COMPARISON OF THE RESULTS OF DIFFERENT METHODS OF EMPYEMA TREATMENT

Abstract

 Which therapy should be adopted as the definitive treatment with alleviation of suffering with the minimum of unnecessary invasive methods:We reviewed the 579 patients treated in our department from Sep 1990 to Oct 1995. They included 236 cases of lung cancer, 53 case of bronchiectasis and 290 cases of nonsorgical EMPYEMA. 12 cases of lung cancer and 5 cases of bronchiectasis developed EMPYEMA postoperativly. There were 4 cases of EMPYEMA necessitates. There were 25 cases of bronchopleural fistula, and was diagnosed by persistant air leakage from chest tube. If bronchopleural fistula was not present, EMPYEMA cavity was vigorously irrigated with saline and appropriate antibiotics. If bronchopleural fistula was present irrigation was cautiously irrigated.After open window thoracostomy muscle plombage and omental flap was some times performed.EMPYEMA occurred in 10-12% of pneumonectomy. There were 85 females ans 222 males. EMPYEMA developed from 27 days to 4 months, after the initial thoracostomy. 25 cases had bronchopleural fistula. All EMPYEMAs without bronchoplemal fistula were treated with closed drainage and irrigation.The outcome was good. All 25 cases of bronchopleural fistula treated first with closed drainage and cautious irrigation, when EMPYEMA was diagnosed concurrently. Nine cases died. Nine cases required open window thoracostomy. Four cases required omentopexy. The other 3 cases treated with closed drainage and cautious irrigation.EMPYEMA with bronchopleural fistula needs further procedures, for example open window thoracostomy, omentopexy, bronchoscopic application of silver nitrite to the bronchial stump, thoracoplasty or other methods. implications: From clinical standpoint the next problem is which therapy should be adopted as the definitive treatment. open window. Thoracostomy or omentopexy? Open window thoracostomy is a widely accepted method and it's outcome is good.
