Abstract: 

Introduction: Myocardial protection is one of the most important determinants of the result of CABG operation. The benefit of aminophylline has been proved in addition to antegrade cardioplegia but has not been studied plus Antegrade and retrograde cardioplegia. This study was designed to evaluate the effects of aminophyliine as a cardioprotective agent for patients undergoing on-pump CABG with antegrade and retrograde cardioplegia.Methods: In this double-blinded randomized clinical trial, 40 patients were randomly assigned to receive either IV aminophylline (5,g/kg) (n=20) or saline (n=20) before anesthesia induction. Troponin T level was measured 30 minuets, 2 hours and 24 hours after aortic clamping. For data analysis, t-test, chi-square and Mann-Whitney test was used. Results: Troponin T level was significantly lower in aminophylline group 30 minutes (P=0.03) and 2 hours (P=0.01) after aortic cross clamp. No difference was observed with clinical Parameters.Conclusion: Pretreatment with aminophylline reduces myocardial injury at sub-clinical level, particularly to benefit the ante-retrograde cardioplegia alone.

Abstract: 

Introduction: Methotrexate (MTX) is a chemotherapeutic agent that used for the treatment of a variety of tumors and inflammatory diseases. Methotrexate cause defective cogenesis, spermatogenesis, fertility impairment and different pathological changes on testis laboratory animals. The aim of this study was to investigate the effect of methotrexate administration on morphometerical changes in the testis of adult rat.Methods: In this experimental study 40 male wistar rats were selected and randomly divided into four groups. Experiment groups were received 1mg/kg MTX (IP) per week for two and four consequence week, respectively. However, control groups just received normal saline (IP) for 14 and 28 consequence days, respectively. The testis were sampled after passing of above times and stained by H & E technique. Then diameter of seminiferous tubules, thickness of interstitial tissue, and thickness of testicular capsule, were studied. Data were analyzed by analysis of variance. Results: Our study showed that methotrexate had destructive effects on testis tissue and spermatogenesis in test group compared to control group. Results showed that within 28 days after methotrexate administration diameter of seminiferous tubules and thickness of epithelium of seminiferous tubules significantly decreased (P<0.05), but thickness of interstitial tissue and thickness of testicular significant increased (P<0.05) compared to controls.Conclusion: Results showed that methotrexate administration causes decline in spermatogenesis and significant changes in diameter of seminiferous tubules, thickness of epithelium of seminiferous tubules, thickness of interstitial tissue, and thickness of testicular capsule in rat testis tissue. These changes may via the reducing of cell divide and toxic effects of methtrexate.

Abstract: 

Introduction: Thalassemia is an inherited disease which leads to an imbalance of globinchain synthesis and consequently, ineffective hematopoiesis. Iron deposits in many important body organs, as endocrine organs due to blood transfusion or ineffective hematopoiessis. The aim of the present study is to assess the endocrine disorders in the patients with thalassemia intermedia referred to the thalassemia center of Bandar-Abbas, Iran.Methods: Eighty-six patients with intermediate thalassemia attended the thalassemia center of Bandar-Abbas were enrolled in a cross-sectional, descriptive-analytic study. They were evaluated for demographic characteristics, past medical history, familial history and physically examined for sexual maturity and thyroid gland weight assessment. Then blood samples were taken to measure thyroid, parathyroid and sexual hormones. Fasting blood glucose, magnesium and ferritin levels were checked and were analyzed by SPSS 16.0. Results: There were 47 males (54.7%) and 39 females (45.3%). Frequency of hypoparathyroidism was 18.6%, diabetes mellitus 8.1%, sub-clinical hypothyroidism 4.65%, hypo-phosphatemia 29.06%, and hypo-magnesemia was 1.16%. Delayed puberty was found in 6.07% girls and, 4.7% boys. Primary amenorrhea was seen in 15.4% of female cases. Moreover, there was not a significant difference in hemoglobin rate between hypoparalhyaism and normal cases (P<0.05).Conclusion: It's recommended to evaluate the patients' suffering from thalassemia intermedia for diabetes mellitus, thyroid dysfunctions, parathyroid abnormality, and sexual maturity routinely.

