Abstract:

Background and Objective: The role of ion channels and particularly cationic channels in the pathogenesis of various diseases are being considered carefully. The diabetes mellitus is a common disease which is initiated by ion channel disturbances. This study was done to determine the characteristics of hepatocyte rough endoplasmic reticulum single cationic channel in Streptozocin- induced diabetic rats.Materials and Methods: This experimental study was done on 10 male adult Wistar rats and animals were randomly allocaied into diabetic and control groups. Diabetes induced by STZ (65 mg/kg/bw) intraperitounally. Rough endoplasmic reticulum vesicles were extracted following rat liver excision, homogenization and ultracentrifuging. The bilayer membrane formation was prepared by painting phosphatidylcholine on 250 mM aperture in between Cis and Trans sides. The RER vesicles incorporation was performed through gentle and delicate touch of membrane using a dentistry needle. The Pclamp9 software was used for ion channel activity characteristic analysis. Results: The cationic channel current amplitude did not change significantly in voltages more than +3o mV but their open probability (Po) decreased in diabetic group (P<0.05). More severe changes in channel activity were seen in potentials less than the reverse potential. In addition to significant increase of channel Po (P<0.05), also, the channel unitary currents were significantly decreased (P<0.05). The mean current amplitude and channel open probability in voltage +40 mV were 17±2.14 pA and 0.68±0.01 in control group respectively, whereas, the values of these parameters reached to 18.5±2.5 and 0.26±0.03, respectively. In voltage -10 mV, the values of mean current amplitude and Po were -22.3±2.14 pA and<0.1 in control group, respectively but the values changed to -13.1±0.08 and 0.62±0.03 in diabetic group. Conclusion: It seems that RER cationic channel is involved in metabolic changes which cause by diabetes mellitus and this disease can cause probably a channel gating kinetic and behavior change by inducing metabolic stresses.

Background and Objective: Oxidative stress is known as the one of the causes of neuropathic pain, reactive oxygen species (ROS) has been reported to be involved in this process. Coenzyme Q10 acts as an antioxidant and is able to reduce resulting oxidative damage. This study was done to determine the effect of Coenzyme Q10 on neuropathic pain in CCI (Chronic Crash Injury) rat model.Materials and Methods: In this experimental study 30 male rats (200-250g) randomizely allocated into 3 groups each 10 rat including CCI, CCI + CoQ10 and CCI + vehicle. Neuropathic pain, was induced by Chronic Crash Injury (CCI) model for sciatic nerve. IP injection of CoQ10 (200 mg/kg) or vehicle was done daily for 10 days. Behavioral tests were done before and after surgery on day 5 and 10 respectively. Paw withdrawal threshold was assessed by Randall Selitto test, Analgesy Metter and Von Frey filaments. Data was analyzed by SPSS-14, Independent T and Mean-whatney tests. Results: Induction of nerve injury decreased pain threshold (P<0.05) and treatment with CoQ10 increased mechanical and thermal threshold in neuropathic rats compared to CCI animals (P<0.05). There was a significant difference in pain threshold between animals treated CoQ10 and vehicle injected animals (P<0.05).Conclusion: This study indicated that coenzyme Q10 prevents deleterious effects following nerve injury in animal model.

Background and Objective: Abdominal operations as gynaecological procedures result in gastrointestinal dysmotility. Early feeding and ambulation are nonpharmacologic interventions which can be useful in re-initiation of bowel function. This study was done to evaluate the effect of early oral hydration on the return of bowel function and woman's satisfaction after elective caesarean section in primiparous women.Materials and Methods: In this randomized clinical trial, 120 primiparous women undergone elective cesarean section were assigned to control and intervention groups in Hajar hospital, Shahrekord, Iran during 2007. In the interventional group, oral hydration with liquids was started 4 hours after surgery regardless of presence of bowel sounds and solid food was started after bowel sounds appeared. The control group recieved liquid diet 12 hours after the operation if it was tolerated, they were given soft diet and regular food at the next meal. The return of bowel activity, time of ambulating, satisfaction, discharge from the hospital and complications were compared in two groups. The data were analyzed using SPSS-15, Chi-Square, T and one way ANOVA tests. Results: The mean postoperative time interval to first hearing of normal intestinal sounds in interventional versus control groups were (9.5±1.38 and 12.5±2.5 hours) the first passage of flatus (15.7±3.61 vs. 22.4±4.1 hours), time to first sensation of bowel movement (10.8±1.99 versus 15.7±3.4 hours) and defecation (18.9±3.65 versus 23.4±4.85 hours). These differences were significant (P<0.05). Also discharge from the hospital (0.96±0.18 versus 1.1±34 days) were significantly shorter in interventional group (P<0.05). The women in the early feeding group got out of bed (patient mobilisation) earlier than their interventional group (14.1 hours versus 18.8 hours (P<0.05). Maternal satisfaction was significantly higher among the early fed women (P<0.05).Conclusion: Early oral hydration after elective cesarean section associated with rapid resumption of intestinal motility and increased woman’s satisfaction.

