TREATMENT OF URINARY INCONTINENCE IN SPINAL CORD INJURY PATIENTS 12 YEARS FOLLOW UP

Abstract

 Objectives. SPINAL CORD INJURY patients have neurogenic bladder and incontinence. In this study we review methods for obtaining continence in SPINAL CORD INJURY patients in a period of 12 years. Materials and Methods. In this retrospective study we performed results of augmentation cystoplasty in 227 patients (71 % with ileum and 29% with ileocecal or sigmoid). within 12 years follow up. Results. Hydronephrosis subsided in 95% and 84% of the patients were completely dry with intermittent catheterization. 457 patient were followed with intermittent catheterization (227 with entrocystopasty and 230 without it). Continence rates of 77%(84% in entrocystoplasty and 75% in others) was obtained. Artificial urinary sphincters (AMS 800) were implanted in 26 patients (14 paraplegic). In nonparaplegic patients (12) continence rate was 85% and reoperation in 3 was needed. In the paraplegic group (14) continence rate was 78% and second surgery was needed in 8 patients (7 removal of sphincter). Conclusion. Entrocystoplasty with or without intermittent catheterization are effective ways for omission of condoms and urine bags in SPINAL CORD INJURY patients. Artificial urinary sphincter in selected patients has good results and high continence rate and is very attractive in some SPINAL CORD INJURY patients. In paraplegic patients with UTI, removal of the sphincter is inevitable in about 50% of patients and is not routinely recommended.
