EPIDEMIOLOGY AND SURVIVAL OF WILMS TUMOR IN CHILDREN OF ISFAHAN CITY

Abstract

 Background and Aims: WILMS TUMOR.is the most common primary and malignant renal tumor during childhood, accounting for 5-6% of all types of cancers during this period. Today, the rate of survival has increased dramatically, partly due to large multicenter studies conducted by the national WILMS TUMOR.study group that have been resulted in new management methods with less morbidity. Since there is not enough information about epidemiological features and survival of these patients in Iran, this study was performed. Methods: This retrospective cohort study was conducted on all of the patients diagnosed as suffering from WILMS TUMOR.and were hospitalized and treated in medical training centers of Isfahan during 1986-2001. The initial data was collected from the hospital medical records, and then the patients were followed up for the complications, recurrence and mortality. Results: From 88 cases studied, 40 cases survived and 45.5% of the patients were male. The most prevalent clinical sign was abdominal mass (88.2%) and 91.1% of the patients had favorable and 8.9% had unfavorable histology .Twenty-three percent of the cases, were treated by surgery only, 50% by combined surgery as well as chemotherapy and 27% by a combination of surgery, chemotherapy and radiotherapy. Overall 4 years survival was 95% and 6 years survival was 32%. Conclusion: Because the SURVIVAL RATE particularly the long-term survival for the WILMS TUMOR.in our patients was lower than the other centers of the world (32% vs.70-90%), more researches and modifications of the current methods are necessary and establishing close communications between the surgeon, pathologist and oncologist is essential to increase the long-term SURVIVAL RATE.