Abstract: 

Introduction: The aim of this study was to determine the effects of Insulin and Lascorbic acid on prevention of neuronal in granular layer of dentate gyrus and pyramidal layer of CA3 of hippocampus in streptozotocin-Induced diabetic rats. Methods: In this experimental study, diabetes type 1 was included by a single intraperitoneal injection of streptozotocin (STZ) (60mg/kg) in male adult wistar rats. After six weeks, STZ-injected rate were divided randomly into four groups as follow: (1) treated with Insulin (2) treated with Insulin + ascorbic acid (3) treated with ascorbic acid, and (4) untreated diabetic. After two weeks the brains were removed. Numerical density of TUNEL positive neurons in granular layer of the dentate gyrus and pyramidal layer were counted. Ultra structure of the injured neurons was studied.Results: Our findings showed diabetes type 1 caused increased numerical density of TUNEL positive neurons in the dentate gyrus, pyramidal layer of CA3. Ultra structure showed server chromatin condensation, cytoplasmic radio-opacity and preserved nuclear membrane integrity indicative of apoptosis. In treated diabetic groups with Insulin, Insulin + ascorbic acid and ascorbic acid, numerical density of TUNEL positive neurons in the granular layer of the dentate gyrus and pyramidal layer of CA3 were lower than those of untreated group (P<0.001).Conclusion: It seems that, treatment of diabetic rats with insulin, ascorbic acid and combined treatment could prevent neuronal apoptosis.

Abstract: 

Introduction: Psychiatric disorders and function of child and adolescent offsprings of bipolar parents were compared to that of their peers with healthy parents. Methods: In this case - control study, 52 bipolar parents along with their 72 children were compared with 48 healthy and their 63 children. The subjects were selected by simple and available methods. K-SADS, C-GAS were to collect data. Statistical analysis was performed using Chi-square and t tests.Results: Relative frequency of psychiatric disorders (at least one disorder) was 51.4 perecent (38 people) in cases and 20.6 perecent (13 people) in controls. The difference was statistically significant (P<0.001). Both groups showed significant differences in mood and anxiety disorders as well as ADHD. According to C-GAS questionnaire the level of function was higher in the control group.Conclusion: Psychiatric disorders and impairment of behavioral functioning among the offspring of bipolar parents were higher that the controls.

Abstract: 

Introduction: Although exercise his effects on human immune system, but its causes is not significantly cleared yet. This study is aimed to investigate the effect of exercise with moderate and high intensity, on changes of respiratory burst activities and neutrophil counts in college judoists.Methods: 10 male college judoists participated in this quazi-experimental study. Subjects run on a treadmill for 60 min at 45% (low-intensity exercise) in first week and in second week run with 75% (high-intensity exercise) heart care. Blood was drawn immediately before and after exercise. Paired t-test was used for data analysis by means of SPSS 11.5 and P<0.05 was the significance level.Results: The result showed that, increase in neutrophil count and neutrophil respiratory burst activities, after high-intensity exercise, is significant (P<0.05), whereas, this was not observed significantly after low intensity exercise. Conclusion: An increase in neutrophil count and respiratory burst activities were significantly associated with high intensity exercise, however, the effect of low intensity exercise was not significant.

