Abstract: 

Background and Objective: The findings of various research indicated that there are different responses to painful thermal stimulation in male and female Rats. In different estrous phases there is different pain thershold border line due to hormonal reaction. Regarding to anti-nociceptive effect of henbane seed. This study was designed to assess the anti-nociceptive afficacy of alcohole extract of henbane seed in different phases of estroes cycle of female Rates. Materials and Methods: In this experimental study 92 female NMRI strain rats weighing 195- 220 grams randomly were divided into three, control, experimental and positive groups. Male Rats (n=8) were chosen as control group. To measure the threshold of acute pain, all animals were exposed to the tail immersion test using 52°c water. In the experimental group, alcoholic extract of henbane seed (2000 mg/kg) was injected, intraperitoneally, 25-30 minutes before the tail immersion test. Afterwards, in the all of control and experimental groups of female rats the smear samples were taken to determine the sexual phases. In addition, the analgesic effect of henbane was compared with anti-nociceptive effect of sodium salicylate (300 mg/kg). Results: Injection of alcoholic extract of henbane seed, diminished the tail immersion testinduced acute pain in female rats significantly (P<0.05). Also, no significant differences of analgesic effect of henbane extract were seen among various phases of estrous cycle in female rats. There are no significant differences of analgesia between henbane and salicylate-treated groups.Conclusion: This study determined that female rats in different phases of estrous cycle have similar pain thresholds and the alcoholic extract of henbane seed is effective in the relief of pain in the various phases of estrous cycle.

Abstract: 

Background and Objective: Unsaturated fatty acids such as turpentine oil can decrease serum leptin level. In regard to this effect and the key role of thyroid hormones and leptin in metabolism. This study was designed to investigate the impact of dietary wild pistachio oil on serum leptin concentration and its relationship with thyroid hormones in experimental hyperthyroidism in male rat.Materials and Methods: In this experimental study thirty white Sprague Dawely adult male rats were divided randomly into the five groups with six rats in each. The first group as a natural control I received only ordinary diet and water during the study period. The second group as a control II received ordinary diet in addition to administration of 12mg Sigma levothyroxine in one liter of water daily for one month. Third, fourth and fifth groups with administration of considered dosage of levothyroxine were received respectively 5%, 10% and 20% concentrations of wild pistachio oil for one month. Blood samples were obtained at 10-day intervals. T4, T3, fT4, fT3 and leptin were measured by RIA and ELISA methods. Serum lipid profiles were measured by enzymatic method.Results: Thyroidal hormones, lipids and leptin level did not show any statistically significant differences between experimental days in the rats of control group I. The serum concentrations of T4, T3, fT4, fT3 in third, T4 in fourth and fifth groups showed significant differences(P<0.05). The serum leptin concentration decreased significantly during the experiment in the third, fourth and fifth groups (P<0.05), but serum lipids did not show any significant differences between various days of experiment. Conclusion: This study showed that the level of T4, T3, fT4, fT3 and HDLc / LDLc ratio at 30th day of study in case groups compaing to controls were decreased and increased, respectively.

Abstract: 

Background and Objective: Biological activity of hydrogen supplied in smooth muscle of vessel and non-vessel tissue are contradictory. The aim of this study was to examine the effect of hydrogen sulphide on smooth activity of muscle mice jejunum.Materials and Methods: Experiments were performed on mice jejunum and motor activity was recorded from in vitro segments of jejunum ~ 4 cm in length. jejunal segments were mounted horizontally in separate perfusion chamber. Furthermore, using vanilloid receptor 1 deficient mice (VR 1-/-) we tested hypothesis that extrinsic sensory nerves mediated alterations, in motor activity responses in the presence different concentration of of hydrogen sulphide (100-3000 mM).Results: Serosal application of NaHS (as hydrogen sulphide donor) produced a dose-dependent inhibition of motor activity that are not significantly different between VR 1-/- and VR 1 +/+ mice. In the presence of TTX (1mM) NaHS (300mM) caused a reduction in basal tone (19.5%, p<0.05, n=5) and inhibited the contraction evoked by 30mM bethanechol by 55% (n=5, p<0.05). Conclusion: This study showed that hydrogen sulphide is an important motor activity inhibitor in mice jejunum.

