SIMULTANEOUS OPTIC NERVE CUT AND POSTERIOR SCLERECTOMY TOGETHER WITH HYDROXYAPATITE IMPLANATION: A NEW PROPOSED TECHNIQUE IN EVISCERATION

Abstract

 Purpose: To evaluate a new technique for evisceration with OPTIC NERVE CUT and POSTERIOR SCLERECTOMY surrounding the optic nerve together with HYDROXYAPATITE IMPLANTATION. Method: In this quasi-experimental clinical trial, all patients were operated under general anesthesia. Evisceration was performed together with two longitudinal incisions on the posterior sclera. Subsequently, the optic nerve and surrounding scleral tissues were cut in 8 mm diameter. Then a suitable hydroxyapatite prosthesis was implanted. Finally the sclera, tenon, and conjunctiva were repaired. All patients were followed up to drilling time. Results: Fourty two patients (30 males and 12 females) within an age range of 6 to 45 years (mean 22 years) were studied. Patients with suspected eye or orbital tumor were excluded. Conjunctival dehiscence less than 5 mm was seen in 4 cases (9.4%) and between 5 to 10 mm in 2 cases (4.7%). In the former group, operation was not necessary. In the latter group conjunctival repair was performed. No cases of secondary infection and hydroxyapatite extrusion were observed. Eye movement was cosmetically acceptable in all cases. Conclusion: Shorter duration of operation, less disruption of orbital anatomy, better prosthesis movement, and less surgical thread consumption are among the advantages of our new applied technique. Therefore this technique could be considered and recommended for patients without eye or orbital tumor and a small sclerotic sclera.
