Abstract

 Introduction: Bleeding is the third common cause of maternal mortality and morbidity. Placenta accreta is the most common cause of postpartum hemorrhage which leads to hysterectomy. The rate of placenta accreta can be decreased by controlling risk factors such as multiparity, age, history of cesarean, and history of curettage. The aim of this study was to determine the frequency distribution of pregnant women with placenta accreta according to risk factors (cesarean, metroplasty and curettage) between 1991 and 2001, in some hospitals in Isfahan.Methods: This is a descriptive study. The cases were collected systematically. The population under study comprised all patients admitted to Alzahra, Beheshti, Sadooghi, Amin, and Sepahan Hospitals in the city of Isfahan with the diagnosis of placenta accreta, between 1991 and 2001. Patient information was extracted from hospital archives. A total of 370 cases were included in the study and the required information was obtained using checklists.Results: The prevalence of placenta accreta was 0.23%. Most of the patients with placenta accreta were between 30 and 35 years of age (25.6%). Among these, the majority were in their 34th _38th weeks of pregnancy (39%). Placenta accreta was more common in women with parity of 7 (17.5%). 28.3% of patients had a history of cesarean, 33% had history of curettage, 22% had history of metroplasty, 35% had concomitant placenta previa, 6.7% of had a platelet counts less than 100000, and 29.1% had prolonged PT-PTT. Low fibrinogen levels (200 mg/d) were found in 30.2% of the patients. 24.3% of the patients had no transfusion and 6.7% received packed red blood cell. Mortality rate was 5.6% and 60.9% of the patients underwent hysterectomy.Discussion: The prevalence of placenta accreta in Iran is higher than global statistics, which may be accounted for by higher rates of parity, cesarean sections, curettage and metroplasty. The incidence of placenta accreta with history of cesarean section or curettage in Iran is higher than figures mentioned in references; hence effort should be made to reduce risk factors, i.e. unnecessary cesarean sections and curettage.