Abstract: 

Background and Objective: Sexual function is an important aspect of quality of life, and reflects on the physical, psychological and mental well being of the individual. The aim of this study was to compare the effects of Tibolone on sexual function, in healthy postmenopausal women.Materials and Methods: This randomized clinical trial study was done on 96 postmenopausal women with no absolute contraindication for hormone replacement therapy. Women were divided into two groups: 52 women in Tibolone group received Tibolone 2.5mg/day + Ca-D (500 mg +200 IU) and 44 women in placebo group received only Ca-D (500 mg +200 IU) for six months. Blood samples were collected at baseline and after 6 months of treatment for hormone determinations. Sexual function was assessed by female sexual function index (FSFI) in six domains. Side effect and satisfaction was assessed. Data was analyzed by Paired and independent t-test, Chi Square and Mann-Whitney tests.Results: 41 women in Tibolone group and 40 women in placebo completed the study. Compare to baseline values and placebo, the Tibolone group significantly improved on all of six FSFI domains (P<0.05). Testosterone, FTI and FEI increased and sex hormone binding golobulin (SHBG) decreased about 57% in Tibolone group (P<0.05). Headache, weightening, vaginal spotting and bleeding, were observed in case groups.Conclusion: Tibolone significantly improved all aspects of sexual function and increased quality of life by desirable changes in level of sex hormones and SHBG in Post menopausal women.

Abstract: 

Background and Objective: Cervical stabilization training is a method of exercise which is designed to improve the mechanisms by which the cervical spine maintains a stable, injury-free state. The aim of this study was to compare effectiveness of muscle stabilization training with dynamic exercises on the chronic neck pain and disability.Materials and Methods: In this double-blind clinical trail study twenty-six patients with chronic neck pain were recruited. Subjects were randomly assigned to either a stabilization (n=13) or a dynamic exercises group (n=13). Before and after intervention, pain was assessed with visual analog scale (ordinal) and Northwick Park Neck Pain Questionnaire (NPNPQ), disability (ordinal) with Neck Disability Index (NDI), neck muscles strength (bar) using Dynatest and cervical ranges of motion (degrees) with specific neck goniometer. A 24 session exercise program which lasted 12 weeks, two sessions per week, and 45 minutes per session was performed for both groups. Independent t-test or Mann-Whitney U and paired t-test or Wilcoxon were used for comparison between the pretreatment and post treatment test results between groups and within groups, respectively.Results: The mean neck pain (NPNPQ) decreased from 18.23±0.77 to 7.54±4.39 in the stabilization group and from 18.31±3.99 to 11.85±3.89 in the dynamic group (P<0.05). The mean disability (NDI) decreased from 22.69±4.99 to 8.23±4.09 in the stabilization group and from 22.23±4.88 to 14.92±5.54 in the dynamic group (P<0.05). After treatment, neck muscles strength and range of extension increased and both neck pain and disability decreased in the stabilization group compared with the dynamic ones (P<0.05). However, there was no significant difference between two groups regarding flexors and left lateral flexors strength and ranges of flexion, right and left lateral flexion.Conclusion: This study showed that neck specific stabilization and dynamic exercises increase, range of motion, decrease pain and disability. Also specific stabilization exercies was more useful than dynamic procedure.

Abstract: 

Background and Objective: Alpha-I antitrypsin deficiency is recognized as a etiological base in lung injury. Therefore this study was performed to determine plasma level of alpha-l antitrypsin in war victims exposed to sulfur mustard gas. Materials and Methods: This historical cohort study was done on 100 incident was survivors from Sardasht, West of Iran who were exposed to sulfur mustard gas in 1987. Fifty non exposed civilians subjects were selected as control. Phonotype and of alpha-I antitrypsin and trypsin inhibitory capacity (TIC) were measured. Results: Phonotype of alpha-I antitrypsin in patientsand control group were normal (MM)and the mean of trypsin inhibitory capacity in patients group was 3.4±0.3 mmol/min/ml which lower than control group 4.2±0.1 mmol/min/ml (P<0.05). Conclusion: This study showed that activity of alpha-I antitrypsin in patients with lung injuries reduced due to sulfur mustard gas.

Abstract: 

