Abstract: 

Background: Cystic lesions with odontogenic epithelial origin and similar clinicoradiographic appearance, show different clinical behaviors.Objective: To compare some factors related to cell proliferation and escape from apoptosis in epithelium covering two groups of odontogenic cystic lesions with different clinical behaviors.Methods: In this cross-sectional study 11 paraffin-embedded samples were selected of each lesions radicular cyst, dentigerous cyst, odontogenic keratocyst, and unicystic ameloblastoma. The sample underwent immunohistochemical staining for investigating the expression of ki-67 antigen and bcl-2 protein. Data analyzed with SPSS17 software and Kruskal–Wallis and chi-square statistical tests.Findings: Most of ki-67 positive cells were observed in parabasal layer of odontogenic keratocyst [35.50±26.29%; P=0.001]. The average of ki-67-LI was more in parabasal layer of aggressive group (26.80±37.79%) compared to non-aggressive group (4.04±3.38%), was not being statistically significant. The highest average of bcl-2-LI was 95±6.70% in basal layer of odontogenic keratocyst (P=0.001). In all layers, the average of bcl-2-LI was more in aggressive lesions compared to non-aggressive ones and the highest amount was found in basal layer (72.45±3.94×10%) which was statistically significant (P=0.001).Conclusion: According to the results of this study, more expression of the markers related to escape from apoptosis in aggressive lesions group compared to non-aggressive group, suggests that escape from apoptosis had a more critical role in aggressive behavior of odontogenic cystic lesions.

Abstract: 

Background: Hypoxia is a common phenomenon in human solid tumors which by increasing in angiogenesis induction cause tumor growth survival and metastasis.Inhibitory factor hypoxia regulatory factor (HIF1AN) by binding to transcription co activators (CBP/P300), inhibits hypoxia inducible factor (HIF1α).Objective: The relationship between HIF1AN expression and vascular invasion in colon tumors.Methods: The study included 101 patients with colon adenocarcinoma which were divided to vascular invasion and non-vascular invasion groups. Tumor paraffin blocks were immunohistochemistry stained for HIF1AN and were assessed for intensity and extent of positivity.Statistical relation of marker expression and clinic pathologic findings were assessed. Data were analyzed by SPSS 21 software and logistic regression and chi-square test.Findings: Nuclear immunoreactivity of HIF1AN was different between two groups. Statistical relation between low HIF1AN expression and tumor vascular invasion were seen (P=0.01). No relation was found between tumor differentiation, depth and HIF1AN.Conclusion: Evidence showed that the low expression or incorrect position of HIF1AN in nucleus of tumor cells was effective on HIF1α inhibition failure and factors associated angiogenesis increased. The HIF1AN played an tumor suppressor gene (TSG) role in colon tumors and decreased protein in the nucleus of colon cancer cells increased the expression of angiogenesis factors and vascular invasion.

Abstract: 

Background: Due to the situation and different functions of tongue, it is always expose to chronic stimulations, that’s why some kinds of lesion in tongue such as reactive lesions found in order to react these reactions.Objective: To determine the prevalence of reactive lesions in the archives of tongue reactive lesions in Tehran (Shahid Beheshti) and Qazvin dental faculties archive during the past 15 years.Methods: In this descriptive-sectional and retrospective study, all records related to tongue reactive lesions were investigated in pathology archive of Tehran (Shahid Beheshti) and Qazvin dental faculties in the period of 1998-2014. In each record, patient’s demographic characteristics (age and gender), lesion type (irritation fibroma (IF), Drug induced fibrous, Pyogenic granuloma (PG), and Hyperkeratosis), lesion location in tongue, patient habit and systematic diseases were recorded. Then data were analysed with Q-square, T-test and Anova test.Findings: The average age of investigated patients was 43±16 and 69/2% of patients were female. Generally from 70 recorded file Irritation fibroma (51/4%) had the most frequency. PG (38/6%) was respectively in second frequency rank.42/9% tongue reactive lesion were located in lateral part, 28/6% in anterior part and then, in the ventral, dorsal and posterior of tongue. Reactive lesions such as PG had the most frequency in male, respectively, while in female reactive lesions such as IF had the most frequency respectively.Conclusion: IF, PG had the most frequency in compare to other reactive lesions in Tehran (Shahid Beheshti) and Qazvin dental faculties’ archive during the past 15 years, so Changes in life style and oral hygiene will help to prevent these damages.

