Abstract:

Introduction: The preferred method in anesthesia of the patients who are candidate for gynecology diagnostic laparoscopic surgeries is general anesthesia (GA) usually, but in patients with any contraindication for GA, Spinal Anesthesia (SA) is used. This study was performed with aim to compare GA with SA technique for gynecology diagnostic laparoscopic surgeries. Methods: This clinical randomized and prospective study was performed on 30 patients who were candidate for gynecology diagnostic laparoscopic surgeries in 2012-2013. The subjects were divided in two groups of GA & SA (n=15 in each group). Data collection tools were checklist and observation. SPO2, MAP, RR, HR, ETCO2 were recorded preoperative and intraoperative. Nausea, vomiting and shoulder pain were evaluated in recovery, 2, 8 and 24 hours after surgery. Data were analyzed by SPSS software (version 15), and using T-test and Chi-square. P<0.05 was considered significant. Results: The changes of SPO2 and HR at the studied times were not significantly difference between two groups (p=0.141, p=0.531, respectively), and only EtCO2 after Inflation (p=0.015) and RR and EtCO2 during inflation (p=0.004, p=0.013, respectively) were significantly different between two groups. MAP in all the state in the SA group was lower except before inflation. The incidence of shoulder pain at 2, 8 and 24 hours after surgery had significant difference between two groups (p=0.009, p=0.035 and p=0.011, respectively); so that it was higher in the SA group. Nausea in the recovery had higher incidence in SA group (p=0.024).Conclusion: The use of SA for diagnostic laparoscopic surgeries in women is safe and reliable and like as any other anesthesia method has benefits and disadvantages.

Abstract:

Introduction: Osteoporosis is the most common metabolic bone disease that could causes many dangerous fractures. In this condition bone mass reduces, so fractures can easily occur. The aim of this study was assessing the effect of eight weeks of aerobic training on serum parathormone, estrogen and Alkaline phosphatase concentration in Women with premature menopause. Methods: Methods: In this study semi-experimental study, in 2014, 21 women with premature menopause (age 35-45 year, body mass index (BMI>30 kg/m2), height: 161.19±2.64 and weight 83.97±1.98) were selected by convenience sampling method and they were randomly divided into two groups experimental (n=10) and control (n=11). The subject in experimental group had an aerobic training program for eight week (3 sessions in week, each session 45-60 minutes, the intensity of exercise was 65 to 75 percent of maximum heart rate). Blood samples were taken from the subjects’ 24 hours before the first session and 48 hours after the last training session at end of eight week in order to measure factors such as (parathyroid hormone (PTH), estrogen and some bone metabolism) by using a radioimmunoassay (RIA) and ELISA method in 12 hours of fasting. Then Data for inter and between group comparison, paired and ANCOVA test were used (significance level p<0.05).Results: The result of this study show that, eight weeks aerobic training lead to a significant increase in mean values of serum calcium (p=0.001), phosphor (p=0.001), estrogen (p=0.001), parathormone (p=0.001) in aerobic training group; while, the amount of alkaline phosphatase did not change at the end of period (p=0.92).Conclusion: Eight weeks aerobic training with 65 to 75 percent of maximum heart rate intensity was efficient in increasing of serum calcium, phosphor, estrogen, parathormone in women with premature menopause. It is concluded that regular training can help to increase bone density and reduces the risk of fractures in women with premature menopause.

Introduction: Leg cramp is associated with high levels of blood glucose, triglycerides, blood pressure, obesity and decreased HDL in middle-aged women that is associated with painful contractions, which leads to disorder of walking and health in the individual. Therefore, due to these disorders, this study was performed with aim to investigate the effects of calf muscles and hamstrings stretching training on pain severity and leg cramps in women with metabolic syndrome. Methods: This semi experimental study was performed by pretest-posttest with control group in 2015 on 24 patients with metabolic syndrome in Fouman city. After the measurement of leg cramps and metabolic syndrome variables, the subjects were randomly divided into two groups of experimental and control. Stretching training was performed for 6 weeks, 5 days per week for 45 to 50 minutes. The intensity and frequency of leg cramps were assessed by visual analogue scale. Data was analyzed by SPSS software (version 20) and dependent t test and covariance analysis. P<0.05 was considered significant. Results: Stretching training significantly reduced the intensity and frequency of leg cramps (p<0.05); while the changes of pain severity was not significant between two groups (p>0.05). Also, the changes of metabolic syndrome indices and anthropometric measurements before and after training was not significant the experimental group (p>0.05).Conclusion: 6 weeks of stretching training can affect the leg muscles cramps and so can be useful in improving of leg cramps indices (intensity of frequency). Therefore, according to the results, women with metabolic syndrome can use stretching training for possible prevention of leg muscles cramps disorders and reducing its complications in daily life.

Abstract:

Introduction: Severe vaginal bleeding after postpartum is an emergency in obstetrics and if not treated appropriately associated with high mortality. Uterine artery embolization is a minimally invasive procedure and effective method in addition to control bleeding, moreover may also preserve fertility. However, this method is also associated with complications such as ischemia and uterine rupture. The aim of this article is to report of a case of necrosis and rupture of the uterus following uterine artery embolization.Case report: A 23-year-old woman with clinical presentation of massive postpartum hemorrhage was referred. She had history of twice Cesarean section and suffered severe vaginal bleeding after last surgery. Uterine artery embolization was performed and following it, because of repeated and uncontrolled vaginal bleeding, inevitably explorative laparotomy was performed. During surgery due to rupture of the myometrium and the impossibility of its restoration, she underwent hysterectomy. Pathology report confirmed the diagnosis. Conclusion: Despite the effective beneficial outcomes of uterine artery embolization, due to its complications, proper indication and careful attention should be considered.
