Abstract:

Introduction & Objective: Large group of premature births alone causes mortality and morbidity in children in developing countries make up and increase the incidence of neonatal complications: respiratory distress (RDS) is a major cause of infant death is. This study aimed to determine the effect of dexamethasone on the outcome. Preterm labor at 36-30 weeks of pregnancy, referring to the training center-Specialized Design and Kosar was performed.Materials & Methods: A Quasi-experimental over 176 pregnant women with 36-30 weeks referred to Centre Educational - Specialized Kosar in 1385 was conducted. Data from the questionnaires and made the case infants hospitalized in neonatal centers - Health Motahari and Imam Khomeini (RA) collected using statistical software SPSS and descriptive and analytical statistical tests (Chi-square, and logistic regression ) were analyzed. Results: Neonatal exposure to the disease than hyaline membranes and respiratory distress in the group receiving dexamethasone 29.1percent in control group 35.1percent, which was statistically significant difference, was found. However, administration of dexamethasone before delivery in the neonatal death rate for group 26.5percent versus 10.3percent of the control group was reduced, which was statistically significant. (Chi=7.36 P-Value=0.005)Conclusion: Received dexamethasone before delivery reduces the incidence of hyaline membranes disease and respiratory distress in the group that had significant impact in terms of absence. But about 50 percent death rate was reduced with regard to this effect in other studies suggest that treatment with dexamethasone, even in case of imminent delivery of a dose as being a routine procedure in preterm deliveries be used.

Abstract:

Introduction & Objective: Pruritus is one of the most common skin lesions of renal patients. Its prevalence among dialysis patients is between 10 to 20 percent. The main cause of itching in these patients is unknown and various medications are prescribed to patients. The purpose of this study is assessing of response to treatment of cromolyn sodium gel 4% and comparison to pimecrolimus cream 1% in patients with end stage renal disease.Materials & Methods: This study was conducted as a randomized single blind study in 60 ESRD patients randomly divided to two groups of 30 each and a group treated with cromolyn sodium gel 4% and another group treated with pimecrolimus cream %1 for 8 weeks two times a day starting immediately after dialysis. Patient's pruritus levels was determined 9 times (before the start of the study and at the end of each week after for eight weeks) using a Visual Analogue Scale. Results: The mean score of pruritus in the group receiving cromolyn sodium gel 4% was assessed 9.4±5.1 and 3.1±5.4 before and after the medication, respectively (P<0.0101). And, in the group receiving pimecrolimus cream %1 was detected 9.1±5.3 and 5.7±3.2, respectively (P<0.0101). About drug side effect, 3 patients of cromolyn sodium group and 5 patients of pimecrolimus group had complained of erythema and burning sensation. Conclusion: According to this study, cromolyn sodium gel 4% to be an effective drug compared to pimecrolimus cream %1 in reduction of pruritus among ESRD patients undergoing dialysis.

Self-efficacy as a mediate cognitive factor between individual judgments about ability of thinking, emotion and action, and determine why people with similar skills and knowledge have a different performance. In this study, we investigate relationship between aggression and perceived self-efficacy among high school students.Materials & Methods: Four hundred and ninety nine students from Hamadan city were selected with multistage cluster sampling and completed Buss and Perry’s Aggression Questionnaire and Morris perceived self-efficacy. Data was analyzed with Pearson correlation and one-way analysis of variance. Results: 16.1% of students had high self-efficacy perceptions, and 48% of students were highly aggressive. Students from different areas of the city had a different amount of aggression, but there was no difference in self-efficacy. There was a negative relationship between aggression and perceived-efficacy.Conclusion: High aggressiveness and low self-efficacy in adolescents are important factors in future life, especially in social, emotional and educational. It is important to consider these variables in the mental health education and health promotion programs. Its need to support vulnerable families that have children with the risk of aggression and low self-efficacy and therefore, emotional and educational problems.

Therefore this survey was conducted in order to determine of rate performance of surgical hand scrub standard in Hamadan educational hospitals.Materials & Methods: in this descriptive study, population and sample were all surgeons and operating room nurses of Hamadan educational hospitals. Data collection tools were 4 check lists about process of hand surgical protocol with alcohol-based and non-alcoholic hand antiseptic solutions, instruments of these protocols. In order to gathering data, each sample was observed 3 times when he or she was accomplished hand scrub. Results: The average score of surgical hand scrub process with alcoholic solutions was 67.6% of total score. Mean score of scrub instruments with alcohol base solutions was 100% and about non alcoholic solutions was %90.3 of total score of its instruments of essential and important points in hand antisepsis process with both solutions that was report in least performance, were to washing hands with water and soap before scrub and after removal of sterile gloves, to disinfect all parts of hand and to clean under nails. Time of scrub was 108 seconds and with alcoholic solutions and 215.6 seconds with nonalcoholic solutions. Differences between antisepsis duration in according to hospital and kind of surgery were significant (p<0.001). Difference between alcohol–base solutions scrub process with hospital under surgery and kin of surgery, was reported significant (p<0.001). Difference between nonalcoholic solutions and kind of surgery, was reported significant too (p<0.001).Conclusion: for correct performance of hand scrub, it is required: 1-infection committee or nursing supervisor office of hospitals, determine hand scrub standards according to latest scientific changes for implementation. 2-Personnel education and equipments were provided to do this procedure correctly according to surgical hand scrub protocol. 3-Surveillances for correct performance were done continuously.

Introduction & Objective: Cardiac glycosids are the oldest drugs in congestive heart failure. Digoxin one of this class drugs has widespread use today. Because the therapeutic index of this drug is small, the changes in kintic of drug induce change in serum level of drug and can be dangerous. The aim of this study was evaluation of serum level of digoxin in hospitalized patients.Materials & Methods: This was a cross-sectional study and the samples were collected from cardiac patients that were hospitalized in cardiology and pulmonology wards in Ekbatan hospital during the study. Serum levels of digoxin were measured by HPLC. Patients demographics, clinical situation and serum electrolyte were recorded in a questionnaire. The data was analyzed by t-test, coefficient of correlation and P<0.05was considered significant. Results: In this study the digoxin serum level in 76.9% of patients was in therapeutic level, 8.9% had insufficient level, 5.8% had intermediate level, and 8.4% had toxic level of digosin. The patients that had lower body mass index had greater serum level of digoxin. Conclusion: We can indicate in Ekbatan hospital, patients had a suitable coundition due to digoxin serum level.
