Abstract: 

Background: Cardiovascular diseases (CVDs) and specially coronary artery diseases (CAD) are of the main causes of mortality and morbidity in the world. Preventing the risk factors of these disorders not only may reduce their incidence in the population, but also increases the efficacy and improves the prognosis of expensive therapies that are being used for cardiovascular patients.Most of the patients suffering from advanced coronary artery disease may need coronary artery bypass graft surgery (CABG). Therefore identification of risk factors of atherosclerosis and CAD is of the most valuable steps toward finding proper therapeutic options.Materials and Methods: This study was a cross-sectional descriptive retrospective study. Medical records of patients who underwent CABG were reviewed. Records gathered from Vil-e-Asr (A) hospital medical archive department. Demographic data, lab results, physical examinations and risk factors related to CAD have been extracted from the records.Results: Total of 120 patients have been studied, 119 were male and 1 female. Ninety two (76.7%) have had no history of cigarette smoking and 28 (23.3%) were smoking. 27.5% of patients have had past medical history of diabetes mellitus which 95% of them were taking oral agents. In biochemical assessments, 20% of patients have had high levels of fasting blood sugar and 33.3% were having impaired fasting glucose. 18.3% of patients have had history of dyslipidemia. 30.8% of patients were overweight (BMI=24.7– 26.9) and 35% were in grade 1 obesity (BMI=27-29.9) and 15.8% were suffering from morbid obesity (BMI>30). 38.3% of patients have had history of high blood pressure.Conclusion: Results of this study shows that interaction and effect mechanism of risk factors of CVDs and effect of other factors like behavioral, genetic and biomedical on them makes a complex field of interactivity between factors that shows the necessity of more studies.

Abstract: 

Background: Low Back Pain (LBP) is a major health and economic problem. The aim of this study is to compare pain, and disabling effects of chronic low back pain in NAJA-Vali Asr Hospital patients before and after a period of medical treatment.Materials and Methods: In this cross-sectional study of 167 patients with LBP attending NAJA Vali-e- Asr (A) Hospital, who were under treatment with NSAIDs for 6 weeks, based on physician’s diagnosis were selected availability and using Cochran formula. Severity and disability from LBP questionnaire of OSWESTRY collected from all patients before and after treatment. A mean change of less than 10% suggested as partial recovery, while the patients who reached more than 10% decrease suggested as significant recovery. Frequency, mean, Standard Deviation, t-test and chi-square test were performed using SPSS software.Results: The results indicated that there is a significant change in the mean score of pain-related disability before and after treatment (respectively 26.37±7.89 and 34.76±8.85, P=0.0001). Results also showed 32.3% of patients reached the recovery level of less than 10% after treatment, while 67.7% achieved 10% or more. Results showed a significant association between gender and the recovery rate (P=0.016).Conclusion: The findings showed that medications is effective in reducing disability from low back pain and Oswestry questionnaire also can be performed beside the medical diagnostic field to assess disability from back pain.

Abstract: 

Background: This paper aims to survey different parameters of servant leadership model and proposes a theoretical scheme to apply in hospital management. It additionally examines the applicability of this model in Police Hospitals.Materials and Methods: The study exploits an applied-development approach as well as a descriptive-survey design among 374 members of hospital executives, physicians and faculty members in two categories. The sample was chosen by Ray Morgan research population estimation method. 251 members were chosen based on simple random sampling. Data gathering was done according to the Likert-type scale with 7 levels and the reliability of 0.89. Moreover, statistical analysis was based on confirmatory factor analysis examination, structural equation modeling as well as fitting model with SPSS and Lisrel toolkits.Results: The analysis of data showed that enabler and insightfulness are the most and the less important characteristics of servant-based leadership in hospital management with the factor weighting of 0.79 and 0.3 respectively. Additionally, the second order conformity factor analysis and the final fitting index demonstrated that twelve variables out of the total number of eighteen were confirmed. So five characteristics and twelve variables were confirmed at the end. It is mentionable that highly agreeable fitting scales (RMSEA=0.098, GFI=0.95, AGFI=0.94) show a reliable fitting pattern.Conclusion: It can be concluded that having a comprehensive enabling plan is the most important expectations of members from an executive chief in a servant leadership approach. Therefore we suggest holding workshops for hospital managers in the light of operational leadership and empowering employees to increase their skill in planning.

