SURGICAL MANAGEMENT OF RETINAL DETACHMENT (RD) IN HIGHLY MYOPIC EYES WITH MACULAR HOLE

Abstract

 Purpose: To evaluate the visual and anatomic outcomes of surgery for RD due to MACULAR HOLE in highly myopic eyes in patients referred to Labbafinejad Medical Center during 1993-2001. Methods: This descriptive study was conducted on existing data. The records of all patients with HIGH MYOPIA who underwent surgery for RD due to MACULAR HOLE were evaluated. Patients characteristics, best corrected visual acuity before surgery and at the last visit, surgical technique, anatomic success, and follow-up period were extracted. Results: We studied the records of 28 eyes of 27 patients. Most of the patients (26 patients) were female (96%), mean age of patients was 59.8 years and mean follow up was 17.3 months. Mean axial length of the eyes was 29.1±2.74 mm and the myopia was -16.4±3.1 diopters. Marked posterior staphyloma was detected in 71 % of patients. Anatomic success was achieved in 92.9% and 85.7% of the patients had improvement of vision. Three different surgical techniques were used in the first operation: 1) Intravitreal SF6 injection and anterior chamber tap in 43% of eyes, 2) pars plana vitrectomy in 32%, and 3) scleral buckling procedure in 25%. The rate of anatomical attachment with one operation was significantly better in the vitrectomy group than the others. In this series there was no recognized, intraoperative surgical complication. In 60% of the eyes the retina didn't attach with one operation and needed more surgery. Conclusion: In cases with RD due to MACULAR HOLE in highly myopic eyes, localized RD can be treated effectively by SF 6 injection in some patients. We suggest pars plana vitrectomy in complicated cases with posterior staphyloma and regarding the functional deficiency of the atrophic pigment epithelium we believe that a longer period of posterior tamponade with siliconee oil is important in these eyes.