Abstract: 

Background: Drug abuse is one of the important problems needs intervention in our country and may influence life and health of a person. One of the important functions and indicates the health of the individual is sleeping.Objective: To determine the effectiveness of multi-component cognitive behavioral intervention on quality of sleep and general health in drug abusers.Methods: This interventional study was conducted in 2015 in drug treatment centers of Qazvin. With consideration inclusion criteria and after preliminary interview, 30 male of treatment center were chosen available from one of the treatment centers of Qazvin and were randomly assigned in two groups. One of the groups received multi-component cognitive behavioral therapy and the control group received no intervention. Research tools were Pittsburg's quality of sleep and general health questionnaires that were filled by clients at baseline and after sessions of the intervention. Data were analyzed by method of analysis of variance differences.Findings: The results demonstrated that multi-component cognitive behavioral therapy significantly improved the quality of sleep and general health in drug abusers in intervention group comparing to control group (P=<0.05).Conclusion: According to findings cognitive behavioral therapy may improve health and quality of sleep in addicts.

Abstract: 

Background: Adipokines such as Vaspin and Nesfatin have different biological roles and may be in relation to cardiovascular and hypertension related diseases.Objective: To investigate the effects of 12 weeks aerobic exercises on plasma levels of Vaspin and Nesfatin in hypertensive elderly women.Methods: In this study, 30 elderly women with hypertension were randomly assigned to control and experiment groups. The training program included aerobic exercises which were conducted increasingly, 3 times a week for 12 weeks. Blood samples were taken before starting the exercise training and also 24 hours after the last training session. The data analyzed using the dependent and independent t-test for comparisons within and between groups.Findings: Number of control group was 15; Age 56.10±9.8 years; Systolic blood pressure 13.10±0.87mmhg; Diastolic blood pressure 7.89±0.56mmhg, number of experiment group was 15; Age: 55.10±6.08 years; Systolic blood pressure 13.28±1.36mmhg; Diastolic blood pressure 7.76±0.78mmhg. After 12 weeks significant changes in serum levels of the control group was observed, increase in Vaspin and reduction in Nesfatin but there was no significant change of the serum level in the experiment one.Conclusion: The results showed that the stimulation threshold is different in each adipokine compared to others and the 12 week aerobic exercise, has been prevented a significant increase of Vaspin or a significant reduction of Nesfatin.

Abstract: 

Background: Stress and negative emotions has a role in prediction type 2 diabetes mellitus and Self- efficacy effectiveness on commitment to self- management behaviors.Objective: To investigate predicting diabetes management self-efficacy base on hardiness and coping strategies in patients with type 2 diabetes mellitus.Methods: In this correlational study, patients with type 2 diabetes (57 mild and 59 severe) were selected by convenience sampling in diabetic outpatient clinics of Bu-Ali and Velaiat hospitals in Qazvin. Data were collected by demographical characteristics questionnaire and scales of Personal Views Survey (PVS), Coping Inventory for Stressful Situations (CISS), and Diabetes Management Self-efficacy Scale (DMSES). Then, data analyzed by T test and multivariate regression.Findings: Results showed mild patient with diabetes are significantly higher than severe patient with diabetes in commitment, challenge and control subscales and total of hardiness. Also, mild patient most used problem solving and avoidance coping strategies and severe patient most used emotional oriented coping strategies. Mild patient have higher score in total of diabetes management self-efficacy.75% of variance for diabetes management self-efficacy was predicted by hardiness and coping strategies. The total regression model explained that hardiness and emotional oriented coping predicted 0.43 and -0.36 of self-efficacy (P<0.001).Conclusion: Components such as commitment, control, challenge, effective coping strategies (decrease negative feelings and emotional regulation) shared with diabetes management self- efficacy in type 2 diabetes mellitus patients and must be paid attention to this factors for health counseling.

Abstract: 

Background: Escherichia coli (E. coli) is the major cause of urinary tract infections (UTIs) in hospital settings. Attachment to the target is the essential step to colonization of this organism in human host. S and A fimbriae are important virulence factors causing urinary tract infection in uropathogenic E. coli strains.Objective: To determine the frequency of S and A fimbriae encoding genes in uropathogenic E. coli isolated from patients admitted in educational hospitals of Qazvin.Methods: In this descriptive study, 126 E. coli isolates were collected from urine samples between 2012 and 2013. All isolates were identified using standard laboratory techniques and further evaluated for the frequency of sfa and afa genes using PCR and sequencing methods.Findings: In total, 37 (29.4%) and 13 (10.3%) isolates were positive for the presence of sfa and afa genes, respectively. The afa and sfa -positive isolates were mostly collected from patients admitted in intensive care unit (5 isolates, 4%) and internal medicine (23 isolates, 18.3%) wards, respectively.Conclusion: The findings showed a considerable rate of sfa and afa virulence factors in urinary E. coli isolates. According to clinical importance of this virulence factors in establishment and progress of urinary tract infection, using useful treatment strategy is essential for eradicating of this pathogen in studied hospitals.