Abstract: 

Background: In systems such as police force that any person is considered as an asset of the organization, noise pollution can lead to the behavioral abnormalities such as aggression that may cause occupational burnout. Therefore this study was conducted to compare aggression between Musicians and Non-Musicians groups of NAJA.Materials and Methods: The present study was a cross-comparative research and tests were applied on all members of music group (n=42) of NAJA university. To access the control group, we selected 42 subjects and matched them with experimental group. To measure aggression we used Buss-Perry Aggression Questionnaire and for collecting demographic information we used a questionnaire designed by authors. Data processing was performed using student T-test and Pearson correlation coefficient by SPSS 20 software package.Results: The results of the comparison between two groups indicated that the score of overall aggression was significantly higher in music group than other one. The similar difference was also achieved for physical and verbal, anger, and hostility subscales of questionnaire (p=0.001). Moreover, we found the significant positive relation between job history and aggression with Pearson correlation (p=0.011). It means that the more year people work in music group, the more aggression they have.Conclusion: Findings obtained from this study indicated the importance of high noise exposure effect on behavioral changes such as aggression among members of music group. The authorities should adopt and rigorously implement approaches to reduce and control exposure to this stressful environmental factor for the mentioned group. Allocation of suitable space for practice and providing acoustic space, proper scheduling for work and rest and psychological interventions to reduce aggression and increase compatibility in workplace are the most important approaches to achieve this goal.

Abstract: 

Background: Suicidal behavior is one of the fundamental strains in the area of mental health. One of the most considerable suicidal behaviors is the suicidal ideation which is an important predictor for suicide completion. This study has conducted the role of borderline personality disorder, impulsivity, and excitement-oriented coping style in predicting soldiers’ suicidal ideation.Materials and Methods: A total of 1, 659 soldiers were selected through multi-level cluster sampling method. Data were collected using Beck Suicide Ideation Scale (BSSI), the Millon Clinical Multiaxial Inventory (MCMI-III), UPPS Impulsive Behavior Scale, and Coping Style Questionnaire (CSQ).Results: The result has indicated that not only the borderline personality disorder directly affects the suicidal ideation but it could also have a role in predicting the suicidal ideation via the way of directly influencing the other variants. The findings resulting from the method analysis are indicative of the intermediary role of excitement-oriented coping style and impulsivity regarding the relationship between the borderline personality disorder and suicidal ideation.Conclusion: Therefore, the research findings exhibited that the relationship between the borderline personality disorder and the suicidal ideation could be more precisely and comprehensively explained via the intermediary variants such as the excitement-oriented coping style and impulsivity. Screening and identifying soldiers through diagnosing the borderline personality disorder and hyper impulsivity and also those individuals more frequently using the excitement-oriented coping style are effective for the purpose of restricting access to weapons in prevention of suicide attempt.

Abstract: 

