PREVALENCE OF MICRO ORGANISMS AND THEIR ANTIBIOTIC SENSITIVITY IN PYELONEPHRITIS IN CHILDREN

Abstract

 Background: The goal of this study is to determine the prevalence of the MICROORGANISMs which cause pyelonephritis as well as sensitivity and resistance to Antibiotics in order to make a good decision in Empiric Antibiotic therapy for pediatric pyelonephritis. Materials and Methods: This study is about pyelonephritis in children and infants (Age 2mo-12 yr) between 1375-1382 in Bahrami Pediatric Hospital in TEHRAN-Iran. Results: In 170 cases of Monoclonal culture-positive pyelonephritis (with more than 105 colony counting) Ranged between 2 months to 12 years, the findings of this Research include: 11 % of the cases were male and 89% were female. The mean age was about 41 months, with the peak age group of 2 to 12 month old (31% of patients). The most prevalent MICROORGANISM causing pyelonephritis was E. coli (61 % cases) and the second most prevalent was Enterobacter (33%); then Klebsiella (3%), Pseudomonas Aeroginosa (1%), Staph Epiderimdis (1%); Proteus (0.5%) and coagulasenegative staph (1.5%). Overall Sensitivity rate of pyelonephritis producing MICROORGANISMs independent of the species are: To Ampicillin (1%), Amikacin (75%), Gentamycin (67%), Cotrimoxazole (16%), Nalidinic Acid (87%), Nitrofurantoin (83%), Cephalexin (25%), Ceftriaxon (95%) and Ceftizoxime (98%). Comparing the Cases between 1375-1377 and the cases between 1379-1382 reveals no significant difference in, mean age and sex distribution (P>0.05) and there is no significant difference in MICROORGANISM species distribution between two groups (P>0.05). Comparing Antibiotic sensitirity in twogroups, we found a reduction in sensitivity to Amikacin (100% to 87%). (P<0.05); a reduction in sensitivity to cotrimoxazole (30% to 12%) (P<0.05); and a reduction in sensitivity to Nalidixic Acid (95% to 82%) (P<0.05). Sensitivity Rate to E.coli shows a reduction: in sensitivity to gentamycin (100% to 87%) and to cotrimoxazole (30% to 7%) (P<0.05). Conclusion: We Can conclude that according to this research overall rate of MICROORGANISM sensitivity to Ampicillin, Cotrimoxazole and cephalexin is too low and maximome sensitivity rate is to ceftriaxon, Nalidixic acid, Nitrofurantoin, Amikacin and Gentamycin orderly and the sensitivity rate has been reduced in most cases during these 7 years. So According to this research, best Empiric therapy in pediatric febrile UTI is Intravenous ceftriaxone.
