EVALUATION OF PSYCHIATRIC CONSULTATION REQUESTS IN IMAM HOSAIN GENERAL HOSPITAL

Abstract

 Introduction: 30-60% of patients admitted at GENERAL HOSPITAL suffer from a psychiatric problem. Unfortunately these problems are not considered properly by physicians and cause to elongate hospitalization time, increase health services cost and disability and mortality. In this paper, we evaluated psychiatric CONSULTATION requests from other wards in a GENERAL HOSPITAL affiliated by a medical university.Methods & Materials: all psychiatric CONSULTATION forms which have requested for in-patients in June 2002 till May 2004 were evaluated retrospectively. Age, sex, ward, CONSULTATION request day, reason for CONSULTATION, medical diagnosis, psychiatric diagnosis, CONSULTATION request quality, CONSULTATION reply quality, method of psychiatric management were compared between patients in different wards.Results: during 2 years 15048 patients were admitted in hospital and 382 psychiatric CONSULTATIONs requested (2.54%). Fifteen CONSULTATION forms omitted because of data insufficiency. Of 367 patients 201 (54.8%) were female and 166 (45.2%) male and age (mean±SD) 43.5±19.82 years (minimum 5 years and maximum 94 years). Tuesday was the most CONSULTATION request day (16.3%) and Friday the least (9%). Internal ward was the most CONSULTATION requests ward (31.1%) and Radiotherapy the least (0.5%) (p<0.0001) but in comparing wards for CONSULTATION requests based on patients numbers the Neurology ward was the most. The main reasons for CONSULTATION was aggression (p<0.0001) and traumatic patients the most frequent patients who were requested psychiatric evaluation (p<0.0001). Delirium (19.6%) was the most psychiatric diagnosis in inpatients (p<0.0001) and pharmacotherapy (38.1%) the most management methods (p<0.0001).Conclusion: in spite of high frequency of psychiatric problems in patients in GENERAL HOSPITAL, psychiatric CONSULTATION request is low. It seems to pay attention to find the major reasons are the aims of further researches.