Abstract: 

Background: Air quality and concentration of airborne particles in internal wards of hospital are considered as factors affecting the health of patients and medical staff.Objective: To determine the air quality and airborne particles concentrations in internal wards of Qazvin Bu-Ali Sina hospital.Methods: This cross-sectional study was carried out in Qazvin Bu-Ali Sina hospital from September to December 2015. Sampling gathered twice a week with hand held real time instrument (micro-dust pro. model HP3275-05, Casella Co), using by airborne particles samplers probe with a diameter of less than 2.5 and less than 10 microns in the air flow 3.5 liters per minute for 10 minutes. Air Quality Index was measured with "Air Quality Index calculator" software and analysis of results was done using statistical paired-T and Pearson tests.Findings: The highest average concentration of 24-hour airborne particles with a diameter of 2.5 and 10 micrometer was 46.38 and 13.49 micrograms per cubic meter respectively in men hearts ward and the lowest was in the laboratory and eyes wards. There was a positive significant correlation between the concentration of suspended particles with size less than 2.5 micrometers in indoor and outdoor environment (r=0.93).Conclusion: The findings indicate poor ventilation and impact of outdoor air quality and internal emission sources on internal wards air quality. Facilitating the spread and transmission of infectious agents grounds for infection transmission outside the hospital, in addition to endangering the health of staff and prolonging period of patients’ hospitalization.

Abstract: 

Background: Existence of coordinated and professional safety system to prevent occurrence of accidents and potential hazards seem to be essential in installing networks of gas distribution projects.Objective: To survey work-related accidents and safety performance indices in project implementation unit of Yazd gas agency.Methods: This analytical study was conducted on 197 of workforce in Yazd gas agency in 2013 that were selected by census and they were male. Demographic and accident information were gathered using a self-made questionnaire and face- to- face interview, and required information obtained from dossier to determine the safety performance indicators. Safety performance indicators were calculated in separately of 13 types occupations in project implementation unit of gas agency and data were analyzed using T-test.Findings: The highest accident frequency and severity rate were related to digging occupation and then metal line welding. Consequences of accidents were cuts (%56.7) and soreness (%14.9). The causes of accidents were related to uselessness of personal protective equipment (%25.2) and lack of precision in the task (%19.3). The highest rate of accident was observed among the age group 20-29 years with work experience of 4-6 years.Conclusion: According to the findings of this study and the risk of gas processes, it seems to be necessary the implementation of integrated management systems and training of workers about safety rules to improve the safety culture and prevent accidents.

Abstract: 

Background: Management of low back pain needs adequate clinical skills, fine utilization of imaging studies and their appropriate explanation, particularly in non-specific low back pains. Therefore, assessment of physicians’ knowledge in this area has a great importance.Objective: The present study was designed to determine knowledge of general physicians in private offices of Qazvin city, Iran in management of patients with acute or chronic low back pain according to the related guideline in clinical skills.Methods: This cross sectional study was conducted in 2014 at the Qazvin University of Medical Sciences, Qazvin city. One hundred fifty general physicians were selected as convenient sampling. The designed questionnaire which assess their knowledge in diagnosis, clinical findings, follow up, and treatment of low back pain was filled by them. The results were analyzed using student t-test and Pierson correlation test.Findings: Sixty physicians were male and 90 were female. Knowledge of physicians about patients’ follow-up has highest score with 65% correct answer. Knowledge about treatment and diagnosis with 46 and 44% correct answer have placed in the next rank, respectively. There was significant association between university of education of physicians with their knowledge of score (P<0.004). This association was not observed between knowledge with other variables.Conclusion: Results of the current study showed that the knowledge of general physicians is unsatisfactory in management of patients with low back pain. So, design and implementation of proper programs for training and retraining of the studying student and post-graduates is recommended to enhance the knowledge and proficiency of physicians in management of patients with low back pain.
