Abstract

Purpose: To report a case of POSTERIOR ISCHEMIC OPTIC NEUROPATHY (PION) after injection of hyaluronic acid as FILLING AGENT for correction of forehead wrinkles.Patient and findings: A 40-year-old man was referred by a dermatologist with sudden onset of inferior visual field defect in the left eye early after injection of hyaluronic acid in the forehead and glabellar area for correction of hyperfunctional skin lines. Visual acuity in the eye was 20/20 but afferent pupillary defect (APD) was present. On slit lamp examination and ophthalmoscopy no abnormal findings were seen. The optic nerve had normal appearance on the first examination. Humphrey perimetry revealed an altitudinal inferior visual field defect in the left eye. Brain MRI and fluorescein angiography were normal. After 3 weeks, necrosis of the skin around the site of injection appeared. After 5 weeks, subtle optic atrophy of left eye appeared. We diagnosed PION, which may be identified as a complication of various surgical procedures.Conclusion: Injection of FILLING AGENTs in the forehead and glabella may produce PION, in which the intraorbital portion of the optic nerve is involved. There is no treatment for PION, so it is important to prevent this complication.
