Abstract:

Background: Multiple sclerosis and ulcerative colitis are two chronic and disabling disease that cause many problems to patients. Until now there is not a good study to determine relation and similarity between this disease. The aim of this study is to compare the prevalence of ulcerative colitis in general population with MS patients. Methods: In this cross-sectional study from March 2008 to September 2009, 250 patients with MS were compared with 250 persons of general population without MS, to evaluate the prevalence of ulcerative colitis.Findings: The mean of age was not different in two groups. 2% (5 persons) of patients with multiple sclerosis had ulcerative colitis, but general population had not ulcerative colitis. This difference was clearly, significant (P=0.03).Conclusion: In this study as the other study reports, the prevalence of ulcerative colitis among patients with multiple sclerosis is more than normal population and it seems that some predisposing factors are responsible in two diseases. So more gastrointestinal evaluation should be done in MS patients with gastrointestinal sign and symptoms to rule out ulcerative colitis.

Abstract:

Background: Chronic low back pain is one of the most health problems worldwide. This study aimed to examine the effects of a designed multidisciplinary program on quality of life and disability among chronic low back pain patients up to 18 months. Methods: This study followed the participants in previous randomized controlled trial up to 18 months. Therefore, data at 18-month follow up, were collected through SF-36, Ronald –Morris Disability Questionnaire, and Quebec low back pain Disability Scale and then analyzed in five stages of 0, 3, 6, 12, and 18 months through repeated measure ANOVA.Findings: Repeated measures ANOVA showed that interactions between time and group were different in all subscales of SF-36 scale (all P £ 0.05).The disability of patients was decreased over time in intervention group that was significantly different from control group (P<0.01).Conclusion: The findings of this study suggest that the designed multidisciplinary program could improve quality of life and decrease disability even up to 18 months in chronic low back pain patients.

Abstract:

Background: Cesarean is one of the most common surgeries. Pain is on of it's side effect which bothers mothers.Opioids are the most common pain relief which used as Intra muscular, Intra venues, sub cotaneous or new methods such as patient control analgesia (PCA). As it is new method, research decided to compare efficacy of these 2 methods. Methods: This is a prospective clinical trial study which has done on primi parous with full term, single pregnancy who were candidate for elective cesarean in Sepahan shahr hospital of Isfahan in year 2010. Excluding criteria was using another drugs, essential disease, not to permit, dizziness, coma. Number of cases was 44 in each group which had been selected simple. In recovery and surgery ward patients were evaluated about vital signs- drugs side effects in 1, 2, 4, 8, 12 hours after surgery. Pain score evaluated by visual analog scale (VAS) by patients. Satisfaction of pain relief asked from patients.Findings: Petidin was significantly more effective for pain relief than PCA in1, 8, 12 hours after surgery (P<0.05). Mean of pain was less in 2, 4hours after surgery in Petidin Im group than PCA but not meaningful. Total Petidin was more effective than PCA for pain relief (P<0.02).Conclusion: This study showed although PCA is new method for pain relief but it is doesn't have efficacy such as old methods such as Intra muscular petidine. Also patient's satisfaction was less in PCA method. It may because of expensive and it's side effect such as nausea and vomiting.

Abstract:

Background: Children with cleft lip and palate experience many problems such as feeding problems, hearing disorders, and speech and language disorders. The purpose of this study was to determine the prevalence of middle ear diseases, hypernasality, and compensatory errors following primary palatoplasty in children with cleft palate with or without cleft lip; and to assess the effect of middle ear problems on speech production in these children. Methods: A retrospective file review of 120 patients with different kinds of orofacial clefts, who coming to Isfahan cleft palate team in Alzahra (SA) hospital in 2005 to 2007, was performed and the prevalence of middle ear problems, hypernasality and compensatory errors of 38 3-6 year old children with cleft palate with or without cleft lip was calculated. Moreover, the associations between the history of middle ear problems and hypernasality and compensatory errors were studied.Findings: 47.4% of patients experienced repeated episodes of otitis media. Also, compensatory errors and some degrees of hypernasality were observed in 78.9% and 71% of our sample, respectively. These speech disorders were not associated with middle ear status of children (P>0.05). But hypernasality was related to articulation disorders, significantly (P=0.001).Conclusion: In current study, the high prevalence of accompanying problems with cleft palate, especially hypernasality and compensatory errors were observed. Therefore in order to achieve better outcomes in children with cleft palate, we should improve our surgical treatment techniques and another intervention through appropriate multidisciplinary team management.

Abstract:

Background: To report toxoplasmosis in triplet.Case Report: We report toxoplasmosis in triplet patient. Two of them presented with congenital toxoplasmosis with chorioretinal manifestation. One of this two has recurrence of chorioretinitis after complete and successful treatment with triplet antibiotic therapy. One of this triplet has no ocular or systemic manifestation of congenital toxoplasmosis. So it may be a case of bichorionic triplet with congenital toxoplasmosis. Conclusion: Due to sparing of one of the triplet, toxoplasmosis infected the one chorion of this bi-chorionic triplet.
