Abstract: 

Background: The Auditory steady-state response (ASSR) is a scalp-recorded auditory evoked potential that is captured by far-field electrodes. It is elicited by periodic, transient or modulated acoustic stimuli. The possible effects of shifting the response recording channel on 40Hz ASSR thresholds and their recording times were evaluated and compared in this study. Methods: Thirty subjects with normal hearing (thresholds of 500 to 4000 Hz≤15 dB HL), with ages ranged from 18 to 30 years, gave written informed consent were enrolled in this study. Right-handed subjects were selected. New chirp stimuli using a two-channel EP recording system, the Eclipse, were presented only to the right ear via insert earphones. Thresholds for central frequencies (0.5, 1,2 and 4 KHz) and their total recording time in the ipsilateral- (recording from the right-EEG channel) and contralateral-array (recording from the left-EEG channel) relative to the stimulus ear were evaluated at 40Hz repetition rate. Results: The mean contralateral ASSR thresholds to 40 Hz rate were about 2.25 dB HL better than the ipsilateral mean (that only significant at 2000 Hz central frequency; P.value=0.03). Also, the mean contralateral ASSR recording time was about 5.31 minutes better than the ipsilateral mean (Pvalue=0.0005). Conclusion: In awaking adults with normal hearing, the contralateral 40Hz ASSR is slightly more accurate in threshold estimation and shorter in recording time than the ipsilateral one. This method is promising despite these differences may clinically be significant.

Abstract: 

Background: The goals for sedation in pediatric patients scheduled to undergo cardiac catheterization include immobility, analgesia, cardiovascular and respiratory stability. We investigated the effects of Propofol and Ketamine on hemodynamic, respiratory status, sedation level, pain score and recovery period in pediatric patients undergoing cardiac catheterization. Methods: We preformed a randomized clinical trial study on 40 pediatric patients. The patients were randomly assigned to two groups, so that 20 patients received Ketamine and 20 patients received Propofol. In all patients, sedation was started with Midazolam (0.03mg/kg), then followed by Propofol in the first group and Ketamine in the second one. The hemodynamic responses, respiratory parameters, recovery characteristics (Ramsey scale, pain score VAS) and relevant adverse effects of the two groups were recorded. Data was analyzed using Paired T Test, ANOVA and Stearman correlation coefficient. Results: Five patients in the Propofol group and on patients in the Ketamine group experienced a transient decrease in mean systolic blood pressure greater than 10% of baseline (p=0.034). Time to full recovery (mean±SD) was not significantly different in the Propofol group and Ketamine group (1.8 min vs. 2.9 min, P>0.05). Pain scores were significantly different in both groups (P=0.010). Patients' heart rates were significantly higher in Ketamine group (P=0.029). No significant difference in respiratory rate was recorded in both groups (p>0.05).Conclusion: Both Ketamine and Propofol are useful and safe in pediatric patients undergoing cardiac catheterization but it seems that it is better to use Propofol in stable hemodynamic pediatric patients under continuous blood pressure monitoring.

Abstract: 

Background: Dennatophytosis or ring wonn is one of the most common fungal infections. Trichophyton verrucosum (T.verrucosum) is common among the agents that cause dennatophytosis. The aim of the present study was to evaluate of antifungal activity of copper chloride (cucl2) and 8- hydroxyquinolin compound on T.verrucosum. Methods: One standard strain and two clinical isolated of T.verrucosum were used in this study. The standardized method, M-38 A for filamentous fungi, was used according to the guidelines of National Committee for Clinical and Laboratory Standards (NCCLS) to determine MIC. The effect of CuCl2 and 8-Hydroxyquinolin investigated by using various concentrations of this compound in T.verrucosum cultures. Results: Cucl2 and 8-Hydroxyquinolin compound exhibited antifungal activity on the three mentioned strains and the MIC of this compound was 25 mg/ml. Conclusion: Cutaneous The new antifungal drug scheming is required to discover new antifungal agent. The data showed that Cucl2 and 8-Hydroxyquinolin compound had antifungal effect, so they could be a good candidate for making of new antifungal compounds.

