THE EFFECT OF LIDOCAINE SPRAY EARLY AFTER TONSILLECTOMY ON PAIN AND VOMITING

Abstract

 Background and aims: TONSILLECTOMY is one of the most common ENT operations and it is accompanied by a series of postoperative complications like VOMITING and PAIN. The aim of this study was to evaluate the effect of LIDOCAINE spray used immediately after TONSILLECTOMY on these complications. Methods: This double blind randomized clinical trial study was performed on 100 cases of TONSILLECTOMY that divided into two equal groups of case and control. For the case group, early after TONSILLECTOMY LIDOCAINE 10% spray (4mg/kg) was used on the base of the tonsils, while in the control patients normal saline spray in an equal volume was used on the base of tonsils as placebo. The amount of ACETAMINOPHEN taken by the patients during the first 3 postoperative days was recorded. Results: The frequencies of ACETAMINOPHEN usage per day in the control group were 3.01± 1.43, 2.80 ±1.62 and 1.86 ±1.2 on the 1st, 2nd and 3rd postoperative days respectively. In the case group, the frequencies were 1.3± 0.92, 1.07± 1.02 and 0.94 ±0.83 on the 1st, 2nd and 3rd postoperative days respectively. The amounts of ACETAMINOPHEN usage in mg/kg/day, were 28.93± 13.02, 26.46 ±12.40 and 18.3 ±12.41 in the control group on the 1st, 2nd and 3rd postoperative days respectively and the amount were 12.07± 9.71, 10.39 ±7.71 and 9.03± 6.23 in the case group on the 1st, 2nd and 3rd postoperative days respectively. t-test revealed that the amount and frequency of ACETAMINOPHEN usage were significantly less in the case group (P<0.001). Times of VOMITING decreased in the case group but difference was not significant. Conclusion: Based on our findings, the use of LIDOCAINE spray after TONSILLECTOMY is recommended.
