Abstract

Introduction: Dialysis adequacy shown by Urea Reduction Ratio (URR) has been established as a determinant of morbidity and mortality in hemodialysis patients. Vascular Access, hemodialysis machine and dialyzer are essential components of hemodialysis. We studied the effects of these factors on hemodialysis adequacy. Methods: This clinical trial study was performed in Noor Medical Center where 34 patients under chronic dialysis were examined. Seventeen of these patients had permanent catheter and 17 had Arterio-Venous Fistula. Each group of patients was dialyzed with two types of machines and the adequacy of dialysis was compared between them. Results: In this study mean URR was 57.21±9.32 percent. Comparison of dialysis adequacy between patients with AVF and those with permanent catheter did not show any statistically significant differences. Our findings showed that in patients with AVF, dialysis machines affected dialysis adequacy (P-value=0.014), while no such effect was seen in patients with catheter.Discussion: We found that AVF and permanent catheter had no significantly different effects on dialysis adequacy; hence dialysis can be continued in patients with catheter until maturation of AVF without any significant effect on dialysis adequacy. Also types of dialysis machines in patients with AVF, but not those with permanent catheter, had a significant effect on dialysis adequacy.