Background and Objective: Cancer is third cause of mortality and morbidity in Iran, after heart diseases and car accidents. This study was aimed to determine the incidence rate gastrointestinal tract cancer during 1998-2001 in Tehran, Iran. Materials and Methods: In this descriptive study, the whole population residing in areas of Tehran, Shemiranat and Eslam'shahr area were supposed to be as the population in risk. Data of cancer recordings were collected from pathology and medical records archives. Those with ICD- 10-based diagnosed cancer during 1998-2001, who have been inhabited in above area for at least 1 year, were supposed as canceric cases. The incidence rate of various kinds of cancer was adapted based on age-structure of world population and were calculated by dividing the total rate of this period to 4 in an annual manner.Results: The annual age-specific standardized incidence rate of gastric cancer in men and women were 12.9 and 5.8 per 100000, respectively. The row incidence rate in men and women were 9.2 and 4.1, respectively. Ealderly age groups of both men and women had higher incidence rate of gastric cancer. The annual age-specific standardized incidence rate of colon cancer in men and women were 5.7 and 5.4 per 100000, with row incidence rate of 4.3 and 4.0 per 100000, respectively. The annual age-specific standardized incidence rate of esophageal cancer in men and women were determined as 5.1 and 3.9 per 100000, with row incidence rate of 3.7 and 2.7 per 100,000, respectively.Conclusion: This study showed that gastric cancer is the most common type of gastrointestinal cancers in Tehran. Therefore it is suggested the related risk factors in this area should be determined.

Abstract: 

Background and Objective: Sexual satisfaction is a significant factor in prevention of sexual disorders among women. There are many factors associated with sexual satisfaction. The aim of this study was to determine some factors related to women sexual satisfaction in Gachsaran, in South-West of Iran.Materials and Methods: This descriptive analytic study was done on 1014 women which refered to health centers in Gachsaran city during Jan-Jun 2008. Data was collected by self– reported questionnaires and analyzed by Chi-Squre and Pearson tests.Results: The scale of sexual satisfaction were 0.4%, 10.9%, 47.5% and 41.7%; unsatisfied, low, moderate and fully satisfied, respectively. There was a correlation between sexual satisfaction and women age, couple age difference, length of marriage, family economical condition and contraceptive method (P<0.05). Conclusion: This study showed that, woman age , couples age differences, length of marriage, couple economic condition, and contraceptive method were related to sexual satisfaction. These factors should be taken into consideration in all marriages and sexual consultation.

Abstract: 

Background and Objective: Aircraft as a safe means of transportation may cause occupational diseases and hearing loss. Prevention should be implemented for airport employee. This study was done to detemine the scale of hearing loss among Isfahan airport employees in 2005. Materials and Methods: This historical cohort study was conducted among Isfahan International airport employees. 80 employees were selected in four different case groups, the control group consist of two sub-groups. The sub-group I (18 subjects) including the administrative airport employees (low noise exposed) and the sub-group II (32 subjects) including non-airport employees (non-noise exposed). The sound pressure level was measured and equivalent level (Leq) was calculated for all groups. Also the history of participants about past noise exposure and other confounding variables was detrmined by a questionnaire. Pure tone audiometry was carried out on conventional frequencies (0.5-8 KHz). Results: The mean age of subjects was 40.03±9.75 and 37.85±8.15 for exposed and non-exposed groups respectively. Leq (noise equivalent level) for Ramp and traffic workers was estimated more than 95 dBA. 35.7% (n=10) of Ramp workers were suffering in their right ear from noise induced hearing loss (26-40dB) and 32.1% (n=9) of them in their left ear. Relative risks of noise induced hearing loss in Ramp workers in comparison with control group were 9.4 and 7.5 for right and left ears. A significant difference was found between the hearing thresholds of exposed and non-exposed groups (P<0.05).Conclusion: This study showed that occupational exposure to noise cause hearing loss among airport employee. It is suggested strategies of noise assessment and prevention should be implemented for airport employees.

Abstract: 

Background and Objective: The Acute Physiology and Chronic Health Evaluation II classification system has been extensively used for predicting the patient mortality in various diseases admission in ICU ward. The aim of this study was to assess the predicting mortality rate of patients in critical care unit using APACHE-II index.Materials and Methods: This descriptive analytial study was done on 204 hospitalized patients with multiple diagnostic diseases admitted to ICU ward in Gazvin Kusar hospital during 2007. We used than demographic questionnaire and APACHE-II tool to evaluate the prediction mortality rate.Results: Out of 204 patients admitted, 147 (72.1%) survived. Mean APACHE-II score of the subjects was 11.38 (with arrange of 0 to 41). The predicted death rate was 30.26% and the observed death rate was 27.9%. There was a perfect correlation between APACHE-II score and predicted death rate (r=0.976 and P<0.05). Mean APACHE-II score in survivors and nonsurvivors subjects was 13.97 and 25.36, respectively (t:9.93; P<0.05).Conclusion: This study showed that APACHE-II score can truly predict mortality rate in patients in ICU ward. In addition, this index is helpful in follow up treatment cycle in critical care patients.

