Abstract

Introduction: There is two treatment policies in acute chelecystitis; Medical Therapy in acute phase and LATE CHOLECYSTECTOMy after 3 months and the other EARLY CHOLECYSTECTOMY within 72 hours of diagnosis. The Aim of this study is to compare outcomes of this two strategies. Methods: In this retrospective case-control study; 100 patients in two groups; early 50 patients and late surgery 50 patients; was selected from shariati and Imam Khomeini hospitals in 1997-1999 and duration of hospital admission and post operative complications compared with each other. Results: In this study duration of disease was 22.88 days in early surgery group compared with 50.4 days in late surgery (P=0.002). Duration of hospital Admission in early surgery was 9 days compared of 18.32 days in late surgery (P<0.0001). 8 patients had post operative complication in early surgery compared with 16patients in late surgery (P=0.061). Conclusion: EARLY CHOLECYSTECTOMY in first 72 h of ACUTE CHOLECYSTITIS has lower duration of disease and hospital admission and lower post operative complications compared with late cholecystitomy after 3 months.
