A STUDY OF THE DRUGS USED IN TREATMENT OF HYPERTENSION IN THE CENTRAL AREA OF IRAN

Abstract

Introduction: Hypertension is the most prevalent chronic disease encountered by physicians. Its prevalence has been reported to be 18% in Isfahan province. Its control can bring about a 50% decrease in stroke events, and a 15% decrease in coronary artery diseases. The rate of blood pressure control is 12.5% according to the World Health Organization (WHO). Effective chain of blood pressure control consists of different stages, namely screening, diagnosis, changing lifestyle, drug therapy, etc. The type of drug used by patients with hypertension is of central importance in effective control of hypertension. The present study was performed to determine the type of drugs used by hypertensive patients in urban and rural areas of Isfahan, Najaf-Abad and Arak. Methods: This descriptive cross-sectional study was performed in three cities, namely Isfahan, Najaf-Abad and Arak, in 2000-2001, along with Isfahan Healthy Heart Program (IHHP). Cluster sampling consisted of 10674 samples in urban and rural areas of the studied cities. The patients had systolic blood pressure?: 140 mmHg or diastolic blood pressure?: 90 mmHg based on standard measurement of blood pressure, or received antihypertensive medications. Data were entered in EPI5 and analyzed with Hest using SPSS software.Results: A total of2015 people were afflicted by hypertension (18.9%). Knowledge, treatment and control of blood pressure in patients with hypertension were 47%, 23% and 9%, respectively. Of 670 patients under treatment, 527 had been treated with one drug, 133 used two drugs with 35 different combinations, and 30 patients used three drugs with 25 different combinations. In single drug use, the prevalent primary option was beta blockers (23%), and the most prevalent drug regimens included atenolol and methyldopa. Frequency of diuretics was 0.9%. Non-appropriate combinations such as combined use of atenolol and propranolol (9 patients) were seen among patients receiving two-drug and three-drug combinations.Discussion: The present study shows that drug used by hypertensive patients did not match the recommended regimen in most of the hypertensive patients. Hence, not only is hypertension not controlled properly, but also there is an increase in drug side-effects and a decrease in patient compliance. The results of this study show the importance of training physicians, health care workers and patients.