Abstract: 

Introduction: Abuse in pregnancy could be a risk factor for poor pregnancy outcomes. Abuse during pregnancy effecting mother may impair the health of fetus directly or indirectly. This study was designed to determine the relationship between abuse in pregnancy and pregnancy outcome in pregnant referred to Gachsaran hospital, Iran. Methods: This descriptive study was conducted on 500 pregnant women attending Gachsaran hospitals chosen by convenience sampling. A questionnaire consisting of demographic characteristics of samples and their partners, abuse screening and information of pregnancy outcome was administered. Validity and reliability of tool was respectively done using content validity and test retest. By using SPSS software, t-test, exact Fisher test, Mann Whitney and Chi-square tests were used for statistical analysis.Results: The results showed significant correlations between pregnancy abuse and maternal (only abortion and preterm birth) and fetal outcome such as and fetal death (P<0.05). Conclusion: We showed that the necessity of care to women health, PROM, low birth weight, especially during pregnancy, and a decrease in abuse improves adverse outcome on pregnant women. Routine screening program to eliminate pregnancy abuse is required.

Abstract: 

Introduction: It is believed that between 50% to 80% of pregnant women, suffer from nausea and vomiting during pregnancy. Nausea and vomiting of pregnancy decrease the ability of functioning, where the degree of this limitation is associated with severity of the symptoms. The aim is to assess the effect of body mass index (BMI) on the tendency of pregnant women to vomit and the effect of the severity of vomiting on the quality of life.Methods: In this analytic study, 140 pregnant women during their first 12 weeks of gestation who came to hospital for prenatal care were recruited by random sampling. Demographic and gestational variables as well as body mass index (BMI) were recorded. Women were requested to report the number of vomiting episodes, the intensity of vomiting and the volume within the last 12 hours. Patients were allocated to mild group (3-6 grades), moderate and severe (grades over 7). We used the health related quality of life for nausea and vomiting of pregnancy. Data were analyzed using t & chi-square test. Results: The BMI was significantly lower in the moderate and severe groups (21.91±2.76 versus 23.77±3.32, respectively, P<0.05). Patients in the moderate and severe groups reported a lower quality of life as compared to the mild group (P<0.05). Conclusion: Patients with lower BMI in pregnancy showed higher frequency vomiting and the severity of vomiting was associated with lower quality of life.

Abstract: 

Introduction: Postpartum depression is the most common serious mental disorder in the postpartum period. Postpartum depression is associated with serious complications for mother, baby and her family. This study is designed to investigate the effect of antenatal education on pregnancy depression in theses who referred to Dezful health canters. Methods: This case-control study was carried out on 140 non depressive pregnant women with gestational age between 26-30 weeks. These women were randomly divided into two groups, experimental group and controls of Antenatal education classes. The research data was collected using standard questionnaire, the Beck Depression Inventor and the Edinburgh Postnatal Depression Scale. The data was analyzed using t and chi-square tests.Results: Result showed that 31.4% of women had the Edinburgh Depression Score of 10 or more. A significant increase in the mean score of the Edinburgh Postnatal Depression Scale in controls compared with the experimental group (P<0.001) was observed. Furthermore, attendance in the antenatal education decreased the likelihood of the Postnatal Depression {OR= 2.7, 95% CI (1.4-4.10)}.Conclusion: Findings showed that the antenatal education decrease the Edinburgh Postnatal Depression Score significantly, hence the screening of depression in pregnancy and antenatal education is recommended.

Abstract: 

Introduction: Given the role of the medical record committee, that is, supervision and quality evaluation of medical records and support of required standards in relation to medical records, a study about current situation of hospital medical record committees was conducted.Methods: This study was an applied research. An evaluation pattern for medical record committees was designed to study the similarities and differences of structure and activities of medical record committees in selected countries and Iran. Then the designed pattern was distributed among experts.Results: Delphi Technique showed, that most of the experts (86.4% frequency) agreed with the components of the model as following: 97.2% with purpose, 63.9% with members' combination of committee, 94.4% with inferior reference, 97.2% with standards, and 91.7% with job description for activities of medical record committee.Conclusion: With respect to deficiencies in medical records instructions and agreement of experts with the proposed pattern, ministry of health and medical education could establish new instructions including comprehensive structure and activities and distribution to all hospitals.
