FREQUENCY OF MULTIPLE INTRACRANIAL ANEURYSM IN THE PATIENTS WHO UNDERWENT ANGIOGRAPHIC STUDY OF CEREBRAL ARTERIES

Abstract

 Introcuction. Regardless of brain damages caused by trauma, which is the most common cause of various kinds of intracranial bleedings, subarachnoid hemorrhage includes 1/3 to 1/2 of spontaneous intracranial bleeding cases. The most common cause of spontaneous subarachnoid hemorrhage is arterial aneurysm which includes 70% to 80% of cases, vascular malformations stand on the second place of frequency. Today, CT scan is the first step and diagnostic method of choice in investigation of patients suspicious to subarachnoid hemorrhage. Despite of considerable improvements in imaging of cerebral vascularity and innovation of methods such as, CT Angiography and M.R Angiography, cathater angiography is still considered as the most accurate diagnostic method for cerebral vascular lesions. Obviousy according to the previous studies, the probability of multiple intracranial aneurysms was reported from 2% to 44.9% in the affected patients.Methods. From this point of view, among the group of patients who underwent angiographic investigation due to various types of intracranial hemorrhages, one hundred patients with aneurysm documented by angiography were selected and the probability of multiple aneurysm was investigated. Angiography was performed with DSA method on all the patients.Results. In this study it was apparent that from the group of patients with cerbral vascular aneurysm, multiplicity was seen in 11% of casesand in 3% of patients aneurysm and vascular malformations were seen concomitantly. Most of the single aneurysms were seen in female patients (57%) and multiple aneurysms were more common in the males (54% comparing to 46%). The most common site of single aneurysm was in anterior communicating artery (27%). In 18% of patients with multiple aneurysms more than two aneurysms were seen. The highest count of aneurysm in on patient was four.In 63.5% of patients with multiple aneurysms, the lesions were located in one of the brain hemispheres (unilateral). The most common site of multiple aneurysms were in anterior communicating artery and internal carotid artery before bifurcation.Discussion. Considering mentioned findings; multipliCity of intracranial aneurysm does not show unusual incidence in comparison with other studies obviously this frequency is significantly lesser than some of the reports. Male preponderance of multiple lesion was detected in this study; it is in contradiction with the most similar researchs.
