Abstract: 

Introduction: Clinical competence of nurses is an effective factor in ensuring the quality of care in a competitive modern world. Given the vital role of nurses and the weakness of existing assertiveness, this concept as a communication style can play an important role in improving performance and improving the quality of care in tense stressful care settings. The purpose of this study was to determine the effect of the assertiveness program on the clinical competence of nurses in intensive care units. Methods: In this clinical trial, 70 nurses of ShahreKord in 2018 were randomly allocated into two groups experimental and control. The experimental group was trained in 6 sessions of ninety minutes, while the control group did not receive the training. Data was collected before, immediately and 3 months after intervention by demographic and CIRN Clinical Competency Questionnaire and analyzed using SPSS 17 with independent t-test, ANOVA with repeated measurement and Chi-square. Results: There was no significant difference between the mean scores of clinical competency between the two groups before intervention, but this difference was significant in the immediately and 3 months after intervention based on independent ttest (P<0. 05). ANOVA test with repeated measurement showed a significant difference in the process of changes in mean scores in the three stages of measurement (before, immediately and three months after the intervention) (P<0. 001). Conclusion: Assertiveness education leads to increasing the clinical competence of nurses in intensive care units. Therefore, it is suggested to be used in nursing education courses.

Abstract: 

Introduction: Asthma is one of the most common chronic respiratory diseases and one of the major problems in health systems in many societies around the world including Iran. Self-care is one of the most important factors in raising the awareness and attitude of individuals. The purpose of this study was to investigate the effect of self-care education on knowledge and attitude of patients with asthma. Methods: In this quasi-experimental study, 104 patients with asthma were selected by available sampling method and randomly divided into two groups (n=52) and control (n=52). Both groups filled the questionnaire of awareness and attitude before the intervention. The self-care group was trained in four sessions of 60 minutes. The questionnaire was filled again this time by both groups, 8 weeks after the intervention. and the data was analyzed using SPSS 16, descriptive statistics, independent t-test and paired t-test at 95% confidence interval. Results: The results showed that there was no significant difference between the mean and standard deviation of knowledge and attitude scores before and after selfcare education (P> 0. 05). There was a statistically significant difference between the mean and standard deviation of the knowledge and attitude of the experimental and control groups after the training (P<0. 05). The results showed that the knowledge and attitude of the experimental group significantly improved after the intervention. Conclusion: The results of this study showed that self-care education is effective on knowledge and attitude in asthmatic patients. Therefore, considering the high prevalence of this disease and its economic and social burden, self-care education is recommended to improve the knowledge and attitude of asthmatic patients.

Abstract: 

Introduction: To start and continue breastfeeding, primiparous women have many challenges. Breastfeeding self-efficacy is an important psychological and motivational variable, but is also modifiable for the continuation of nutrition with mother milk. The aim of this study was to investigate the effect of education through cyberspace on breastfeeding in women with their first birth. Methods: This classic experimental study was carried out on 65 primiparous women that referred to Shahrood maternity preparatory centers during their 28-32 pregnancy weeks in 2018. Two centers were randomly selected from four health centers and then, in an accessible way, 65 people were assigned to control and experimental groups. The experimental group received breastfeeding training educational program in 7 sessions with intervals of 45-60 minutes. The data was collected using a demographic questionnaire and breastfeeding self-efficacy and were analyzed using SPSS 21, independent sample, paired sample and covariance tests considering P<0. 05 significant. Results: The independent t-test did not show a significant difference between the mean self-efficacy of the two groups before and after the intervention (P= 0. 1), but there was a significant difference between the two groups after the intervention (P<0. 01). Conclusion: According to the results, education through cyberspace can be used as a new and effective method for self-efficacy of breastfeeding in primiparous women. Therefore, it seems that mothers' education can improve the factors related to lactation self-efficacy and subsequently improve breastfeeding status.

Abstract: 

Introduction: Nurses are the most effective pillars of the hospital to improve the quality of services. Their organizational citizenship behaviors are an important factor for improving patient's nursing care. Organizational Citizenship Behaviors (OCB) can prevent additional system costs. The present study was an attempt to determine the relationship between demographic characteristics and OCB of nurses in Hamadan educational centers. Methods: This descriptive-analytic correlational study was conducted in 2016-17. The sample was 200 nurses selected from five educational centers by stratified random sampling method. Data gathering tool were Demographic Information and Organizational Citizenship Behavior of Podsakoff Questionnaires. Data were analyzed by SPSS 21 and using descriptive and analytical statistics (T-test, ANOVA and Pearson correlation coefficient). Results: The results showed that a relatively favorable level of nurses’ OCB. There were statistically significant relationship between OCB of nurses with age and work experience respectively (r = 0/20 and P<0/005) and (P<0/029 and r = 0/16). Mean score of Courtesy was 4. 03± 0. 5, Conscientiousness 4. 0± 0. 5, Magnanimity 3. 83± 0. 4, Civic Virtue 3. 33± 3. 8, Altruism 3. 83± 0. 4 and the mean score of OCB was 3. 8± 0. 3. Conclusion: Organizational citizenship behavior of nurses increases with increasing their age and work experience. Therefore, to take advantage of the OCB in hospitals, intensive attention should be paid to training younger nurses and supporting them.