Background and Objective: Ghrelin is an acylated 28-amino-acid peptide that is the most recently identified adipocytokines, but its role in diabetes is poorly clarified. The objective of this study was to determine the relation between serum ghrelin and blood glucose levels in type 2 diabetic obese males.Materials and Methods: This descriptive study was done on 45 adult obese males with type-2 diabetes in Saveh city, Iran during 2010. Fasting blood glucose, insulin and ghrelin concentrations and Glycosylated hemoglobin HbA1C were measured after overnight fasting. Multiple regression was used for determine ghrelin in relation to glucose, insulin and HbA1C.Results: The multiple regression analyses revealed that HbA1C is not correlated with serum ghrelin levels, while, fasting blood glucose level had positive corrolation with serum ghrelin concentration (P<0.05). Serum ghrelin level had high negative correlation with insulin (P<0.05).Conclusion: This study indicated that elevated endogenous ghrelin led to hyperglycemia. Therefore, serum ghrelin is a precise index of blood glucose level in obese male patients with type-2 diabetes.

Background and Objective: Regarding to probable high frequency of Glucose - 6 - phosphate dehydrogenase (G6PD) deficiency in Iran, screening of all neonates by cord blood is under consideration. This study was conducted to determine the prevalence of G6PD deficiency in newborns and the relation between gender, jaundice, hemolysis, anemia and the G6PD deficiency in neonated born in in Tehran, Iran.Materials and Methods: In this descriptive study, cord blood of 450 neonates born in Akbarabady hospital in Tehran, Iran during 2008-09 were screened. Demographic information was recorded by questionnaires and the newborns were examined for detection of jaundice till discharge. G6PD level was determined by Fluorescent Spot Test (FST). G6PD deficient neonate were put under close observation for detection of jaundice. Enzyme activity was rechecked by spectrophotometry. Results: Nine neonates out of 450 were G6PD deficient (8 boys and one girl). Prevalence of G6PD deficiency was 2% (3.3% for boys and 0.5% for girls). Six neonates of nine G6PD deficient neonates (66%) developed pathologic jaundice. Four neonates were managed by phototherapy and two by exchange transfusion. Conclusion: This study showed that G6PD deficiency is more prevalate among male neonates, therefore, G6PD determination is recommended to prevent the possible neonatal jaundice.

Background and Objective: According to world health organization statistics, at least 5.2% of world population is carrier for a main hemoglobin disorder. Previous reports showed that more than 10% of people are carrier for beta-thalassemia Northern Iran. This study was done to determine the prevalance of hemoglobinopathies in premarriage individuals referred to Babolsar, Iran.Materials and Methods: This descriptive study was carried out on 8500 individuals (4200 women and 4300 men) whome were attended the thalassemia counseling program in Babolsar, North of Iran during 2006-09. After performing the CBC test, for those MCV and MCH were less than 80 and 27 respectively, Hemoglobin A2 was evaluated. Subjects whome were volunteers for more comprehensive tests, basic and acidic electrophoresis and genetic tests were applied, subsequently. Results: 1200 (14.11%) subjects had low hematological indexes.474 (5.57%) subjects had high HbA2 and were classified as beta-thalassemia carriers and 726 (8.54%) had normal HbA2 level and were classified as alpha-thalassemia carriers.6 (1.2%) subjects were identified with HbF level more than 10 and were identified as carriers for beta-gene cluster deletion carrier. Also, 16 (3.2%) individuals had HbE, 16 (3.2%) had HbS, 4 had HbD and 4 had HbH (0.33% in 1200 and 0.047% in 8500 subjects). Genetic study of 317 individuals for beta carriers and 145 subjects for alpha-carriers showed IVSII-1G>A (74.5%) in beta-globin and single gene deletion of 3.7 (47.5%) in alpha-globin genes were the most frequent mutations. Conclusion: This study showed that carriers for alpha - thalassemia (8.5%) are more frequent compared with beta- thalassemia (5.57%). Also other hemoglobin variants included HbS, HbE, HbD or different beta-gene cluster deletions in the region are considerable and should be screened.
