Abstract

Purpose: To evaluate the results of TRABECULOTOMY on children with PRIMARY CONGENITAL GLAUCOMA referred to Labbafinejad Medical Center, Tehran, Iran from 1995 to 1999.Method: The study was based on existing data and records of children who had undergone TRABECULOTOMY as the initial procedure. All the patients were thoroughly examined at the end of the study and data were analyzed by Kaplan Meyer survival analysis.Results: In this study, 122 eyes of 65 patients including 32 (48.2%) girls and 33 (50.8%) boys were examined. Age at presentation ranged from 1 day to 5 years (6.4±11.8 months). Follow-up lasted from 6 to 72 months (38.7±25.8). From 220 operations, 182 were trabeculotomies. Macroscopic hyphema in 16 cases (8.7%) was the most prevalent complication. Success rates after primary and repeat TRABECULOTOMY were 58.2% and 58%, respectively. Cumulative success rate for primary and repeated procedures was 79%. Duration of the effect of the first TRABECULOTOMY was 45.7 months (CI 95%, 38.3-53.2). Prognosis was better in the age group of one month to two years, but it was not statistically significant (P=0.31).Conclusion: TRABECULOTOMY is an effective operation in the management of PRIMARY CONGENITAL GLAUCOMA. Regarding the low prevalence of this disease, patients, especially high risk groups (age at presentation less than one month or more than two years) are best managed in qualified centers with enough experience and facilities. Further studies in this field for the sake of its importance are recommended.
