Abstract

 Considering little knowledge on the conditions of patients with SEVERE HEAD CONCUSSION, and its SECONDARY CEREBRAL DAMAGES as a result of systemic disorders and increased intracranial pressure is one of the most important causative factors of mortality, this study was carried out to determine the incidence of systemic disorders and their role in the fate of patients with SEVERE HEAD CONCUSSION hospitalized in Imam Hossein hospital during the years 1997-1998. The descriptive and sequential strategy of this study was performed in patients with SEVERE HEAD CONCUSSION (GCS<8) during an 18-months period referred to Imam Hossein hospital. In this respect, demographic characteristics and causative factors for SECONDARY CEREBRAL DAMAGES including HYPOXIA, HYPOTENSION, hypercapnia, and anemia were evaluated. In addition, clotting problems and the presence of intracranial masses were studied in 50% of patients, at emergency admit, there existed one or several systemic disorders. In 35%, 22%, 16%, 10%, and 40% of patients there existed HYPOXIA, hypercapnia, HYPOTENSION, anemia and different intracranial lesions respectively. In addition, in half of the patients, PT disorder and in 25%, PIT disorder, and in 20% there existed thrombocytopenia. In those patients with systemic disorders, mortality rate was 71 % and for those not having such complications, the rate was 37%. In patients with PIT disorder, mortality rate was twice the normal ones. The mortality rate in patients with HYPOTENSION, HYPOXIA, no disorder, and a combination of HYPOXIA and HYPOTENSION was 68%, 64%, 42%, and 90% respectively. In patients with HYPOXIA and HYPOTENSION, the failure was twice and its risk was 44% (P<0.005). It is concluded that systemic disorders is a common problem in patients with SEVERE HEAD CONCUSSION (50% of cases) and can lead to SECONDARY CEREBRAL DAMAGES and increased mortality rate (nearly twofold). Thus, prompt transfer of patients and performance of necessary measures is very essential and is accompanied with increased recovery in suffered patients.
