Abstract:

Background: Diabetes is a chronic and prevalent disease. In recent years, its prevalence has increased because of lifestyle changes, immobility and subsequent obesity. The increase in diabetes prevalence has provoked multiple and scattered interventions by specialists and health care providers in order to control and regulate blood sugar and its related factors. The purpose of this study was to collect and integrate the results of these interventions in two fields of psychological and sport interventions and to investigate the impact of them on control of Glycosylated haemoglonin (HbA1c) in patients with diabetes by meta-analysis method. Methods: For this purpose 38 acceptable studies (19 sport interventions and 19 psychological interventions), in terms of methodology, were selected and meta-analysis performed on them. This study is based on 38 effect sizes and research instrument is checklist of meta-analysis.Results: The study findings showed that the effect size of psychological interventions on control of Glycosylated haemoglonbin is 0.67 (p<0.00001) and the effect size of sport interventions is 0.79 (p<0.00001).Conclusion: according to Cohen’s chart, the effect size of both psychological and sport intervention on control of Glycosylated haemoglonin in patients with diabetes is medium. However, the effectiveness of sport interventions have been obtained more than the psychological intervention.

Background: Irisin was identified as a myokine secreted by exercise which plays an important role in energy metabolism and regulation of metabolic diseases such as diabetes. The aim of the present study is to investigate the effects of high intensity interval exercise (HIIE) and moderate intensity continuous exercise (MICE) on serum irisin and Subcutaneous UCP-1 in diabetic male rats. Methods: In this study, 29 diabetic Wistar rats (12 week- age, 220-240 gr- weight) were assigned to 5 groups: immediately and 2 hours after high intensity interval exercise (HIIE0, HIIE2), immediately and 2 hours after moderate intensity continuous exercise (MICE0, MICE2) and control (C). Both MICE groups performed on the treadmill with intensity 60-65% vo2max for 40 minutes and both HIIE groups with intensity 90-95% vo2max in the 12 interval-one minute period and 1 minute rest intervals.ELISA was used to measure serum irisin and subcutaneous fat UCP-1. One-way ANOVA and Tukey post hoc test has used to data analysis, the level of significance has been considered at P≤0.05.Results: Data analysis showed serum irisin levels in the HIIE0 group and subcutaneous fat UCP-1 in the HIIE2 significantly increased compared to the control group (p˂0.05). However, there was no significant difference between other research groups (P>0.05).Conclusion: It seems high intensity of exercise had important roles in significant changes of serum irisin levels and subcutaneous fat UCP-1in diabetic rats.

Abstract:

Background: The purpose of this study was the investigation effect of gender on plasma glucose, insulin and insulin resistance index in response to acute endurance activity and subsequent recovery periods in healthy individuals. Methods: Eight females and seven males 20-30 years old (females 22.37±1.50 and men 23.42±1.27 years old, female weight 55.50±8.14 men and 79.85±15.16 kg, women height 160.12 ± 4.18 men and 179.71±6.57 cm) voluntarily participated in this study. Subjects performed two control activities during two consecutive weeks. The implementation of the sessions was randomly determined for each subject. Exercise program consisted of 45 min acute endurance at 75% of maximum heart rate on the treadmill and subsequently the subject was in a sitting position for one clock recovery. Four blood samples (6 ml) before and immediately after exercise, as well as 1 and 24 hours after the activity was collected. Two-way analysis of variance for data analysis was used. Results: Regardless of gender, effects of activity and recovery on plasma insulin and insulin resistance index was significant (p<0.05) but not significant for glucose. Also when the data were corrected by changes in plasma volume, similar results were observed for insulin. Although changes of all factors (glucose, insulin, and insulin resistance) in response to activity session of acute exercise endurance was higher in men than in woman, gender has not significant effect on factors. Conclusion: acute endurance activity and recovery induce change in insulin resistance index but these changes were not related to gender.

Abstract:

Background: Prevalence of diabetes increases along with aging. Due to various reasons, rate of falling in the aged suffering from diabetes is higher. Therefore, this study aims at determining factors relating falling of the aged suffering from type 2 diabetes. Methods: A total of 60 years old or older with type 2 diabetes that referred to selected hospitals of Tehran University of Medical Sciences using continues method. Cases that had experienced falling during last year and controls that had no such experience were assigned to two separate groups. Questionnaire used in this study included demographic data and factors relating falling. Data obtained were analyzed using descriptive statistics, independent t- test, Chi square and logistic regression. Results: Results showed that postural blood pressure (p=0.02) dizziness (p=0.05), fecal incontinency (p<0.01), auditory disorder (p<0.01), visual disorder (p<0.01), pain in the lower extremity (p=0.027), taking anti-coagulation medicine (p=0.017) had a significant correlation with falling. Conclusion: Multivariate logistic regression illustrated that fecal incontinency and auditory disorder happened along with falling in the aged suffering from type 2 diabetes. Therefore, preventing falling must be considered in care plan for such groups of people.

Abstract:

Background: The objective of the current study is to assess the effectiveness of Mobile Short Message Service (SMS) intervention on education of basic self-care skills in patients with type 2 diabetes. Moreover, we aimed to determine whether delivering individually-tailored educational messages can be more effective than general educational messages. Methods: A total of 150 patients with diabetes type 2 were randomized into three groups: tailored SMS group, non-tailored SMS group, and the control group. Biochemical parameters including HbA1c, FBS, lipid profile were evaluated for the three groups at baseline and after 12 weeks. Moreover, self-care Inventory (SCI), Diabetes Management Self-Efficacy Scale (DMSES) and Diabetes Self -Care Barriers assessment scale for Older Adults (DSCB-OA) were completed. In the tailored SMS group, each person received 75% of their messages based on the top two barriers to adherence that they had experienced and reported in their scale. In the non-tailored SMS group, random messages were sent to every patient. Results: After12 weeks, although HgA1c levels did not significantly change, significant decline was observed in FBS and mean BMI in both intervention groups. Mean SCI-R scores significantly increased and mean DSCB and DMSES scores significantly decreased in both tailored and nontailored SMS groups. In the control group, mean SCI-R scores decreased and mean DSCB and DMSES scores significantly increased (P<0.001).Conclusion: Sending short text messages as a method of education in conjunction with conventional diabetes treatment can improve glycemic control and positively influence other aspects of diabetes self-care. According to our findings, sending SMS regularly in particular times appears to be as effective as sending individually tailored messages.
