Abstract

Introduction: Tumor progression is a multistep process, including loss of control of cell proliferation or cell death (apoptosis). The BCL2 ONCOPROTEIN plays an important role in carcinogenesis by inhibiting cell death (apoptosis). It was initially discovered in follicular B cell lymphoma with t (14, 18) and subsequently in other malignant lesions. The aim of this study was to investigate role of bcl2 immunostaining expression in colonic adenoma and carcinoma, and to identify any relationship between bcl2 marker and several HISTOPATHOLOGIC PARAMETERS.Methods: 100 cases consisting of 50 COLORECTAL CARCINOMA and 50 adenoma cases were selected randomly. 4µm sections of paraffin block of tumor were prepared. Then the slides were stained with anti bcl2 based on avidine-biotin-peroxidase complex method. The results were analyzed by SPSS. Results: The expression of bcl2 was consistently higher in adenoma than carcinoma. It was positive in 92% of adenoma and 40% of carcinoma cases (P=0.001).Also BCL2 ONCOPROTEIN expression in well-differentiated carcinoma was significantly higher than in moderately differentiated carcinoma. However, there was no correlation between bcl2 expression and other HISTOPATHOLOGIC PARAMETERS such as tumor stage, size and lymph node involvement. There was only a statistically significant correlation between bcl2 expression and size of adenoma.Discussion: Our results regarding the correlation of bcl2 expression in colon adenoma and carcinoma with tumor differentiation are similar to those obtained by most previous studies in this field. These results show the role of bcl2 in progression of adenoma to carcinoma, as well as a direct relation with tumor grade as an independent prognostic factor.