Abstract: 

Introduction: Today, despite of the emphasis on family-based care strategies, there is little understanding of the adult patient's perception of the presence of family members during emergency care and invasive procedures. The aim of this study was to compare the effect of family presence and their training before the presence on the anxiety of patients undergoing invasive nursing procedures in the emergency department. Methods: In this randomized clinical trial, 90 patients undergoing invasive nursing procedures and their family members were selected based on inclusion criteria and allocated randomly in three groups of 30 (with the presence of trained family members, with the presence of untrained family members and without the presence of family members) by minimization method. The data was analyzed by SPSS 16 using Kolmogorov-Smirnov test, Chi-square test, Kruskal Wallis Independent-sample, paired sample t-test, and ANOVA at the significance level of 0. 05. Results: The three groups were similar in terms of demographic variables. All three groups had significantly lower anxiety scores in the post-intervention phase than before the intervention (P=0. 001). There was no significant difference between the mean and the standard deviation of the patient's anxiety scores before (P=0. 131) and after the intervention (P=0. 309) in the intergroup comparison. Conclusion: The presence of family members and their training before presence did not reduce the anxiety of patients under nursing invasive procedures. It's necessary that other aspects of family members’ presence during invasive procedures to be considered in future studies.

Abstract: 

Introduction: The aim of this study was to determine the effect of prenatal counseling on knowledge and practice about postpartum care in primigravida women. Methods: A quasi-experimental study was conducted in health centers of Hamadan city in 2017. Using a stratified sampling, 96 primigravida women at their 32-34 weeks, were qualified to enter the intervention and control groups. For the experimental group, three 45 minutes weekly counseling sessions were held. Knowledge of postpartum care were gathered by questionnaires before intervention and 42 days after delivery and practice of postpatum care was assessed 42 days after delivery using national Sib Samaneh. Data was analyzed using SPSS 20. Results: The mean of maternal knowledge scores from postpartum care in the intervention group before and after intervention increased from 17. 4± 7. 1 to 33. 3± 6. 6 and there was a significant difference with the control group (20. 8± 5. 4 to 21. 6± 5. 8) (P<0. 001). The mean score of mothers' performances on postpartum care in the intervention group was higher than that of the control group (13± 1 vs 10± 1/9), and comparing the two groups, a significant difference was observed (P<0. 001). Conclusion: The results of this study showed the positive effects of prenatal counseling on increasing the level of knowledge and improving the practice of postpartum care in primigravida women. Therefore, due to various postpartum problems, it is recommended that appropriate education be provided before delivery.

Abstract: 

Introduction: Pressure ulcer is a complication from which many patients suffer. Preventing this matter is one of the duties of nurses. The purpose of this study was to determine the effect of massage of olive oil with sweet almonds on prevention of pressure ulcer in patients hospitalized in ICU. Methods: This study is a double-blind randomized clinical trial that was performed on 90 patients admitted to ICU Hospital in Maybod city, Yazd, Iran. Patients were selected by purposeful sampling and random blocked method in enrolled in two groups of test and one control group. The patients of the test groups received a massage in the areas exposed to risk of pressure ulcer for a week once a day with 3-1 mL of olive oil or sweet almonds. Also these two groups received the usual skin care including a change in position every two hours and a wavy mattress and control group received just the usual care of the skin. Data was collected using a demographic questionnaire and a checklist for potential areas with the risk of pressure ulcer using Braden's scale. Data was analyzed by Chi-square test, Fisher’ s exact test and variance analysis test in SPSS 16. Results: There was no statistically significant difference between the groups in terms of demographic variables. The results showed that the incidence of pressure ulcers in the experimental groups was lower than the control group and in the olive oil massage group was lower than the sweet almond oil massage group (P<0. 05). Conclusion: The results showed that olive Oil and sweet almond massage is effective in preventing pressure ulcer. A similar study to this study should be done using other herbal products such as Aloe Vera gel to prevent Pressure ulcer.

Abstract: 

Introduction: Patients with epilepsy need to learn to identify seizure-inducing factors in order to adapt to seizure activity and disease management and taking precautionary measures to minimize seizure damage. The aim of this study was to determine the effect of an educational program on self-management empowerment of patients with epilepsy. Methods: This quasi-experimental study was conducted on 90 patients with epilepsy referring to Iranian Epilepsy Association. The study used convenient sampling and samples were randomly divided into two groups: control (n= 45) and experimental group (n=45). The control group received routine training and the experimental group in addition to routine training, participated in three educational sessions held by the researcher in Epilepsy Association. All the patients completed a self-management questionnaire before and six weeks after the intervention. The data were analyzed by SPSS 22 using Kolmogorov-Smirnov, independent paired t-test and descriptive statistics. Results: The findings showed that there were no significant differences between the two groups in terms of demographic characteristics (P>0. 05). Before implementing educational program, the two groups showed no significant difference in medication, knowledge, information, safety and seizure scores (P> 0. 05). After implementing educational program the two groups showed significant difference in the scores (P<0. 01), which is indicative of the increased scores in experimental group. Conclusion: The results of the study indicated that the education has been effective in information, seizure control and safety of people with epilepsy. Implementation of such programs in order to better accommodate people with epilepsy with their illness is recommended.