Background: The aim of present study was to investigate the effects of work-family conflict of working mothers on anxiety disorders in their children and to compare anxiety disorders in children who their mothers work in military hospitals with civilian hospitals. In this research anxiety disorders includes 5 anxiety subtype (Separation, generalized, school phobia, somatization and social).Materials and Methods: This study is a casual-comparison that was performed among 176 working mothers who have children with ages between 8-12 in six military and civilian hospitals in city of Kerman, Iran. The data has obtained from 44 cases in military and 132 case in civilian hospitals. The SCARED and Work-Family Conflict questionnaires were used. Data was analyzed by descriptive statistics (mean and standard deviation), inferential statistical methods (ANOVA) and Pearson correlation with using SPSS version 19.Results: Obtained results showed that there is a significant difference between work-family conflicts of working mothers with anxiety disorders of their children in 5 levels. Also the results revealed that there was not any interactive effect between work-family conflict of working mothers in military and civilian hospitals with their children’s anxiety disorders. Moreover, the result showed that anxiety disorders in children that their mothers work in civilian hospitals was more than military hospitals.Conclusion: Despite of interactive effect of work-family conflicts on anxiety disorders was not significant in military and civilian hospitals; all of their children showed some symptoms of this anxiety disorders.work-family conflicts of working mothers specially in hospitals and effect on anxiety disorders on their children should be managed to decreased the conflicts and their effects on family.

Abstract: 

Background: The widespread use of explosive weapons such as mine is cause of various injuries in the victims. It is very important to care about life safety of the people and military personnel as specialized people in charge of security of the nation. Epidemiological studies in these events are one of the most effective ways to manage and reduce the damage caused by the explosion of groups in the border areas contaminated by mines and other explosive materials. This study deals to evaluate kinds of injuries related to mine and munitions explosion and their related factors in Ilam province-Iran.Materials and Methods: This survey is a cross-sectional (descriptive-analytic) study. Data for this study were collected from census of all case records of injuries caused by the explosion of mines and other munitions between 2001-2007 that were registered in department of forensic medicine, Ilam, Iran. Descriptive statistics and Chi-square, Fisher exact, one-way ANOVA and Tukey tests were used to describe the results of this study.Results: From the total of the 106 injuries over the years there were 95.3% male and the rest were female, that age group of 20 to 40 years had the highest number of injuries (47.2%). Also among the injuries, limb defects were 43.4% (CI95%: 0.29-0.57), skin lesions were 16% (CI95%: 0.01-0.33), eye injuries were 15.1% (CI95%: 0.01-0.32), upper extremity neurovascular defects were 3.8% (CI95%: -0.14-0.22), upper and lower limb bone defects were 18.9% (CI95%: 0.02-0.36) and of skull bone injuries were 2.8% (CI95%: -0.15-0.21). Also, we found significant relationship between of the type of injury and age, gender, occupational status and type of explosives in individuals injured.Conclusion: The degree of impairment caused by the explosion of mines and other ammunition in this study is higher than the other similar studies. These events can leave irreparable harm to the physical or mental health of the affected people. Also ignoring the areas affected can much damage mental health residents, such as farmers, ranchers, and the troops stationed at the border.

Abstract: 

Background: Gastric cancer is known as the 4th cause of malignancy and mortality in the world. Several risk factors have been defined for this malignancy as diet and environmental factors. Curcumin is a herbal derivate which has obvious effects on suppression of malignancies via inhibition of apoptosis, cell proliferation, invasion and angiogenesis. It has also been shown that Curcumin has no adverse effect on body normal cells. The most important limitation factor in its clinical use is being water insoluble, low absorption rate and bio-instability. The aim of this study was to use Dendrosomes as carriers of Curcumin and to study the induction of apoptosis on AGS cells.Materials and Methods: In this study, AGS cells have been exposed to different concentrations of Polymer Nano Curcumin (5 to 20 micromole). Metabolic activity of these cells has been evaluated by using MTT test. Also apoptosis has been studied using Annexin test.Results: This study showed that Nano Curcumin concentration of 13 micromole is able to suppress metabolic activity of 50% of cellules and also to suppress apoptosis in 26% of them in compare with control group.Conclusion: The concentration of encapsulated agent that may lead to inhibition of metabolic activity and apoptosis is too low. As Dendrosomes have been shown to be non-toxic on body cells and also the amount of free Curcumin which is needed to have effects like this study is too high, it looks that 13 micromole concentration of Nano Curcumin polymer has significant better effect on clinical targets in compare with free form.
