Background and Objectives: The multi-drug resistance phenomena can limit the effect of chemotherapy and lead to the recurrence of leukemia. One of the main mechanisms of multi drug resistance is the increased expression of MDR1 gene that codes P-gp, a transmembrane carrier that exports drugs out of the leukemic cells. The aim of this study was to explore the effect of resveratrol, a natural compound, on the expression of MDR1 gene in leukemic cell line MOLT-4.Material and Methods: MTT (Methyl Tiazol Tetrazolium) assay was used to determine the subtoxic doses of resveratrol for treatment of leukemic cells line real Time PCR was used to determine the expression of MDR1 gene. MOLT-4 cell line resistance was assessed after MTT implementation. Results: Resveratrol could not suppress the growth of MOLT-4 cells. Genetic studies revealed increased expression of MDR1 gene. Resistance of MOLT-4 cells to vincristine was increased after coadministration of vincristine and resveratrol. Conclusion: we did not find evidence that resveratrol can reverse multi drug resistance in MOLT-4 cells.

Background and Objectives: exploration of factors associated with re-infection of Mycobacterium tuberculosis can be helpful in prevention of disease and reduce the burden of tuberculosis on health care system. This study was designed to identify factors associated with re-infection of tuberculosis in patients with sputum smears positive pulmonary tuberculosis in five bordering provinces of Iran.Material and Methods: In this cross sectional study that performed of 2010-2012, different and clinical, socio economical factors were evaluated to identify influencing factors on re-infection. Data were gathered from questionnaires and survey of medical records. Data were analyzed by statistical methods. Results: In 300 studied patients with average age 51.9±24.2 years. 7.2% of patients were diagnosed with re-infection. 64% of patients were in low-income classes, (10.6%) of patients had concomitant renal diseases, 10.2% had diabetes, and 9.8% of patients had liver dysfunction. In multiple analysis, migration from rural to urban areas (P=0.31) and weight loss (P=0.42) were significantly associated with increased risk re-infection.Conclusion: Compared to global reports recurrence rate in our cohort was in a moderate degree. Future studies should be done to identify the influential environmental and socioeconomic factors that affect the M. tuberculosis in re-infection.

Background and Objectives: Infusion of large volumes of normal saline is recommended for facilitating graft function during kidney transplantation surgery but it can cause severe metabolic acidosis. Here we studied the effects of intraoperative rapid normal saline infusion on vascular anastomosis, acid base, electrolytes and graft function.Materials and Methods: In this study 30 patients were evaluated during kidney transplantation. Infusion of normal saline was started and increased from 10 ml/kg/h to 50 ml/kg/h during vessel anastomosis. Acid base balance electrolytes and anion gap measured at the beginning and end of surgery serum creatinine and urine output was measured up to 48 hours after surgery. Data analyzed was performed by statistical method. Results: Serum creatinine at 24 and 48 hours after surgery were 3.04±1.59 and 2.22±1.53 mg/dl respectively. PH change during the surgery was not significant (p=0.818) but serum chloride increased from 94.23±3.95 to 99.37±3.84 (P<0.001). Base excess and anion gap decreased respectively from -3.55±3.47 to -8.40±2.65 (P<0.001) and from 30.46±7.72 to 26.15±8.25 (P=0.023) respectively. Conclusion: Rapid infusion of normal saline to vessel anastomosis during the kidney transplantation is a risk for development of metabolic acidosis while it has beneficial effects on graft function.

Background and Objectives: Kidney Disease has been known to be associated with bone abnormalities as early as 1883.Despite of the emergence of new and more inclusive terms; low turnover and high turnover definition still are useful definitions. Aluminum toxicity also is a great threat among these patients.Materials and Methods: The study was started on 30 patients recruited from several hemodialysis (HD) units in Tabriz. Six patients left the study and only 24 patients completed their work up (10 female and 14 males) Immunoradiometric assay was used to measure the bone specific alkaline phosphatase (BAP) and intact parathyroid hormone (i PTH). Serum Aluminum was studied by atomic absorption. Results: In 8 patients (8.24, 33%) serum level of iPTH was lower than 150pg/ml, and in 14 patients (14.24, 58%) serum BAP levels was lower than 27 U/L, but only in 5 patients (5.24, 20%) the combination of low iPTH (<150 ng/ml) and low BAP (<27 U/L) was present. serum aluminum of lower than <60 ng/ml is considered normal in HD patients. We had 21 patients (21.24, 87%) with serum Aluminum level above this defined limit. Conclusion: This preliminary study at first revealed us the probable percentage of 20% for low turnover bone disease among our patients. Secondly and more importantly disclosed the high rate of aluminum exposure in our dialysis population
