Abstract: 

Introduction: Clinical competency is the use of skillful, technical and communication skills, knowledge, clinical reasoning, emotions and values in clinical settings. Developing the professional competence of investment nurses to ensure the safety and quality of patient care. The factors influencing the clinical competence of experience, environment, and the use of opportunities, motivation, theoretical knowledge and individual characteristics can be mentioned. The competence of nurses in critical care units is very effective because of the sensitivity and complexity of these wards in improving their scientific and practical quality and ultimately improving the care of patients with critical conditions. Aim: The purpose of this study was to determine the clinical competency of nurses working in critical care units and its relation with the underlying factors. Materials and Methods: The type of study is descriptive-analytical. The statistical population included nurses working in 11 critical care units, 3 educational-medical hospitals in Kurdistan University of Medical Sciences in 2018. Sampling method was census. 122 (87. 2%) nurses completed the questionnaires by self-report method. The research instrument consisted of a demographic questionnaire with clinical competency questionnaire of nurses in Ebadi and colleagues with five factors of care management, Technical Competency, individual management, Human-oriented Care and Scholarship-oriented Care. To measure the items, a 5-point Likert Scale (from a minimum score of one to a maximum score of 5) was used. The validity of the questionnaire was carried out in Ebadi et al., in 2014. Reliability of the questionnaire in this study was obtained using Cronbach› s alpha coefficient (α = 94). Data analysis was performed using SPSS 24 software and descriptive and inferential statistics. Results: The mean and standard deviation of nurse’ s clinical competency score were 4. 24 ± 0. 42 from 5. The highest score of its factors was in the factor of individual management with a score of 4. 37 ± 0. 49 and Technical competence and the lowest scores related to the Scholarship-oriented Care factor with a score of 3. 72 ± 1. 31. Among factors, care management has a direct correlation with female gender (p = 0. 017 t = 2. 002). There was no relationship between other factors with clinical competency and its significance (α = 0. 05). Conclusion: The clinical competence of nurses in intensive care units is high in this study. Due to the attitude of women toward nursing, they are more competent in managing patient care. However, given the low scores of nurses in the field of scholarship-oriented Care, it is necessary to pay more attention to topics such as the use of modern technologies and information technology in nursing, evidence-based care, research-based, continuing education, and inservice training to enhance the competence of nurses, especially in critical care wards. This requires focusing more on these topics in policy makers and managers the limitations of the evaluation study are self-reporting, which requires further studies in other ways.

Abstract: 

Introduction: Occupational stress is highly associated with how organizational management and support work. In our country, organizational management is often private and public and has a different support pattern. The current study was conducted to Comparison of occupational stress level of nurses in public and private hospitals in Qom, 2016. Material & Methods: This descriptive-analytical study was carried out with 320 nurses working in public and private hospitals in Qom using random sampling. The data collection tool was a demographic questionnaire and a The Expanded Nursing Stress Scale (ENSS). The collected data were entered into the SPSS21 software and analyzed by descriptive statistics, t-test and ANOVA. Results: In this study, 89 (27. 81%) males and 233 (72. 19%) female with a mean age of 35. 52 ± 6. 69 years participated. The results of the study showed that the occupational stress level of nurses in public and private hospitals was at a mild level (55 ± 0/34 and 51 ± 0/79). There was no significant difference between the level of occupational stress among nurses in Qom public and private hospitals (p=0. 78), however the high levels of occupational stress in private hospitals nurse were significantly higher than public hospitals (p<0. 001). Conclusion: Regarding the higher level of occupational stress among nurses in private hospitals in Qom province, it is advisable to periodically monitor occupational stressors to provide effective management functions to reduce the situation in these hospitals.

