EVALUATION OF RELATION BETWEEN CARDIAC DISEASE AND ANTI CARDIOLIPIN ANTIBODY IN INACTIVE LUPUS PATIENTS

Abstract

 Introduction. Propably about 25 percent of patients with systemic lupus erythematosis (SLE) have cardiac involvement. Pericardial involvement is the most common complication but myocardial, endocardial and vasculitis are reported. It is unclear whether cardiac abnormalities are significantly more frequent in patients with elevated titres of antiphospholid antibodies. About 20 percent of patients with SLE have elevated levels of APL antibodies. Most sensitive antibody in APL Ab group is anticardiolipin antibody. In this study the relation between ACLAb with cardiovascular abnormalities of inactive phased lupus patients were evaluated.Methods. 100 patients with stable lupus were selected. 45 patients had inculsion criteria for study. The exclusion criteria were past medical history of hypertension, diabetes, smoking, hyperlipidemia, rhumatic or congenital heart disease. All patients were in inactive phase of disease. Cardiovascular evaluation of endocardial, myocardial, pericardial and full hemodynamic study were performed by trans thoracic echocardiography. In the same time, ACL of all patients were assessed by refference laboratory. All data were collected in data bank of SPSS10 under windows 2000, all results were analyzed by t-test and chi-square with 95 percent confidence interval.Results. 23 and 22 patients had IgG ACLAb positive and negative respectively, SLE patients with positive IgGACL in mitral regurgitation, MR and thickening of mitral valve, vegatation, pericardial effusion, high pulmonary artery pressure, high titer of anti DNA double stranded and recurrent abortion had significant statistical differences with IgGACL Ab group (P<0.005). 15 and 30 patients were IgM ACLAb positive and negative respectively. SLE patients with positive IgMACL Ab in letf ventricular mass index (LVMI), stroke- volume, MR and thickened mitral valve, high PA pressure, high titer of Anti DNA double stranded and recurrent abortion had siginificant statistical differences with negative IgMACL Ab (P <0.005).Discussion. High titers of IgG and IgM ACLAb in lupus patients may be associated with increase risk abnormality of one or three layers of the heart and high PA presure.