Abstract: 

Background: Fatigue and exertional dyspnea caused considerable impairment in the functional performance and quality of life in heart failure (HF) patients, therefore it influence all aspects of life negatively. This study was aimed to relation between quality of life and functional performance in patients with heart failure. Methods: This study implemented on 60 patients with heart failure (class II, III) in Ahvaz city (2006). Quality of life of the patients measured by Minnesota living with heart failure questionnaire (MLHFQ) and also 6-minute walking test (6-MWT) was used for determination of patient's functional performance. Spearman's correlation coefficient, t-test and liner regression used for data analysis. Results: There was a significant correlation among quality of life with age, functional class (II, III), length of disease, smoking, left ventricular ejection traction and 6-minute walking test (p< 0.05). Also patients' ability in 6-minute walking test was statistically correlate with above-mentioned variables. Linear regression analysis showed that age, functional class (II, III), functional performance (6-minute walking test) could predict quality of life in patients with heart failure (P< 0.05). Conclusion: According to the research findings, high functional performance in patients with heart failure associated with better quality of life, therefore improvement of functional performance by cardiac rehabilitation and controlling other manageable variables could promote health in this patients.

Abstract: 

Background: One of non pharmacologic strategies influencing diabetes is physical activity which is not easy to change and maintain due to its complexity. The main objective of the present study is to identify the factors which influence physical activities on the basis of health promotion model. Methods: In this study 350 women suffering from diabetes responded to the standard questionnaires related to the perceived self efficacy (belief about capabilities for doing physical activity under different sets of conditions), perceived barrier and benefit, family and friend support and perceived health status constructs. Linear and logistic regressions, t tests, and chi square tests were used to analyze the data. Results: The results of the present study indicated that self efficacy has positive and direct impacts on physical activities and the perceived benefits, the perceived health status, and the body mass index (BMI) has indirect impacts on physical activities. Moreover, the perceived health status in addition to the indirect impacts has direct and positive impacts on physical activities and influence by factors such as job and duration of disease. Conclusion: The perceived health status is one of the most influential factors on physical activities of diabetic patients which is necessary to be taken into consideration especially for patients with long term disease. It is also urgent that through increasing self efficacy with using different techniques and emphasizing different aspects of physical activity benefits, resulted to promote the activity level of the patients.

Abstract: 

Back!!round: Nausea and vomiting is common post operative complications that often necessitate the use of antiemetic agent sand it is still a problem. This study evaluated the effect of additional fluid therapy on PONV on patients undergoing Shirodkar's operation. Methods: In a randomized, double blind Trail, the effect of pre operative moderate fluid administration on post operative nausea and vomiting (PONV) following anesthesia was studied. One hundred twenty women undergoing shirodkar surgery was randomly assigned to receive Npo fluid (control group) or Npo fluid plus 10 ml.kg (moderate hydration group) of ringer solution pre operation. During recovery time and in the ward nausea and vomiting were evaluated. Results: Groups were similar with respect to demographic data, surgical procedures and baseline homodynamic variables. During the recovery room and in the ward, the incidence of nausea and vomiting were significantly lower in the hydrated group as compared with control (p<0.05). Conclusion: The present study showed that moderate hydration reduces the incidence of post shirodkar surgery nausea and vomiting, and seem that IV fluid therapy is a simple, effective, safe and well-tolerated technique for prevention of postoperative nausea and vomiting.

Abstract: 

Background: More than 70%of diabetes patients die from cardiovascular causes. Furthermore, consequences of coronary artery disease (CAD) are more severe, because myocardial ischemia is often silent in these patients. Selecting only patients who met American Heart Association guideline have failed to identify 41% of patients with silent ischemia. Pointing to the need to modify these guidelines, further better procurers to identify those who may beneft from screening are required. Methods: The sample size of this study was the articles indexed in pubmed, Elsevier, Tripdatabase, Embase, Black Cochrane, New spring link, Ovid and contained the terms "diabetes, cardiac perfusion scan, silent ischemia". Results: 698 articles were found that 68 articles were investigated for this issue. The results are discussed under the subtitle such as silent ischemia mechanism, myocardial perfusion imaging and clinical indeces; and also the efficacy of myocardial perfusion imaging in comparison with other modalities. Conclusion: In this summary, we tried to review the current literature about who will benefit from myocardial perfusion scan screening as well as the best individualized test and reach a comprehensive guideline for practical significance of this subject in routines practice.

We present a 17 years old man who referred due to absence of puberty, micro penis decreased Intelligence quotient and also some other sign and symptoms. The presented case stresses performing of kariotype examination (chromosome analysis) in the evaluation of patient with such problems.
