CONGENITAL DISLOCATION OF THE HIP IN NEWBORNS OF MASHHAD CITY

Abstract

 Developmental dysplasia of the hip (DDH) is one of the common congenital malformations, which were seen in all of world with , different frequency in respect to race and geographic distribution. Our objective of the present study was determining incidence and value of click in physical examination and predisposing factor relation to CDH. In a cross sectional study, over nine months in 1999, all of neonates were born in four hospitals was examined at 48 hours of age by ortolani test .For all NEWBORNs with unstable joint data collected regarding maternal history. If clunk felt at first examination or click on repeated examination, for accurate diagnosis soqography was done. Infant with definite CDH or unstable hip referred to orthopedic clinician.A total number of 6576 NEWBORNs were examined after birth over nine months. Clunk or click was found in 3% or 197 neonates. Only 10 NEWBORNs had CDH by clinical and sonography finding. Of these eight infants had clunk at birth and two NEWBORNs with persistent click at repeated examination finally diagnosed as CDH. Breech delivery was present in 4 % or four neonates. CDH was bilateral in 60 %, with left side only in 30 % patients. Incidence congenital dislocation of the hipin this study was 1.5/1000 NEWBORNs. These were significant association between first parity and CDH (P<0.05), but not with maternal age. CDH- incidences in Iranian NEWBORNs (1.5/1 000) are similar to other studies reported in literature. Many of unstable hips on repeated examination were without abnormality and click is unvalued in diagnosis of CDH.
