Abstract

Purpose: To determine visual and anatomical OUTCOME, COMPLICATIONs and prognostic factors of PARS PLANA VITRECTOMY for removal of intraocular foreign bodies in patients referring to Rasool Akram hospital during 1993-99. Material and methods: We retrospectively studied patients with intraocular foreign bodies who underwent PARS PLANA VITRECTOMY in order to removing intraocular foreign bodies and had at least 3 months square test. Results: We studied the records of 60 patients including 58 (96.7%) young men, a bout 38% of them were in their third decade of life. The antecedent event was hammeringin 40% of the patients, the site of penetrance was in the cornea in 48.3% and 58% of the cases had a single foreign body. In 77.2% of the patients the nature of the foreign body was iron. Mean size of fo reign bodies was 3.6 mm. The foreign body was located in vitreous in 51.7% and in the retina, between the equator and the vascular arcades in 31.7%. Vitreous hemorrhage was seen in 53.3% of the cases. In 51 (81.7%) patients, vitrectomy was performed within 3 months after penetration. In 13 (21.7%) cases primary intraocular lens was implanted. The most common intraoperative COMPLICATION was iatrogenic break (23.3%) and the most common postoperative COMPLICATIONs were retinal detachment associated with PVR (20%), and cataract (15%). Primary visual acuity was significantly associated with final acuity (p< 0.05). Retinal detachment associated with PVR and presence of relative afferent pupillary defect were predictors of poor final visual acuity (P<0.05). Primary intraocular lens implantation significantly improved final acuity (P<0.05). Conclusion: Consideration of prognostic factors can help the surgeon to predict the final visual acuity. The most important prognostic factors were primary visual acuity, presence of relative afferent pupillary defect, and retinal detachment associated with PVR. PIimary intraocular lens implantation, whenever indicated may improve visual rehabilitation.
