectiveness of reality therapy on general health and automatic negative thoughts in divorced women, it is recommended that this training be used to reduce psychological injuries of divorced women.

Abstract: 

Introduction: Workaholism as a negative condition is associated with various individual and social consequences. The aim of the present study was to investigate the mediating role of rumination in the relationship between self-esteem and maladaptive perfectionism with workaholism. Methods: The research design was descriptive –,correlational. From the statistical population of health care workers in public hospitals in Tehran, a total of 316 workers were selected using the voluntary sampling method. Data were collected using the Ruminative Response Scale (RRS), Workaholism Battery (WorkBAT), Positive and Negative Perfectionism Scale (PNPS), and Rosenberg Self-Esteem Scale (RSES). To analyze the data structural equation modeling was conducted. Results: The results showed that the direct effect of self-esteem on rumination (β,=-0. 44, P<0. 001) and the direct effect of workaholism (β, =-0. 15, P<0. 001) were significantly negative. Also, the direct effect of maladaptive perfectionism on rumination (β,= 0. 37, P<0. 001), the direct effect of rumination on workaholism (β, = 0. 28, P<0. 001) and the direct effect of maladaptive perfectionism on workaholism (β,= 0. 26, p <0. 001) were significantly positive. Finally, results of the bootstrapping test showed that mediating role of rumination in the relationship between self-esteem (P <0. 01) and maladaptive perfectionism (P <0. 01) was significant. Conclusions: The present study emphasizes the mediating role of rumination as an important cognitive mechanism in the relationship between personality factors and workaholism. The findings of this study could help to guide targeted preventions and interventions for workaholism in health care workers. Therefore, paying attention to dysfunctional cognitive mechanisms and modifying them can be a good way to reduce workaholism.

Abstract: 

Introduction: Cardiovascular patients face many problems in the field of health and marital life, and in this field couple therapy methods with compassion therapy can be effective. Therefore, this research was conducted with the aim of determining the effectiveness of couple therapy with compassion therapy method on depression, married quality of life, and optimism in cardiovascular patients. Methods: This study was quasi-experimental with pretest and posttest design with the control group. The research population was cardiovascular patients referred to Javad Al-Aeme Cardiovascular Hospital of Mashhad city in the first half of the 2021 year, which from the 30 people after reviewing the inclusion criteria were selected by purposive sampling method and by simple random with lottery replaced in two equal groups. The experimental group 8 sessions of 90-minute (two sessions per week) underwent the couple therapy with compassion therapy method and during this period the control group did not receive any training. Data were collected through the questionnaires of depression (Beck & Clarck, 1988), marital life quality (Zhang et al, 2013), and optimism (Scheier et al, 1994) and analyzed by tests of Chi-square, independent t, and multivariate analysis of covariance in SPSS-19 software. Results: The findings showed that the experimental and control groups did not significantly different in terms of education, gender, age, and duration of cardiovascular disease (P>0. 05). Before the intervention, there was no significantly different between the mean scores of depression, marital life quality, and optimism of the experimental and control groups (P>0. 05), but after the intervention, was found significantly different between the mean scores of all three variables (P<0. 05). The couple therapy with compassion therapy method led to reduced depression (F=151/44) and increased marital life quality (F=139/48) and optimism (F=381/53) in cardiovascular patients (P<0. 001). Conclusions: The results of this research indicated the effectiveness of couple therapy with compassion therapy method on reducing depression and increasing marital life quality and optimism in cardiovascular patients. Therefore, health professionals and therapists can use this method to improve the characteristics related to health and married life in cardiovascular patients.

Abstract: 

Introduction: Adherence to treatment is the main determinant of success in the treatment of chronic diseases, which are affected by various factors. The aim of this study was to investigate the mediating role of coping strategies in the relationship between Illness Perception and Perceived Social Support with Treatment Adherence in Cardiovascular Patients. Methods: This research is correlation research. The statistical population of the study is cardiovascular patients hospitalized or referred to the specialized cardiology clinic of Shahid Madani in Tabriz in the first quarter of 1400, where 234 of them were selected as available samples for the present study. Data were collected using Broadbent's Illness Perception, Zimet's Perceived Social Support, coping with the stressful situations of Endler & Parker, and Modanloo's Adherence questionnaires. then, through the Pearson correlation coefficient, bootstrap and Sobel were analyzed using SPSS and AMOS (version 24). Results: The results showed Perceived social support (p<0. 05), Illness perception, and coping strategies (p<0. 001) have a direct, positive, and significant effect on Treatment Adherence. Also, Illness perception on coping strategies (p<0. 001) and perceived social support on coping strategies (Problem-Focused and Avoidance strategies, p<0. 001,Emotion-Focused, p<0. 01), have a direct, positive, and significant impact. The results of the Sobel test showed that the Illness Perception through coping strategies (Problem-Focused Strategy, p˂, 0. 001,Emotion-Focused and Avoidance, p˂, 0. 01) had an indirect, positive and significant effect on treatment adherence. Perceived social support is also due to coping strategies (Problem-Focused Strategy, p˂, 0. 001,Emotion-Focused, p˂, 0. 05,Avoidance, p˂, 0. 01) having an indirect, positive and significant effect on treatment adherence. Conclusions: According to the research findings, planning and goals can be designed to promote Illness Perception and the necessity of social support, and training the use of effective coping strategies in cardiovascular patients.
