Abstract

Patients suffering from chropic renal failure(CPR) undergo different changes after renal transplantation. These changes, combined with the effects of HEPATITIS C VIRUS (HCY) infection are subjects still under investigation. In order to further clarify the changes in liver enzyme levels after RENAL TRANSPLANTATION.(TX) and the correlation between these changes and LIVER BIOPSY scores before TX, we performed a longitudinal observational study in which 40 patients with CRP and HCY infection were followed before TX and one year Arter. Liver biopsies were done for 10 patients before TX. LIVER ENZYMEs, including AST, ALT, and alkaline phosphatase were checked before and monthly after TX for all patients. The Study population was comprised of 28 male(70%) and 12 female (30%) patients aging 13 to 57 years (mean ±SD= 33.5± 10.2).Mean dialysis duration was 42.1±35 months. A Significant rise in AST levels was noted after 6 months of follow up (27.3±23.1) before TX vs. 51.9±32.7 p=0.0.12) which kept rising until the end of the study (73.5± 85.7). ALT levels showed a similar trend (34.4+34.6 before TX vs. 65.5±64.8 after 6 months P=0.013 and 76.2±57.3 at the end of the study P=0.006). Yery slight (but significant) rise in total and direct bilirubin levels were also noted after 6 months which resolved in the next follow up(0.76 vs 1.17 P=0.023 and 0.3 YS 0.54 P=0.009 respectively). No correlation was noted between LIVER BIOPSY scores enzyme levels. Dividing the patients into those with biopsy scores of 4 or less and above 4 revealed no significant difference between the enzyme level of the two groups. The percent of cases with abnormal ALT and AST levels (above 45) rose from 22.5% to 75% (chi - square significance = 0.001) and from 5% to 68.8% (chi-square significance= 0.001) respectively. LIVER ENZYME levels are not reliable predictors of liver scores. CRP patients with HCY infection are likely to have a rise on their LIVER ENZYME level after TX regardless of the preoperative LIVER BIOPSY score or LIVER ENZYME levels.
