COMPARING THE EFFECT OF PREOPERATIVE MELATONIN VS. GABAPENTIN ON PAIN AND ANXIETY IN PATIENTS UNDERGOING CATARACT SURGERY ( PHACOEMULSIFICATION) WITH TOPICAL ANESTHESIA

Abstract

 Aim and Background: The main purpose of sedation in patients under ocular surgery is preparing them to stay calm. Inadequate or deep sedation may lead to patient’s sudden movement and subsequent serious complications. This study was performed to compare the effect of MELATONIN and GABAPENTIN on PAIN and ANXIETY during CATARACT SURGERY under topical anesthesia.Methods and Materials: This study was a single blinded clinical trial conducted on 81 patients undergoing CATARACT SURGERY with topical anesthesia in Amir-al-Momenin Academic Hospital in 2014. Patients were randomly assigned into 3 groups: 27 patients received 6 mg MELATONIN, 27 patients received 600 mg GABAPENTIN, and 27 patients were on placebo (control) 90 minutes pre -operatively. In operation room (OR), placebo group received 1 mg Midazolam; and all patients received 0.5 mic/kg Fentanyl. PAIN and ANXIETY were evaluated on premedication time (T1), in the OR (T2), and post operatively (T5).Sedation was evaluated at T1, T2, at the time of topical anesthesia (T3), 15 minutes after the start of surgery (T4) and T5.Findings: In our study, there was no statistically significant difference in demographic variables and the intensity of ANXIETY and PAIN before surgery among the three groups. There was a significant difference between mean PAIN score among the groups while entering the OR and discharge from recovery (p=0.001 and p=0.0001). The intensity of ANXIETY among the groups showed a significant difference at the time of entering the OR (p=0.034).However, there was no significant difference regarding ANXIETY among the three groups at discharge time (p=0.316).Conclusions: The results show that pretreatment with MELATONIN-Gabapentin and Midazolam decrease ANXIETY scores to the same extent when Midazolam is used. In addition, a single dose of GABAPENTIN or MELATONIN as premedication reduces PAIN intensity in patients.