Abstract: 

Introduction: Problems such as poor academic performance or dropouts in medical students who are considered messengers and community health authorities have attracted the attention of many researchers to find way or ways to enhance Find the academic motivation in them. Aim: This study aims to develop a causal model of hardiness Psychology of medical students was based on personality characteristics mediated by academic self-efficacy was done Material & Methods: This study was a descriptive and structural equation study. The statistical population consisted of all medical students of Islamic Azad University of Boroujerd Branch during the year 2019-2020. Psychological Hardiness Questionnaire (Goult & Long, 2003), Academic Self-Efficacy Questionnaire (Patrick et al., 1997) and Short-Form Five Personality Traits Questionnaire (Costa & McCree, 2004) were used for data collection. Data were analyzed using descriptive statistics (mean and standard deviation), Pearson correlation coefficient, multivariate regression and structural equation modeling using SPSS-24 and AMOS-22 software. Results: The conceptual model of the study was acceptable. Overall, in the proposed model of research, the results showed that personality traits are directly and indirectly correlated with psychological hardiness through medical self-efficacy in medical students. The results also showed that there was a positive and significant relationship between academic self-efficacy and psychological hardiness (P<0. 05). The relationship between personality traits and academic self-efficacy was positive and significant (P<0. 05). Conclusion: Based on the results we can say Personality traits and academic self-efficacy are important determinants of students' psychological hardiness.

Abstract: 

Introduction: Improving patient safety is one of the main pillars of health services. On the other hand, the professional communication between the health team members and some personality characteristics of nurses, such as Assertiveness, can affect the quality of services provided to the patient. Therefore, this study aimed to determine how safety culture is associated with nurse-physician collaboration and Assertiveness from nurses' perspective. Material & Methods: A cross-sectional study was done in 2018, in this study300 nurses from 9 teaching hospitals located in South Khorasan province were selected through convenience sampling. Data were collected using the HSOPSC Hospital Safety Culture Questionnaire, Ushiro Nurse's Contribution, and Ratus Determinism. Finally, using SPSS-22 software, Pearson correlation and And analysis of the Amos route. Results: The results of the study showed that the mean and standard deviation of the safety culture score was 123. 40 ± 14. 26, the Assertiveness was equal to 1. 99 ± 14. 82 and the mean value collaboration was 81. 5 ± 17. 96. On the other hand, the results of Amos analysis showed that the cooperation of the physicians and nurse directly and Assertiveness indirectly affects the culture of safety. This is how Assertiveness influences the culture of safety through the collaboration of physicians and nurses. Conclusion: The findings suggest designing programs aimed at improving the relationship between physicians and nurses and enhancing the skill of nurses and thus enhancing patient safety.

Abstract: 

Introduction: Expert employees’ turnover such as experienced nurses in hospitals is a problem that disrupts the growth of hospitals and is a serious problem for managers seeking to hire, maintain and utilize human capital. Aim: The purpose of this study is to investigate the effect of transformational leadership style on turnover intention, with emphasis on the mediating role of job engagement, employer branding and organizational commitment of nurses. Material & Methods: The study population was nurses of public hospitals in Tehran. 332 of them were selected by random sampling and 287 correct questionnaires were analyzed. To collect data, standard questionnaires of transformation leadership style of Bass & Avilio (2000), job engagement of Schaufeli et al. (2002), organizational commitment of Allen & Meyer (1990), employer branding of Ambler & Barrow (1996) and turnover intention of Mobley et al. (1978) were used. Reliability and validity of the questionnaires were accepted through content validity, confirmatory factor analysis and Cronbach's alpha coefficient. For data analysis, structural equation modeling and Amos software were used. Results: The results of this study showed that transformational leadership has a negative significant effect on turnover intention of nurses through two ways; two variables at the individual level, i. e., job engagement and organizational commitment, and one variable at the organizational level, i. e., employer branding. Conclusion: It is recommended that managers of Tehran's public hospitals provide conditions through adopting a proper leadership style in a way that engage the nurses to their jobs and reduce their turnover intention by promoting the employer branding and organizational commitment of the nurses.
