Abstract

Introduction: Cement is a substance with applications in various industries. It is composed of such elements as silicium and asbests. Workers at cement factories are potentially at risk of developing respiratory diseases due to the presence of various particles in air. The present study was conducted with respect to the presence of two cement factories in Isfahan and lack of any research studying the prevalence of respiratory signs and symptoms among workers of cement factories in this city.Methods: All the personnels of the factory in the year 1999(1038 people) comprised the subject, of this cross-sectional observation descriptive study. The test subjects were selected by a census, which was undertaken in the factory, and data collection was performed with questionnaires. Respiratory examinations were performed for all workers. Data was analyzed with SPSS software package ver 6.0 and p-trend were determined. Respective charts and graphs were drawn.Results: In this study prevalence of cough, dyspnea, sputum discharge, wheezing, ronchi and crackles was determined versus age, lenght of period they had been working in the factory and workplace. The commonest respiratory symptoms were coughing (33.7%), sputum discharge (21.8%) and dyspnea (1.9%). Respiratory sounds were normal in 90.9% of workers. There is significant linear relation between coughing for 3 months or longer and the age of workers (p < 0.05). There is also a significant linear relation between dyspnea and age (p<0.05).Discussion: Coughing in this study had a greater prevalence compared to foreign studies (18. 7%). Insignificant ventilation and inattention to hygienic principles as well as lack of workers' knowledge about pollutants might account for this difference. The prevalence of sputum discharge increased with age and length of period working at the factory. The reason is more frequent exposure to dust and cement particles. 1.9% of subjects had dyspnea, increasing with age which is lower then other countries (17. 5). The reason was that we only studied pulmonary dyspnea. It is suggested that more attention be given to work place hygenic in order to prevent occupational pulmonary diseases.