Abstract: 

Background and Objective: Acute appendicitis is one of the most common surgical emergencies. Preoperative diagnosis of acute appendicitis remains challenging, despite improvements in history taking and clinical examination. Methods advocated to assist in the diagnosis of appendicitis include laparoscopy, ultrasonography, computed tomography and magnetic resonance imaging. The purpose of this study was to assess the accuracy of ultrasonography in the diagnosis of acute appendicitis.Materials and Methods: This prospective descriptive study was done to evaluate 265 patients suspected to have acute appendicitis and admitted to the emergency department of 5 Azar hospital of Gorgan from Aug 2005 to Aug 2006. Suspected patients visited by the general surgeon and ultrasonography of the appendix was performed subsequently. The ultrasonography examination was done using high graded-compression ultrasongraphy with 5 MHz variable focus linear array transducer. The sonographic data were compared with clinical, operative and pathological findings.Results: The sensivity and specifity of ultrasonography was 66% and 4%, respectively. Also positive and negative predictive value of ultrasonography was 98% and 40%, respectively. Conclusion: This study showed that ultrasonography is a useful method in the diagnosis of acute appendicitis. It also increase diagnostic accuracy of acute appendicitis and minimize negative laparotomy and prevent unnecessary appendectomy.

Abstract: 

Background and Objective: Irritable bowel syndrome (IBS) is a disturbance of colonic function characterized by abdominal pain or discomfort, bloating and abnormal bowel function, resulting in episodes of chronic diarrhea, chronic constipation, or both in alternation. It affects approximately 3 to 20 percent of the population. The aim of this study was to investigate the prevalence of Irritable bowel syndrome among the high school pupile in Gorgan, in North of Iran.Materials and Methods: In this descriptive-analytical cross sectional study, data was collected by clustering, quota and randomized sampling. 1080, 15-19 years old male and female high school pupiles, residing in Gorgan participated in the survey during 2007. The data collection tool was a demographic questionnaire and standard Rome-II criteria.Results: 1063 pupiles completed the questionnaire. The Irritable bowel syndrome prevalence according to Rome-II was 5.7%. The relation between ethnicity and Irritable bowel syndrome was significant (P<0.05). The rate of Irritable bowel syndrome was higher among the Turkmen students. There was no significant relation between Irritable bowel syndrome prevalence with gender, age and family income.Conclusion: This study showed that irritable bowel syndrome prevalence in this area is nearly similar to that reported in national studies in western Iran as well as with Asian countries.

Abstract: 

Background and Objective: Jaundice is a common problem among neonate undiagnosed case finally lead to kern-icterus, with significant increasing rate of subsequent morbidity. This study was done to determine the etiology of of severe indirect hyperbilirubnemia in term neonates. Materials and Methods: This cross sectional descriptive study was performed in Taleghany hospital of Gorgan on neonates admitted due to severe hyperbilirubinemia from Sep 2004-Sep 2005. Severe hyperbilirubinemia was considered as bilirubin³18 mg/dl in term neonates weighing more than 2500g. Bilirubin (total, direct), blood culture, retic count, coombs test, level of glucose 6 phosphate dehydrogenase enzyme, complete blood cell count, mother's and neonate's blood group, urine culture and C-reactive protein tests were measured. Results: 766 term neonates (>2500g) were hospitalized due to hyperbilirubinemia. Severe hyperbilirubinemia was detected in 12% of cases (54 boys, 38 girls). The etiology of 41 cases were unknown, 25 cases were diagnosed as sepsis, 15 neonates were glucose 6 phosphate dehydrogenase enzyme deficient, 7 cases had UTI, 3 neonates had ABO incompatibility and one neonate was due to breast feeding. Conclusion: This study indicated that the most common etiology of severe hyperbilirubinemia in this region is unknown. Sepsis, glucose 6 phosphate dehydrogenase enzyme deficiency, UTI, ABO incompatibility and breast feeding were among other reasons for severe indirect hyperbilirubinemia.

Abstract: 

The prevalence of struma onset in uterine tube is lower than ovary’s stuma. In this report a 63 year old woman was referred to main hospital in the city of Yazd, Iran in 2008. The patient referred with abdominal swelling in pre-umbilical and supra pubic region associated with losing weight and frequent constipation since 5 years ago. After the hospitalization the patient underwent the laparatomy. in pathology report the masses was struma ovarii and mature cystic teratoma associated with struma salpingi and peritoneal steremosis. Special demonstration accompanied struma salpingi in this case are struma ovarii and multiple vesicular vegetasion in choledosac, Peritoneum and bladder. This report was a very rare case of struma salpingi accompanied with struma ovarii and steremosis.
