A COMPARATIVE STUDY OF THREE METHODS OF PAIN RELIEF FOLLOWING APPENDECTOMY

Abstract

 Introduction: Pain is one of the most post-APPENDECTOMY complications which can enhance metabolic response to stress and create further problems. Pain prevention or relief via using methods such as laparoscopy, thoracoscopy, pharmacologic and non-pharmacologic methods has been the focus of several studies during recent years. This study was conducted to determine whether or not a single injection of the long-acting anesthetic can effectively reduce post-APPENDECTOMY pain.Materials and methods: In this prospective double-blind randomized clinical trial a consecutive series of 90 patients in three groups (30 patents in each group) undergoing APPENDECTOMY at Alzahra Hospital Isfahan from summer of 2000 until March 200I. The patients were allocated to the study groups using a table of random numbers. A dose of 1.5 mg/kg bupivacaine 0.5% was used. Group one patients received half the dose of bupivacaine subcutaneously and half the dose to the peritoneum. Group two patients received the entire dose of bupivacaine subcutaneously. Group three were treated as the standard group receiving morphine as analgesis for post-surgical pain. Pain score was assessed preoperatively and at 2.5,6, 12, and 24 hours postoperatively using Visual Analogue score. Time of administering the first analgesic (morphine) and the total dose of morphine required in the first 24 hours were recorded. One-way ANOVA method was used to analyze results using SPSS 11 software.Result: Receiving SUBCUTANEOUS BUPIVACAINE in combination with INTRAPERITONEAL BUPIVACAINE significantly reduced the experience of pain (P<0.0001).Discussion: This study demonstrated that a single injection of bupivacaine in the surgical area, combined with intraperitoneal and subcutaneous injections can significantly reduce pain and lower the need for opioid analgesics which have potential side-effects.
